Abstract: 

Introduction: Nursing occupation is considered as a major source of stress which leads to anxiety, depression and procrastination. In this regard, the hospital environment is one of the factors that influence the tension. The present study aims to compare the anxiety, depression and procrastination among nurses of general and psychiatric hospitals of Shiraz city.Methods: The research method is descriptive-comparative approach. The population consists of all nurses of the psychiatric and general hospitals in Shiraz city. The research sample consisted of 126 nurses (63 from psychiatric hospitals and 63 from general hospitals) who are selected using accessible sampling method. Data collection instruments consisted of “General Health Questionnaires” and “Tuckman Procrastination Scale” (TPS) that their validity and reliability are determined in previous study. Data is analyzed by SPSS.19.Results: There is no significant difference in depression and anxiety between general and psychiatric hospitals’ nurses (P=0.418), however, the value of procrastination in general hospitals’nurses is significantly higher than the one of psychiatric hospitals’ nurses (P<0.05).Conclusions: Both groups of nurses have equal anxiety and depression. Therefore, training course in this regard, to increase quality of care is recommended.

Abstract: 

Introduction: Cancer and its treatments affect the quality of life of those who are suffered from breast cancer. This study aims to compare the quality of life in women survived with those suffered from breast cancer while under chemotherapy in 1394-1395.Methods: In this descriptive-comparative study is conducted on 40 women survived breast cancer with the history of chemotherapy with 40 women who have cancer while under chemotherapy. Each of these cases was matched for age, sex, education, marital status and job. A sample of respondents is selected from Shohdaye Tajrish Hospital, Tehran through purposive sampling. The instruments consist “Depression, Anxiety, Stress Scales” (DASS)” and “The EuropeanOrganization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30)”. The validity and reliability are measured in previous study. Data is analyzed by SPSS.22.Results: Despite the significant difference and the relative increase of scores of functional components like physical performance, role-play and social performance in women survived breast cancer (P<0.05). Important components like cognitive emotional performance and overall components of life quality shows no significant difference in signed components (P>0.05). Also the study shows that women survived breast cancer in dyspnea, constipation diarrhea, and the economy position are just like their matches under treatment (P>0.05).Conclusions: The quality of life in women survived and those suffered from breast cancer while under chemotherapy is low. Therefore, it is recommended that the quality of life of women survived and woman under chemotherapy can be improved by supportive and psychological approaches.

Abstract: 

Introduction: Self-efficacy and social support are two important factors that influence mental health of teachers and enables them to tolerate stressful situations. The aim of this study is to predict the mental health of teachers through self-efficacy, social support and socio-economic status variables.Methods: The population of this descriptive-correlational study consists of all elementary and secondary school teachers in Tehran city.447 teachers with cluster random sampling are selected.To collect data, “General Health Questionnaire” (28- GHQ), "General Self-efficacy Questionnaire", "Social Support Questionnaire" and demographic form are used. The collected data are analyzed by SPSS.22.Results: Results show a negative significant correlation between self-efficacy, social support with physical symptoms, anxiety and insomnia, social dysfunction and depression. Therefore, selfefficacy and social support can predict changes in physical symptoms, anxiety and insomnia, social dysfunction and depression among teachers (P<0.001). Moreover, between housing status and salary there is a significant negative correlation with anxiety and insomnia, so they can significantly predict anxiety and insomnia among teachers (P˂0.05).Conclusions: Negative correlation between self-efficacy and social support with physical symptoms, anxiety and insomnia, social dysfunction and depression show that those variables could predict psychological disorders among teachers. Therefore, the higher self-efficacy and social support among teachers, better mental health. In order to increase the self-efficacy of teachers, it is recommended to provide social support and promote their socio-economic status. This will lead to mental health improvement.

Abstract: 

Introduction: Restenosis after stenting constitutes is one of the major concerns among cardiologists’ experts. This may lead to repetition of angioplasty, coronary artery bypass graft surgery, myocardial infarction, and even death. The present study, by using Cox's regression model aims at determine the demographic and clinical factors affecting time to incidence of cardiovascular restenosis in patients’ undergone angioplasty in Zanjan, Iran.Methods: In this descriptive longitudinal study, 421 patients who visited Zanjan's Ayatollah Mousavi hospital for drug eluting stent implantation between April 2009 and June 2011 are scrutinized with respect to the time of the incidence of restenosis. Next, demographic variables and patients' clinical records are entered into “Data Gathering Form of Patients’ Undergone Angioplasty”. Data is analyzed using Cox's regression and SPSS 21.Results: The mean time of restenosis incidence is 736 days and the rate of restenosis incidence is 30.4%. Hazard ratio of restenosis in unemployed patients is 1.39 times, in drug addicted patients 1.75 times, in patients with a history of hyperlipidemia 1.57 times, and in patients with chronic kidney disease is 1.58 times more than other patients (P<0.05).Conclusions: This study indicated that the most important factors leading to the incidence of restenosis are unemployment, drug addiction and abuse, suffering from hyperlipidemia, and chronic kidney diseases. Accordingly, it is highly recommended to prevent patients from above-mentioned risk factors and plan a periodic and continuous follow-up for patients with any of these factors.

