Abstract:

Introduction: Intentional releases or threat to use viruses, bacteria, fungi, or toxins produced by living organisms to cause death or pathogenicity in humans, animals and plants is bioterrorism.Contamination with biological agents could occur before or during the journey. In Iran the risk of such attacks is high because of the strategic situations and the presence of hostile forces in neighboring waters. The aim of this study is to present a scenario based prevention and response planning forbattle cruiser andbiological threats.Method: This is a qualitative and comparative study in which by using the risk management methods and mitigation planning and resiliency in different social environments, we propose a model in dealing with biological attacks in cruisers. Results: The beginning phase of disaster management cycle is mitigation and risk reduction. Hazards will be analyzed according to potential threats in the region, the environment, human recourses, equipment and time. According to the threats in the Middle East, biological agents in group” A” can be the biological hazards in the cruisers. We can determine the points of risk by calculating the return period, the severity of impacts and vulnerability of resources and the target group in a geographical area. According to the probability of each risk, we can identify priorities and regulate the possible scenarios. Then, the problems, which extracted from the scenarios, will be analyzed and the possible strategies for prevention and response will be developed. At least the scenario based response plan was developed. These plans need the planning teams and scenarios to develop. The biological attacks warning plan includes: the isolation of patients and carriers, surveillance system, drug prophylaxis and vaccination programs, Treatment, dead bodies management, provision of water and healthy food, a multi-layered system designed to communicate with the outside of the disaster area, decontamination, Triage, rescue and transfer to the referral hospitals and etc. Conclusion: Scenario-based risk management model in bioterrorist attacks reduces the biological effects of the bioterrorism agents.

Background: Laryngeal mask airway (LMA) is using for control of breathing during short-period operation. The patients may experience hemodynamic changes during cuffing or cough, nausea, and vomiting after driving out the mask. This randomized double-blind study was performed to compare postoperative hemodynamic changes and emergence reactin frequencies with preoperative topical lidocaine jelly and lubricant gel in patients.Materials and methods: Two groups of the patients consisted of 40 male in each group were candidate for elective varicocelectomy under general anesthesia. They were randomly assigned into two groups. The first group was received preoprative topical lidocaine jelly on LMA and the second group received preoperative lubricant jelly during LMA insertion. Gathered data was analyzed with SPSS-18 software using Chi – square and T-Student tests. Results: Two groups were similar with demographic parameres as age and weight. In the lidocaine jelly group hemodynamic changes and emergence reactions were lower than the lubricant jelly group. Conclusion: Usage of preoperative lidocaine jelly during LMA insertion can decrease hemodynamic changes and emergence reactions frequencies.

Abstract:

Introduction: rapid detection of pathogenic organisms is crucial in diagnosis and treatment of the diseases. The aim of this study was to evaluate efficiency of Fourier transform infra-red (FTIR) in detection of common bacteria in Khanevadeh hospital and Fallahi clinic, from NEZAJA military of health organization. Methods: we use 300 colony of bacteria from different wards of these two centers.20 colony was finally selected using some filters. These isolates were compared with literature and reference stoked bacteria using FTIR. The software IR solution used for peak correction. Finally, biochemical methods used for re-detection of bacteria. Results: there constituted six group of references bacteria from decomposition to the main component in the spectrum field CM-1. There were four bacteria in the reference cluster of E.Coli, four bacteria in the reference cluster of Klebsiella, three bacteria in the reference cluster of Pseudomonas, Three bacteria in the reference cluster of Enterococcus, three bacteria in the reference cluster of Staphylococcus Aureus, and three bacteria in the reference cluster of Staphylococcus Epidermidis. Biochemical detection methods confirmed FTIR in detection of bacteria. Conclusion: this technic for detection of bacteria was successful despite variation in bacteria structure and physiology. It may be used in some critical situation such as war, which need to rapid and exact diagnosis. FTIR method has various sensitivity and specifity which may result confusing in analysis. So it’s necessary to optimize this method using some another study.

Introduction: Wild birds have important pacts in distribution of parasites and other infectious disease.Their study has importance in ecology of parasites study. Wild and migratory birds may act as reservoir for zoonotic disease and monitoring of their disease has medically importance. Methods: For study of intestinal parasites of birds in Khuzestan province, this study was conducted on 46 dead birds found in the area. Specimens were stained by Carmen dye and were morphologically identified by diagnostic keys. Results: Herons (Egretta spp.) was heavily infected. They had highest prevalence of infection and were host of most variety of parasites. In parasitologic point of view Trematoda, mainly Priosoma sp (Family Echinostomidae) was most prevalent parasite. Cestoda like Taufiqia, Laritenia and diorchis were found in 4 and 5 carcasses of 10 cestod-infected birds respectively. Infection to Nematoda was seen less. In 9 cacesses which was infected with nematoda, 2 were infected with Anisakis sp. and in 4 cases immature larva were seen. Conclusion: This study may show table of helminthic parasites in wild birds of Khuzestan, in which Herons (Egretta spp.) seems to be more important. This is the first report of Taufiqia iranica which is parasite of necrophagous birds in Himantopus himantopus.

Background & Aims: Tapering can be defined as a period of training before the competition lasting a few days to several weeks in which training volume is progressively reduced while a portion of training is maintained at a high intensity. Despite the widespread use of the taper, little is known regarding how training volume and training intensity impact the time course of adaptations. We the effect of two tapering strategies on Anabolic to catabolic hormone ratio and performance in young male swimmers.Materials & Methods: In this semi-experimental study 12 young male swimmers as available sampling were selected. Subjects based on their final record were divided into two groups: two-stage tapering group (N 6) progressive tapering group (N 6). Before and after the completion of the 2 weeks tapering Program, performance test and blood samples were taken for assessment plasma levels of testosterone-to-cortisol ratio. To evaluate the result paired T-test and ANCOVA (analysis of Covariance) were used. Results: after2weeks of tapering program, in both groups compared with pre-test, testosterone-tocortisol ratio was reduced, but only in two-stage tapering group the reduction was significant. In both groups compared with pre-test, performance was increased significantly, while changes in the twostage tapering group performance record and testosterone-to-cortisol ratio levels compared with the progressive tapering group was not significant. Conclusion: generally it can be said, both two tapering strategies (two-stage and progressive tapering), while improving performance, have no effect on Hormonal Anabolic on catabolic response in male young swimmers.
