USE OF FENTANYL FOR ANESTHESIA INDUCTION IN CESAREAN SECTION

Abstract

 In order to avoid of neonatal respiratory depression during management for cesarean section (CS), opioid administration usually is done after delivery of fetus and umbilical cord clamping.In the previous studies, high dose fentanyl administration during induction of anesthesia for CS had been accompanying with neonatal respiratory depression, while low dose administration of this drug had not been succeeding to prevention of cardiovascular effects due to tracheal intubation and skin incision. Present research studied the effects of 2µg/kg fentanyl supplementation during induction of anesthesia for CS, on mother's hemodynamic and neonatal APGAR score.Ninety normal women presenting for CS under general anesthesia were allocated randomly to receive either fentanyl 2µg/kg or placebo as part of anesthetic induction sequence. In all patients the cardiovascular response to tracheal intubation and skin incision was measured. This response also measured at 15, 30 and 45 minutes after skin incision. Neonatal APGAR score was measured at 1, 5, 10 and 60 minutes after birth.Mean of heart rate, diastolic and mean arterial blood pressure at skin incision time and 45 minutes after that, were significantly lower in study group (P<0.05). Mean of systolic blood pressure at skin incision time was significantly lower in study group (P<0.05). The 1, 5, 10 and 60 minutes APGAR score of neonates were 8-10 and there were not differences between two groups.Administration of 2µg/kg fentanyl as part of anesthetic induction for CS can prevent homodynamic effects of tracheal intubation and skin incision in mothers, while it does not exert adverse effects on neonatal APGAR score.
