Title

DIAGNOSTIC TESTING: VALIDITY, PREDICTIVE VALUE POSITIVE AND NEGATIVE, PRE AND POST - TEST PROBABILITY

Pages

  41-49

Abstract

If a test result provides perfect information, the post - test PROBABILITY will be 100% if the test is positive and 0 is negative, regardless of the pre-test PROBABILITY. Unfortunately, few tests are able to discriminate perfectly between patients with and without disease. The discriminating ability of a test is defined by its VALIDITY (sensitivity + specificity). The way information about a DIAGNOSTIC method is used must be adapted to changes in disease prevalence. Thus the PROBABILITY of disease given that a test is positive, termed the PREDICTIVE VALUE positive depends not only on the ability of the test to be positive accurately and negative accurately, but also on the degree to which VALIDITY of the test is relevant for the particular patient or population. We must consider not only the probabilities of correct and incorrect diagnosis, but their consequences as well. How good is it if we treat on the basis of a true - positive test or not treat on the basis of a true negative test? How bad is it if we treat on the basis of a false positive test or not treat on the basis of a false negative test? Answers to these questions depend on the risk and benefits of treatment.
