Background & Aim: Antibiotic resistance to quinolones as a therapeutic agent of urinary tract infection in increasing in the world. Thus, this study was designed to evaluate the resistance to quinolone antibiotics in Escherichia coli isolated from urinary tract infection of hospitalized patients in Tehran, Iran in 2017. Methods: In the present study, 150 E. coli isolates was collected from urine of hospitalized patients with urinary tract infection in Tehran, Iran. Evaluation of resistance to quinolone antibiotics was done by disk diffusion method. After genome extraction with phenol l& chloroform method, amplification of qnrA, qnrB and qnrS genes in the isolates was performed by PCR. Results: The rate of resistance to antibiotics nalidixic acid, ciprofloxacin, norfloxacin and levofloxacin in these isolates was 76%, 48%, 30%, and 18%. The results of amplification the qnr genes showed that 40% of isolates harbored at least one of the qnrA, qnrB and qnrS genes. Among the 60 isolates harboring the qnr genes, 73. 3%, 23. 3% and 3. 4% had qnrS, qnrB and qnrA genes, respectively. Statistical analysis of results showed that there was a significant relation between the co-presence of qnrS and qnrB with co-resistance to nalidixic acid and ciprofloxacin antibiotics Conclusion: In this study, a significant rate of resistance to quinolone antibiotics was observed in E. coli isolated from patients with urinary tract infection in Tehran hospitals that showed a remarkable rate of resistant genes. It is suggested to study the origin of these infections in the collected samples in the future.

Background & Aim: Due to hazardous working conditions, firefighters are exposed to different types of stress which might cause occupational burnout and impact their quality of life. Therefore, the present study was conducted to determine the effectiveness of stress management training on the life quality and occupational burnout among firefighters in Tehran. Methods: Current study is a quasi-experimental study including pre-and post-tests and control group. The statistical population of the study was male firefighters of Tehran Fire Department in 2016. Thirty participants were selected through random sampling and were randomly assigned into the experimental (N=15) and control (N=15) groups. The experimental group underwent ten training sessions of stress management (90 minutes each). The control group did not receive the intervention. Two life quality scales, namely The World Health Organization Quality of Life-BREF ((WHOQOL-BREF) and Maslach Burnout Inventory (MBI-22) were used to gather the data. The data were analyzed based on covariance analysis using SPSS software version 21. Results: The results showed that there was no significant difference between the two groups at pretest, while stress management training of the experimental group significantly increased life quality components (P<0. 05) and caused meaningful decrease of occupational burnout scores (P<0. 05). Conclusion: The present study showed that stress management training program can increase the life quality and decrease occupational burnout of firefighters. Therefore, managers of Fire Departments are recommended to use this program to improve the mental health of firefighters.

Background & Aim: Nowadays, training of efficient human resources and providing appropriate context for achieving lifelong learning are among the main duties of universities. The purpose of this study is to determine the psychometric properties of the Persian version of Lifelong Learning Scale using Weighted Least Square method. Methods: This is a methodological and psychometric study. The utilized tool was the Persian version of JeffSPLL. The sample population consisted of 171 staff members of Birjand University of Medical Science. To investigate the factorial structure of the questionnaire considering ordinal scale for data, weighted least squares methods (WLS, WLSM and WLSMV) were used. Furthermore, three indices including RMSEA, CFI and TLI were adopted to check the model fitting. Statistical analysis was conducted using MPlus version 6. Results: The results of confirmatory factor analysis using WLSMV method was 0. 1 for RMSEA. CFI and TLI indices were 0. 91 and 0. 90 respectively. So, four-factor model was appropriate. Conclusion: The present study showed that the Persian version of the JeffSPLL, like the English version of the scale, has four factors including professional learning motivation, scholarly activities, attention to learning opportunities, and technical skills in information seeking. Therefore, it can be used to evaluate lifelong learning of Persian-speaking medical staff.

Abstract:

Background & Aim: Heart failure is a chronic disease that requires selfcare behaviors until the end of life. Therefore, identifying these behaviors and encouraging the patient to participate and have the responsibility for doing them rightly, will lead to control of the disease complications. This study is designed to investigate related factors with self-care and self-efficacy in patients with heart failure. Methods: This is a cross-sectional descriptive-analytic study that was done in Shahid Modarres hospital in Kashmar in 2016. Data collection was conducted through an interview using three-part questioner: demographic information, self-efficacy and self-care in patients with heart failure. Multiple linear regression was used to examine the factors affected on self-efficacy and self-care. Data analysis was done in 95% confidence level using SPSS version 16. Results: The mean of age was 57. 83± 8. 56. Mean score of self-efficacy and self-care was 16. 35± 8. 24 and 31. 11± 8. 21, respectively. The results show age increasing and being a smoker, had negative relationship and higher education and being urban had positive effect on self-care and self-efficacy (P<0. 0001). Also, there was a significant direct relationship between self-care and self-efficacy (r=0. 86, P<0. 0001). Conclusion: Considering that patients did not have high self-efficacy in performing their self-care behaviors and poorly performed, it is recommended that the factors affecting self-efficacy and self-care in the examined patients are further investigated and measures are taken to deal with these factors.

Abstract:

Background & Aim: Conjunctivitis is caused by a large group of infectious and non-infectious factors. However, no evidence of conjunctivitis following anal fissure surgery has been reported and such cases are not common. The present study is to report three cases of uncommon conjunctivitis following anal fissure surgery. Cases: Patients presented in this study included three middle-aged women who suffered from conjunctivitis following anal fissure surgery. Searching medical databases, we did not find traces of such conditions due to anal fissure surgery; therefore, these cases can be considered as the first reports of the condition. Conclusion: although further investigation revealed that disinfection of anesthetic mask using highly concentrated deconex accompanied with lack of appropriate wash before usage and thus a toxicity caused by high concentrations of dconex could have been the most probable cause of the incident, however, it is still unclear why this abnormal condition occurred.
