Abstract

Background: Difficult intubation is one of the most important problems of an anesthesiologist and is the main cause of mortality and morbidity of their patients. So it is really important to predict such patients by useful means and tests. Different references introduce different tests but their actual value has not determined yet. The present study was conducted over the patients who had been scheduled for elective surgery in Shohada- Tajrish and Imam Hossein hospitals, in 2000-01. Materials and methods: In this clinical trial, the following criteria have been used for predicting different endotracheal intubation: mandibular subluxation, thyromental distance, mouth opening, malampati, atlantoocciptal extension, and ramus length. Different intubation means more than 2 times try for laryngoscopy or class 3 or 4 of laryngoscopic view. Results: 400 patients (59% males and 41 % female) with the mean (±SD) age of35.7± 15.3 years have been enrolled. The positive predictive value of the aforementioned tests were, 9%, 61.1%, 3.5%, 4.4%, 2.3%, and 13.6%, respectively. Their related NPV were 100%, 98.9%, 91.2%, 85.8%, 85.9%, and 98%, respectively. Conclusion: We realized that none of the above-mentioned tests are reliable alone. Further studies are strongly recommended.
