Abstract: 

Background and Aim: Alveolar ridge reduction after tooth extraction could be minimized via ridge preservation by using grafting material. The aim of this study was to compare OSSEO+ (allograft, made by IMTEC Corporation) with Cano Bone (allograft, made by Tissue Regeneration Corporation) in preservation and bone formation of the alveolar ridge after tooth extraction.Materials & Methods: This randomized double blind clinical trial was accomplished on 20 patients. Subjects were divided into two groups of 10 each with respect to similarity in age, gender, jaw being treated and cigarette smoking habits. The effect of each material was studied clinically and histologically. Horizontal and vertical ridge dimensions were determined immediately after tooth extraction and three months later before implant placement. Three months after application of each material, histologic analysis of the trephine cores was done to determine the percentage of vital, non-vital, trabecular, amorphous bones and number of osteoblasts. Statistical analysis was done using Repeated Measure ANOVA .T-test was used for statistical analysis of bone formation.Results: In both groups minimum reduction in height and width of alveolar ridge was observed with no statistically significant difference. The extent of vital, non-vital, trabecular and amorphous bones wasn’t statistically significant. The amount of osteoblasts was significantly higher in CenoBone group (6.98±1.57) than in OSSEO+ group (4.88±1.56) (P<0/05).Conclusion: Both materials were similar in preservation of alveolar ridge height and width. The extent of vital, non-vital, trabecular and amorphous bones in both groups were similar. It should is said that according to the present findings, foreign sample of DFDBA isn’t better than Iranian sample. Only the number of osteoblasts in CenoBone group was higher.

Abstract: 

Background and Aim: Adherence and growth of Candida albicans denture soft lining materials can result in clinical problems, and deteriorations of the materials. Prevalence of denture stomatitis in association with Candida albicans has been reported to range between 11%-67%.The aim of this study was to evaluate the adherence and growth of Candida albicanson two type of denture soft liners (GC and Acropars) and comparison to control group(Acropars acryl).Materials and Methods: Twenty-two specimens of each soft lining material and 10 acrylic specimens (polished and not polished) were made and incubated in a suspension of Candida albicans. After rinsing, the specimens were stained with acridine orange and examined under fluorescent microscope. For statistical analysis One-way ANOVA and Post Hoc, Tukey HSD tests were used.Results: The highest and the least adhesion and growth were found in the not polished and the polished groups respectively. Also, adherence and growth on Acropars soft liner was significantly more than GC soft liner (p<0.05). Significant difference was found between polished and not polished acryl (p<0.05).Conclusion: According to results, precise attention in selection of correct material with the lowest side effects and its production steps seems mandatory.

Abstract: 

Background and Aim: Approximately 500 bacteria species inhibit in periodontal pockets. Association between Aggre gatibacter Actino my cetem comitans and destructive and progressive forms of periodontitis has been demonstrated. The aim of this study was to evaluate the microbiological and clinical effects of metronidazole-ciprofloxacin therapy in subjects with periodontitis.Materials and Methods: In this experimental randomized double-blind controlled clinical trial, 24 patients with at least 4 sites with ³4mm of clinical attachment loss, bleeding on probing and detection of A.a were included. Measurement of Clinical attachment loss, gingival index, plaque index and bleeding on probing were done. All the patients received scaling/root planning and oral hygiene instruction. The patients received drug and placebo. Bacterial culturing and measurement of Clinical attachment. Attachment loss, gingival index, plaque index and bleeding on probing were done at the 10days, 3 and 6 months later. Data were subjected to Paired t-test and Mann-u-Whitney test for data analysis.Results: Bleeding on probing, Gingival Index  and A.a Colony count on 3rd and 6th month in control and test group were (33/337 و /5)Vs (0), (1/2±0/46-1/21±0/42 Vs(0/06±0/11-0/10±0/13), (9/15±10/88-12/56±11/35)Vs(0/75±0/99 -1/96±2/15) respectively. These differences were statistically significant (p<0/05). No significant difference was seen between two groups in Clinical attachment. Attachment loss and plaque index in all studied periods also gingival index, colony count in10 days showed no significant difference.Conclusion: Administration of Ciprofloxacin plus Metronidazole as an adjunct to mechanical therapy can provide effective periodontal healing and Aa eradication from periodontal tissue.