Abstract: 

Introduction: Improving the quality of services in hospitals and dealing with the health of people are very important. The aim of the present study is to evaluate and rank the quality of service in four public hospitals in Iran through “Multi-criteria Decision Making Methods” in 2016.Methods: In this descriptive study, a sample of 8 faculty members from both Semnan University and Isfahan University of Medical Science and a random sample of 384 patients from four public hospitals in Isfahan are calculated by using Cochran method with 0.05 error.Two “Pairwise Comparison Questionnaire” and “Decision Matrix Questionnaire” are used to weight the identified criteria and determine the score of each hospital in each criterion, respectively. Cronbach's alpha of the first questionnaire is approved by 10 experts and the second questionnaire by patients. The hospitals are ranked, using two combined methods: 1- Analytical Hierarchy Process and TOPSIS, named AHP-TOPSIS, and 2- Modified Digital Logic and TOPSIS, named MDL-TOPSIS. Data is analyzed by Expert Choice.11.Results: Based on considered error level of 5%, results of both AHP-TOPSIS and MDL-TOPSIS methods, with closeness index values of 0.893 and 0.873 indicated that Al Zahra hospital provides the best quality of services.Conclusions: The Al Zahra hospital has acceptable performance rather than other hospitals. It is suggested that MDL-TOPSIS and AHP-TOPSIS methods be applied to all hospitals in the country in order to identify the strengths and weaknesses so that hospitals recognize their overall status among the other hospitals compared.

Abstract: 

Introduction: Exercises may alter the structure and function of the heart. Achieving the optimum performance of endurance activity depends creates compatibility in different body systems especially in the heart. The purpose of this study is to compare left ventricular structure of young taekwondo athletes’ men and non-athlete ones of Qazvin province.Methods: In this descriptive-comparative study, 15 young and adult athletes are randomly selected and 15 non-athletes are selected by convenience sampling method. Echocardiograph machines Kontron with two-dimensional method, M- mode and Simpson measurement are used to investigate the left ventricular structure of the subject. Validity of Echocardiographic device is examined through a survey completed by cardiologist and reliability of the device is measured by the Pearson Correlation Coefficient P=0.91. Data are analyzed using SPSS.18.Results: The indexes of LVPWD, LVPWS, IVSD, IVSS and LVM (P=0.001) of left ventricular of athletes are significantly higher than the one of non-athletes. But in the variables of LVEDV (P =0.022) and LVESV (P=0.023) there is no significant difference within two group.Conclusions: The thickness of the walls and left ventricular mass of taekwondo athletes increased but dimensions did not change. Hence, it is suggested that coaches practice aerobic exercises and increase cardio respiratory fitness and left ventricular capacity such as taekwondo through annual examination of athletes' heart structure.

Abstract: 

Introduction: Emotional divorce is an abnormality in family system that transforms couples into marriages that lack love, companionship and friendship. The aim of this study is to determine the association between child maltreatment and emotional divorce by mediating roles of attachment styles, early maladaptive schemas and difficulties in emotion regulation.Methods: This is a descriptive-correlational study. The statistical population consists of all married women and men in Tehran in 2016.150 married couple (75 married women and 75 married men) who reside in Tehran in 2016 are selected by Purposive sampling method. The data is collected by“Young Schema Questionnaire Short Form”, “Child Abuse and Trauma Scale”, “Adult Attachment Scale”, and “Difficulties in Emotion Regulation Scale” questionnaires. Collected data is analyzed using statistical methods (i.e. Pearson's correlation coefficient and path analysis) and SPSS.24 and LISREL analysis softwares.Results: Results show a significant positive correlation between child maltreatment and disconnection/rejection schema domain (P<0.01, r=0.394, B=0.35, t=4.04). In addition, it reveals a positive correlation between this variable and emotional divorce via mediating role of early maladaptive schemas (P<0.01, r=0.319, B=0.154, P=0.0007). There is a positive correlation between child maltreatment and difficulties in emotion regulation through mediating role of early maladaptive schemas (P<0.01, r=0.534, B=0.175, P=0.0001).There is a positive correlation between insecure attachment styles and emotional divorce via mediating role of difficulties in emotion regulation (P<0.01, r=0.261, B=0.08, P=0.02).There is a positive correlation between difficulties in emotion regulation and emotional divorce (P<0.01, r=478, B=0.23, t=2.71).The model of present research was approved (AGFI=0.99, CFI=0.96, NFI=0.96).Conclusions: Child maltreatment is directly related to early maladaptive schemas and indirectly related to emotional divorce with mediating roles of early maladaptive schemas and difficulties in emotion regulation. Child maltreatment causes insecure attachment styles, early maladaptive schemas and difficulties in emotion regulation to be formed. Therefore, it is recommended that training programs could facilitate the prevention of interpersonal problems and marital dissatisfaction in families.

Abstract: 

Introduction: Managers as main decision-makers play a crucial role in success or failure of an organization with respect to internal and external issues. Thus, some dimensions and components are necessary to select the qualified managers for the position. In this respect, this study aims to develop a competency model for hospital managers.Methods: This descriptive study is conducted in 2016. At the initial stage, studies on competency concept were studies and 46 components are sent to 30 experts of the Delphi panel. The first group of the panel experts consist of expert faculty members in the field of hospital managers’ competency.The second group are hospital managers of Tehran city whom were experienced in the field of health management and hospital. According to the results of the first phase of the Delphi panel, in the second round 36 components are sent to the expert panel. The components of the third round is resubmitted to the experts without modification. The data is analyzed using SPSS.24.Results: Based on the results, the competency model is developed. The model consists of three individual, technical and interactive dimensions. The individual dimension has seven components, the technical section consists of 25 components and the interactive part has four components.Conclusions: The developed competency model for hospital managers consists of three dimensions.It is recommended to use the model to select suitable hospital managers.
