Abstract

Considering the incidence of PEMPHIGUS VULGARIS and its fatal consequence if untreated, and various reports on the treatment efficacy of CYCLOSPORINE in its control, this study was performed on referrals of Loghman Hakim hospital (dermatology clinic) during the years 1995-1997. The clinical trial strategy of this study was performed on 60 patients including 26 women (43%) and 34 men (57%) at an age range of 20-60 years that randomly divided into two equal control and case groups. The case group received CYCLOSPORINE and prednisolone and control group received AZATHIOPRINE and prednisolone. The patient's characteristics, rate of improvement, relapse, and complications were recorded and analyzed for a follow-up period of at least 3 months. Out of 30 patients in case group, 27 cases (90%) respond acceptably to treatment, whereas in control group only 23 patients (77%) responded to treatment. The rate of relapse in case group was 11 % and in control group was 22%. There were also similar conditions for the two groups after treatment. Furthermore, there was a state of hypertension in case (37%) and control (13%) groups. In addition, there existed an alteration in the activity of hepatic enzymes in case (3% increase) and control (13% increases) groups. The results showed that there is no significant difference between the two groups regarding recovery and relapse. Considering the severe complications of CYCLOSPORINE and its high price in relation to AZATHIOPRINE, CYCLOSPORINE can not be the first option drug for the treatment of patients with PEMPHIGUS VULGARIS, and it can be used only in conditions of resistance to other immunosuppressive drugs.
