ANTIPHOSPHOLIPID ANTIBODIES IN ARTERIAL AND VENOUS STROKE IN YOUNG ADULTS

Abstract

 Introduction: There are few comparative studies concerning the prevalence of anti cardiolipin antibody (aCL) and other antiphospholipid antibodies (aPL) as perdisposing factors in venous and arterial ischemic stroke. In this study, the presence of aCL and aPL antibodies has been investigated and compared between 2 groups of younger patients with venous and arterial ischemic stroke. Methods: Both IgG and IgM types of aCL and other aPL (Antiphosphatidyllnositol, Antiphosphatidyl Serin, Antiphophatidic Acid and beta 2- Glycoprotein I) were measured in 147 patients with ischemic venous and arterial stroke (aged<45 years) in AI-Zahra hospital, Isfahan, Iran and demographic, clinical and laboratory characteristics of patients with positive titer were recorded. Results: From 147 patients, 114 and 30 patients had arterial and venous ischemic stroke, respectively. Twenty three (7 men, 16 women, 19.6%) and 6 patients (2 men, 4 women, 20%) had increased IgG or IgM or IgM types of aCL or aPL in arterial and venous ischemic stroke groups, respectively but it was not significont. IgM type of aPL and aCL in arterial and venous stroke patients has been detected more than IgG type in both group (P>0.05). In 5 (21%) patients of arterial stroke group, aPL was increased but aCL was in normal range (none anticardiolipin -antiphosbolipid antibody). None of the patients in venous group had raised non-anticardiolipin antiphospholipid antibody titers (P <0.05). Discussion: Considering the concomitant increase of aCL and non-anti cardiolipin antiphospholipid antibodies (non-aCL aPL) in a considerable proportion of younger (45 years) patients with ischemic arterial stroke and the presence of non-aCL aPL in approximately 20% of arterial stroke patients without detectable aCL titers, it is necessary to measure antibodies against all phospholipids proteins in the case of recurrent arterial stroke. It seems that the role of non-aCL aPL in venous ischemic' stroke is less important than aCL. Despite European studies, high titers of IgM antiphospholipid antibodies found in a large number of our patients in both venous and arterial groups suggest the presence of unknown triggering factors (infections or poisons), which should be investigated more extensively.
