RELATIONSHIP OF SERUM FERRITIN CONCENTRATION, IRON AND ITS PARAMETERS WITH PREECLAMPSIA

Abstract

 Background: Pregnancy-induced hypertension (PIH) disorders are one of the most important complications in pregnancy and are associated with increased maternal and fetal morbidity. Pathogenesis of the disease is unknown. It is seemed that antioxidant activity is reduced in this state and may be IRON trigger oxidative stress. The purpose of this study was determination of relationship between FERRITIN concentrations, serum IRON and its parameters with preeclampsia. Materials & Methods: This analytic case-control study that measure serum IRON concentration, FERRITIN and total IRON binding capacity in preeclamptic patients and compare to normal pregnancy was done in Imam hossein hospital in 2002. There were 33 patients in preeclamptic group and 33 cases in control group. Exclusion criteria were anemia and hemoglobolin<11mg/dl. Blood samples were taken for CBC, IRON, FERRITIN, TIDC and liver function tests before or 24 hr after delivery. Data were analyzed by SPSS 10.0.Pvalue<0.05 was considered as significant.Results: The mean age of patients was 27.7±6.3 years in preeclamptic and 24.5±5.5 in control group. The differences was not statistically significant. Serum IRON level was 79.9±32.41 µg/dl in preeclampsia and 88.6±40.8 µg/dl in the other group (Pvalue>0.5).Total IRON binding capacity (383.7±63.3 g/dl) was lower in women with preeclampsia when compared to normal pregnancies (443.1±50.5g/d1, P<0.002). Serum FERRITIN was significantly higher in the preeclamptic patients than in the healthy pregnant women (123.8±46.1 ng/dl vs. 32.H16.2ng/dl, Pvalue<0.001).Serum FERRITIN level was 10 times in HELLP SYNDROME (5 cases) and 3.6 times in preeclamptic group. There were not any significant differences in IRON, hemoglobin, thrombocytopenia and liver function tests.Conclusion: According to these finding there was a powerful difference in serum FERRITIN and TIDC in preeclamptic patients versus normal pregnant women without any relation with hemoconcentration that is seen in preeclampsia. It is seemed increased FERRITIN and decreased total IRON-binding capacity in preeclampsia may promote oxidative damage of vascular endothelial. Therefore, FERRITIN could be used as diagnostic marker in preeclampsia. Elirnination of IRON supplement possibly, will prevent high risk pregnant women from preeclampsia.