Abstract: 

Background and Aim: Patient satisfaction is one of the most important factors to improve the quality of educational centers thus increasing patient referral and promote practical training of the students. To now in this faculty no such study has been done, so this research aimed to evaluate patients’ satisfaction from dental care given in different departments of Tehran faculty of dentistry, Islamic Azad University year1387.Materials and Methods: In this cross sectional study 1500 subjects who referred consequently were selected from clinics with the same proportional ratio of patient referral to every clinic. After explanation of project to every subject through interview, satisfaction was estimated by 13 questions about distant way, waiting time, treatment duration ….with a range of 3 options of completely satisfied, relatively satisfied, dissatisfied and ranking of 0, 1, and 2. Total rank less than 1/3 was known as dissatisfied, between 1/3 – 2/3 as relatively satisfied, and higher than 2/3 as satisfied. The role of related factors was subjected to chi-square statistics. Results:71%  of all subjects spoke out satisfaction, 27.5% were relatively satisfied, and 1.5% dissatisfied. The prevalence of dissatisfaction in the population was estimated from 26.7% to 31.3%.The most dissatisfaction was related to removable denture patients:64.7% and the least was in oral surgery clinic:13.5% (p<0.05). The most dissatisfaction factor was long distance and waiting room ambiance but the least dissatisfaction was about student behavior, hygiene and professors behavior.Conclusion: The satisfaction level of patients in Tehran dental faculty of Islamic Azad University was acceptable but according to presence of dissatisfaction in patients and its consequences, appropriate improvements are recommended.

Abstract: 

Back ground and Aim: Micro leakage and shear bond strength in composite restorations are the most problematic challenges to days. There is disagreement about bond strength in the literature. On that basis this invitro study was designed to evaluate micro leakage and shear bond strength of three bonding, Adhese, Prompt L-pop, Clearfield SE bond, in composite cavities. Materials and methods: This experimental study was performed on 60 freshly extracted non carious human premolars. 30 samples were used to test micro leakage and 30 for shear bond strength. Class V cavities were prepared in buccal surface of 30teeth with dimensions of 2´2´3 mm so that the gingival margins located 1 mm below the CEJ and occlusal enamel margins beveled to 0.5 mm. Samples were randomly divided in to 3 groups of 10 teeth each; group A: Clear fill SE bond, Group B: Prompt L pop, Group C: Adhese. Cavities were then filled with Z100 (3M, ESPE, USA). 30 other samples were grounded up to 1mm below the central groove to expose fresh dentin, and then composite tubes were bonded to dentin according to bonding strategy of the group. Shear bond strength was evaluated with Zwick (Roel, Germany) testing machine and micro leakage with stereomicroscope. Micro leakage was recorded and Kruskal-Wallis analysis was performed followed by Mann-U-Whitney tests. Statistical differences in shear bond strength of tested bonding were determined by ANOVA followed by Tukey test.Results: There was a significant difference in regard to micro leakage between the studied groups (p<0.005) with the minimum leakage in SE bond group (75/0±1) and the maximum in Prompt-L-pop group (15/1±15/2). Shear bond strength analysis revealed statistical significant differences between Group A and the 2 others with the superiority of Group A (p<0.001).Conclusion: With the limitation of this in vitro study we concluded that :Among 3 self etching dentin bonding agents used in this study Clear fill SE bond showed the highest level of shear bond strength and Among 3 self etching dentin bonding agents used in this study Clear fill SE bond showed the least micro leakage.

Abstract: 

Background and Aim: Finding an accurate and inexpensive method in measurement of bone width for determination of implant size is critical. Linear tomography is a cost effective method and there is no accurate finding about accuracy of linear tomography in evaluation of mandibular wide .The purpose of this study was to determine the accuracy of linear tomography in estimation of mandibular width.Materials and Methods: In this diagnostic study (estimation of one method), 23 sites of four dry mandibles were selected and marked at the crest with a metal ball markers. After linear tomography in these sites, the width of mandible in the superior border of inferior alveolar canal was measured. The mandibles were then sectioned. The Pearson's rank order correlation coefficient between linear tomography and real values was determined. Then with estimation of mean absolute differences by magnification factor, the percentages of errors in ±1mm error limits were determined. The regression equation was written for better determination of agreement rate between radiographic and real values. The percentages of errors in ±1 mm error limits were determined.Results: The Pearson's rank order correlation coefficient between the linear tomography and real values was 0.813 (P<0.0001). The mean absolute differences between the mandibular width in tomographic sections and real values was 0.3 mm (SD=1.13) and 56.5% of width measurements were within the ±1mm error limits. By the resulting linear regression equation 51.8% of tomographic values were located within the ±1mm error limits.Conclusion: Linear tomography in evaluation of mandibular width should be used more cautiously.

