Abstract

In this retrospective study, we investigated the frequency-encountered pattern of GI bleeding abnormalities in various GI complaints in various illnesses with the aid of GI ENDOSCOPY. Among 4168 patients suffering from various GI complaints who were referred to Imam Reza Hospital's Internal ward and categorized according to age, gender, symptoms, type of mucosal lesions, ultimate diagnosis was made according to causal links with administrated medication. The resultant information was evaluated. Clinical manifestations that were found in decreasing frequency included: UPPER GI BLEEDING (45.4%), dyspepsia (14.5%), ASCITES (14.4%), dysphasia (6.2%), hepatomegaly (3.8%), chronic vomiting (4%), anemia (3.6%), splenomegaly (1.9%) and others (6.2%)(eg epigastric masses, chronic diarrhea...). The frequency of endoscopic findings included: duodenal ulcers (19.21%), esophageal varices (16.24%), gastric ulcers (14.32%), esophagit is (6.95%), gastric adenocarcinoma (6.74%), esophageal cancer (4.96%), erosive gastritis (4.36%), duodenitis (2.87%), others uncommon (5.52%) (eg non-specific gastritis, gastric lymphoma, achalasia...) and normal (18.83%). Mean age and standard deviation of duodenal ulcer, gastric ulcer esophageal varices and gastric adenocarcinoma were 42.9 and ±16.2, 53.6 and ±17.3, 50.2 and ±17.3,64.5 and ±16.1 respectively. The prevalence f duodenal ulcers and gastric ulcers as cause of UPPER GI BLEEDING were 24.3% and 21.4% respectively. NSAIDS was responsible for 25% of UPPER GI BLEEDING. A striking finding in our study about causes of UPPER GI BLEEDING in patients with PORTAL HYPERTENSION was esophageal varices in about 95%of cases, in contrast to conventional studies in literatures that were caused by other etiologies in 30% to 50%. This discrepancy has an etiologic basis (i.e. alcoholism is usual practice in western countries, the main cause of cirrhosis). In every patient with a complaint of hematemesis and splenomegaly esophageal varices is the major cause of bleeding.In this study mean age of duodenal ulcer id one decade less than gastric ulcer compatible with original references.We should not consider any ASCITES as a result of PORTAL HYPERTENSION, because there are other causes for ASCITES, for example peritoneal metastasis and tuberculosis peritonitis, even in the presence of other manifestations of PORTAL HYPERTENSION.
