Abstract: 

Background and Objectives: Artificial airway suctioning is an important intervention to take care of mechanically ventilated patients. The aim of this research was to determine the effect of artificial airway suction based on the comprehensive criteria for suctioning on the patients' hemodynamic status in ICU.Materials and Methods: This study was a randomized clinical trial by before-after design with one control 60 group. mechanically ventilated patients admitted to the ICU were selected by convenience sampling and were assigned into two 30-member groups using a block random allocation method. To collect the data, we used demographic and hemodynamic status sheet. In experimental and control groups the need for suctioning was determined on the basis of comprehensive and common suction standards respectively. Hemodynamic status was determined before suctioning and 2 and 5 minutes after suctioning. The data were analyzed by SPSS 17 software.Results: Compare the mean of arterial pressure (MAP) and O2 saturation between two groups respectively showed a significant difference in 2minutes (P=0.001 and P=0.015) and minutes (P=0.006 and P=0.001) after intervention. The mean difference of MAP (p=0.041), oxygen saturation (p=0.001) and diastolic blood pressure (p=0.021) were significantly different from the Baseline after 2 and 5 minutes of the intervention.Conclusion: It seems suctioning based on comprehensive criteria is effective in preventing side effects and maintaining hemodynamic stability in mechanically ventilated patients.

Abstract: 

Background and Objectives: Heart failure is more common in the elderly than in the general population. Among the non-drug treatments, the easiest and most comprehensible method is direct implementation of sports program. Due to high cost of hospital care for patients with heart failure and complications, this study examined the effects of walking as a form of rehabilitation program. The subjects were patients hospitalized for heart failure.Materials and Methods: This study was a double-blind trial carried out on 50 elderly patients with hearth failure evacuation fraction less than 50% in 2014. Then, they were randomly divided into two intervention and control groups. The intervention group did exercise program on all days of the week except Friday. A questionnaire was used to collect date before and after intervention in both groups. To analyze the data, we used Chi-square, Exact Fisher test and Friedman test and SPSS16.Results: Fridman test of cardiac function levels in the control group both before and after the implementation period showed no significant difference (p>0.05). In the intervention group, however, a significant difference was observed between two intervening periods (p<0.05).Conclusion: The results of the study indicate that an exercise program can be safely performed in patients with class II and III heart failure and prevent deterioration of the disease.

Abstract: 

Background and Objectives: Due to the side effects of drugs, non-drug treatments today have attracted the attention of nursing systems. This study aimed to determine the effect of aromatherapy massage with peppermint oil on physiological indicators after surgery.Materials and Methods: In this clinical trial, at Razi Hospital in Ahvaz, 60patients were selected and matched for laparoscopic cholecystectomy surgery based on inclusion criteria for age and gender and were assigned to intervention (N=30) and control groups (N=30). The intervention group members were massaged at hands and feet with peppermint oil twice immediately after entering the recovery room and 6hours later; for the control group sweet almond oil was used as placebo. Physiological indicators in the 0, 6, 12, 24, hours after surgery were recorded. To analyze the data, we used independent t- test, chi-square and repeated measures test and SPSS19.Results: Using duplicate values, a significant relationship was seen between the average systolic blood pressure (P=0.03), average diastolic blood pressure (P=0.02), and average heart rate (P=0.04) in both groups. But, there were no significant differences in the average number of breathing times in the 2groups (P=0.08).Conclusion: Based on the results of this study, physiological indicators in both groups were in the normal range; therefore, aromatherapy with peppermint oil is not considered to be clinically valuable.

Abstract: 

Background and Objectives: Pregnancy includes a temporary stress, and if not managed well, it becomes a prolonged problem which causes the mother and the family great distress. Some results show that training has a significant effect on reducing anxiety, pain and duration of labor. Mothers trained as compared with the control group experienced less pain and anxiety. This study aimed to investigate the effect of childbirth preparation classes on childbirth attitude in parous women at Zanjan.Materials and Methods: This is a quasi-experimental study. A sample of 60parous women were divided into two 30-member groups. The case group attended childbirth preparation classes, whereas the control group received only standard antenatal care. Questionnaires were completed one hour after delivery in postpartum delivery room by the lead researcher with face to face interviews. The Child Attitude Questionnaire (CAQ) was also used. To analyse the data, we used t-test and on way anova, and SPSS19.Results: The mean score of the case group was 34.87±9 and that of the control group was 34.43±9.31. Independent t-test showed no significant difference between the two groups.Conclusion: We recommend that prenatal preparation classes be held to change parous women's attitude towards childbirth.