Abstract: 

Background and Aim: One of the challenges of restoring endodontically treated teeth with fiber reinforced composite (FRC) posts is their deboning from root canal dentin. Different factors such as cement type, affect the post retention. According to the controversies in this regard, this study aimed to evaluate the effect of three luting cements on retention of FRC posts in the root canal.Materials and Methods: In this in-vitro study 30 selected root canal treated premolar teeth were randomly allocated to three experimental groups. After post space preparation, RTD fiber posts, with 10mm length and ¼ mm diameter, were cemented in the canal with one of the following luting agents: zinc phosphate(Harvard), Glass-Ionomer(GC) and resin cement Maxcem(Kerr). They were then fixed and force was applied using Instron 5500 machine. The Data was subjected to ANOVA for statistical analysis.Results: The observed retention in studied groups was as follows: resin cement 127±18.5 (MPa), zinc phosphate 102±29.7 (MPa) and glass-ionomer 51±14.8 (MPa).Conclusion: In order to increase the retention of FRC posts, cementation with resin cements is recommended.

Abstract: 

Background and Aim: Tooth shade selection has long been a challenge in dentistry. According to huge number of edentulous people, in need for prosthetic treatment, determination of natural teeth shade distribution is necessary. Based on deficiencies in regard to prevalence of tooth shade and their matching shade guides, this study was designed to compare upper central incisors shade with three different shade guides. Methods and Materials: This cross-sectional study was conducted on 120 maxillary central incisors of dental students, all over 18 years old. Census of the students was performed to complete sampling. After stain and plaque removal two observers (a dental student and a prosthodontic) determined the shade of upper right or left incisor visually and then by aid of three different shade guides (Polident, Ivoclar Chromascop, Vitapan classic). In order to compare the chosen shades by the two observers and also to with the shade guides kappa coefficient was used.Results: 58.3 % of the samples were female and 41.7% were male. The average age was 24±3.5 years with minimum 19 and maximum 41 years. The highest rank reported by the two observers in Vitaclassic shade was A hue (prevalence: 77-78%) followed by A2 (42.9%, 45.9%) and A1 (23.5%, 25%). Hue C had the least prevalence (1.2%). Shade selection by Ivoclar chromascop shade guide revealed that the color (120) and the color (110) had the greatest prevalence (44.2% and 41.7% respectively). In Polident shade guide group, the color (Y1) had the highest prevalence (71.7%).Conclusion: According to the findings distribution of natural teeth shade is higher in Vitaclassic shade guide which makes it a more practical guide. The shades in Ivoclar and Polident guides were found in smaller proportion of natural teeth determining their lower matching to natural teeth.

Abstract: 

Background and Aim: Treatment is successful when the patient refers on time and the disease is recognized soon. On the other hand, evaluation of the most common and important diagnostic-treatment needs in every center has an important role in program planning. The aim of this study was to determine diagnostic needs and performed treatments in different departments of Dental School, Shahid Beheshti University on 2007-2008 Materials and Methods: This research was designed to evaluate the existing data. 2000 records (written- computerized) of the patients, referred to the Oral Medicine department of dental school, on 30 active days were randomly selected and studied. Also 150 records were randomly selected to evaluate the causes of seeking dental services. The data was analyzed using chi-square test. Results: This study was accomplished on 2000 patient records of which 53/6% were female and 46/4% were male. Samples age was 31/6±16/2. The most common complaints were toothache (31%), tooth restoration (19/5%), checkup (9/2%), tooth replacement (8%) and periodontal diseases (6/9%). Diagnostic- treatment needs were as follows: tooth restoration (71%) radiography (69/5%), periodontal treatment(56/6%), endodontic treatment (52/2%) surgery (38%), fixed prosthesis (12/9%) orthodontics (5/4%). Maximum referral was to radiology (97/3%) and minimum was to removable prosthodontic department (54/4%). The maximum performed diagnostic- treatment procedures were accomplished in radiology (36%) and operative (15%) and the minimum were in pathology and orthodontic (1/4%) departments.Conclusion: Needs in patients referring to Oral Medicine department were more than their requests but they have not returned to complete their treatments on a regular basis.

Abstract: 

Background: Inverted Papilloma is the most common form of sinonasal Papilloma and accounts for 50-78% of sinonasal Papilloma and estimated to represent 0.5-4% of sinonasal tumors, with greatest potential for local invasion and malignant transformation. Recurrence rate of the disease is about 14 to 75% depending on the treatment modality. It is associated with malignancies usually squamous cell carcinoma in 3 to 24% of cases. Case Report: The patient is a 43-year old man with chief complaint of trismus and pus drainage on his cheek from six months ago who came to Taleghani hospital in Tehran. Radiographic examinations revealed a mass in his right maxillary sinus involving the medial and lateral sinus walls but the orbital floor was intact. And cortical perforation was seen in Water's radiographic and CT scan. Incision biopsy was done from the palatal ulcer. According to histopathologic features we diagnosed a squamous cell carcinoma on a previous inverted Papilloma.Conclusion: Inverted papilloma has a great potential of recurrence and malignant transformation so a complete surgery and prolonged follow up is highly recommended. In this article patient didn't agree complete surgical removal lesion and attending follow up sessions.
