Abstract:

Introduction: Quality of life is one of the influencing factors in children with hemophilia. The quality of life of children with hemophilia is affected by complaints of permanent pain, bleeding, and discomfort related to treatment complications. The aim of this study was to determine the effect of family-centered empowerment on quality of life of children with hemophilia in Ahvaz city in year 2018. Methods: In this quasi-experimental study, 50 children with hemophilia who referred to hematology clinic of Baqaei Hospital, Ahwaz, Iran, were randomly selected. The experimental group received 9 training sessions based on the familycentered empowerment model comprising four steps of threat perception, problem solving, educational participation and evaluation by the researcher with the presence of primary caregivers and children with hemophilia. The control group received only the routine care program. At baseline and one month after the intervention, both groups completed the demographic and quality of life questionnaire for hemophilic children. Data were analyzed using descriptive and inferential statistics (independent t-test, paired t-test, Fisher exact test, and Chi-square test) using SPSS 22. Results: The study showed no significant difference in total score of quality of life in children (P=0. 444) before intervention between the two groups; but after the intervention a significant difference was observed in terms of quality of life (P=0. 030) and family dimension (P=0. 019), friends (P=0. 015) and perceived support (P=0. 011) between the two groups. Conclusion: According to the results of this study, it can be said that familycentered empowerment can be used as a low cost and easy way to improve the quality of life of hemophilia patients by nurses.

Introduction: Patients with multiple sclerosis (MS) suffer from pain. Usually, the severity of other symptoms causes the pain of these patients to be ignored or considered to be normal. This study was carried out to determine the impact of telephone counseling and follow-up on pain in patients with MS. Methods: This randomized clinical trial was conducted on 70 MS patients attending the multiple sclerosis Clinic at Sina (Farshchian) Hospital, Hamadan, Iran. A two-month intervention was administered, with the phone calls organized as follows: Week 1 (two calls revolving around medication schedule Week 2 (two calls about a diet plan, Week 3 (two calls about the exercise schedule، Week 4 (two calls about the role of stress in MS; and Weeks 5 to 8 (four weekly reminder calls to ensure patient compliance with the contents delivered within the first 4 weeks. Pain assessment scale was completed before and after the intervention. Results: The mean of pain severity in the experimental group before and after the consultation with telephone follow up was statistically significant (P=0/001). The mean of pain severity in the experimental group after intervention was reduced to 4. 32 with a standard deviation of 0. 17. However, the mean and standard deviation of pain in the experimental group also showed a downward trend. The comparison of the mean pain severity in the two groups after the intervention showed a significant difference (P<0. 001). Conclusion: Telephone counseling together with telephone follow-up reduces pain in patients with multiple sclerosis and improves patient health. Therefore, counseling with telephone follow up is recommended in reducing pain intensity of patients.

Abstract:

Introduction: Current data on changes in contraceptive use is required to prevent unintended pregnancies. This study aims to study trends, and patterns of contraceptive use from 2000 to 2014. Methods: This survey research uses data from three fertility surveys conducted in 2000, 2009, and 2014 respectively among representative samples of 1396, 2, 934 and 3012 married women aged 15-49 living in the capital city of Tehran. The prevalence of contraceptive methods was estimated according to women’ s socioeconomic and demographic characteristics. Results: In 2014, 82% of women used contraceptive methods (48% modern methods, and 34% traditional methods). Withdrawal (33%) and condoms (21%) had the highest prevalence, compared with female sterilization (9%), IUD (7%) and Pills (7%). Over 2000-2014, the use of withdrawal and condoms increased by 69% and 20%, respectively. In contrast, the prevalence of female sterilization, pills, male sterilization, and IUD decreased respectively by 20%, 42%, 45% and 51% over the same period. The use of withdrawal method increased with women’ s age. Withdrawal and condom were used by 67% of contraceptive users, who mostly hold higher education levels, were employed, and lived in upper residential districts. In contrast, users of modern methods, namely sterilizations, IUD and pills, mostly hold lower levels of education and income, were unemployed, lived in the lower residential districts, and had two or more children. Conclusion: The increasing prevalence of withdrawal and condom signifies a need for provision of effective family planning education and counseling to prevent unintended pregnancy.