Abstract: 

Background and Objectives: Lack of general health and marital intimacy has a major role in causing couple burnout. Considering the importance of assessing and identifying constituents of burnout, this study aimed to determine the predictive role of general health and marital intimacy in the couple burnout among nurses.Materials and Methods: This is a correlational research in which 163nurses were selected through convenience sampling method and then assessed by General Health questionnaire (GHQ), Couple Burnout Measurement (CBM), Marital Intimacy questionnaire (MIQ) and Demographic Characteristic Questionnaire. Data was analyzed using Pearson correlation and stepwise multiple regression analysis.Results: The results of Pearson’s correlation coefficient showed that there was a significant and negative relation between burnout and general health, marital intimacy, income and marriage duration. In addition, the results of stepwise regression indicated that four factors could predict 48percent of couple burnout.Conclusion: According to the results, general health, marital intimacy, income and marriage duration play a considerable role in the couple burnout among nurses. Therefore, with regard to these factors, it is necessary to pay more attention to couple burnout.

Abstract: 

Background and Objectives: Difficult intubation is one of the challenges for patients undergoing anesthesia that leads to severe complications and death. Obstructive sleep apnea is one of the factors that may be associated with difficult intubation. The present study was carried out to investigate the relationship between obstructive sleep apnea and difficult intubation in the elderly.Materials and Methods: This cross- sectional study was conducted in 2015 from February to June on 175 elderly patients using convenience sampling method. In order to gather data Demographic form, Berlin questionnaire and Comark-Lehane Index were used. Data were analyzed by SPSS (version 18) Statistical software and Kolmogorov-Smirnov, independent t-test, Mann-Whitney-u, chi-square tests and Logistic Regression.Results: The mean age and SD of samples were 69.9±9.3 years. There were 18 (26.9%) people with difficult intubation and 10 (9.3%) people with normal intubation at high risk of sleep apnea. The results of this study showed that there was a significant relationship between sleep apnea and difficult intubation, and the risk of sleep apnea in the elderly with difficult intubation was 3.6times of those with normal intubation (p=0.002, OR=3.6, CI95%=1.5-8.9).Conclusion: There is a relationship between obstructive sleep apnea and difficult intubation in the elderly. Therefore, a screening procedure to identify the elderly at high risk of sleep apnea that are candidate for anesthesia seems to be necessary.

Abstract: 

Background and Objectives: Among significant factors that cause satisfaction of hospitalized patients are the care and respect within confines of the patient's rights shown by the health care providers. The present study was carried out to determine the level of respect offered by midwifery Staff at Ayatollah Mousavi Hospital, Zanjan.Materials and Methods: This descriptive study was conducted on 256 mothers who gave birth to children at Ayatollah Mousavi hospital, Zanjan, Iran. The subjects were selected randomly. Data was collected using a questionnaire which included demographic characteristics and human territory questions. The questionnaire was distributed two hours after delivery. Data was analyzed using SPSS software, Mann Whitney, Kruskal–Wallis, Spearman rank correlation.Results: The results showed that mean score of attitude of mothers towards respect of human territory was 32.22±4.79, for the attitude of mothers was 13.35±2.51, and for physical and. for mental territory. Generally, majority of the mothers had good attitude toward the respect of physical (67.9%) and mental (77.4%) territory.Conclusion: Mothers showed good compliance with human territory. Thus, it is recommended in-service training courses be held and barriers be removed for a better compliance with human territory in the health care system.

Abstract: 

Background and Objectives: Delay in patient care is the most common error that threatens patient safety and increases mortality, and hospital costs. This study investigated the relationship between delay in patient care and nurses professional commitment.Materials and Methods: This is a descriptive correlational study. Through a stratified sampling method, 300 nurses were selected from Tehran University of Medical Sciences teaching hospitals. Data were collected by a three-part questionnaire consisting of demographic data, the nurse's professional commitment scale, and delay in care questionnaire. Data were analyzed by SPSS software version 16 using descriptive statistics and Pearson's correlation coefficient.Results: The participants were between 22-50 years old and the majority of them were women (86.7%) and married (64%). The mean score for delay in patient care was 21.54 for each nurse in the past 3 months (high) and it was 86 for professional commitment and there was a significant weak and reverse relationship between delay in patient care, and nurses professional commitment (p<0.008, R=-0.15).Conclusion: Patient care delay is a common error in health system regardless of individual factors such as professional commitment; other factors must also be identified and addressed. Strategies to report the errors and get help from other members of the care team contribute to reducing the complications of such errors.
