Abstract

CHRONIC HBV INFECTION is a major medical problem in the world, and yearly a large number of patiens die by sequele of this virus in the form of cirrhosis and hepatocellular carcinoma. Because of the prevalence of 3-5% of this infection in our society, this prospective survival analysis was conducted to evaluate the outcome of 221 individuals with CHRONIC HBV INFECTION in Babol from 1991- 2000 . Liver function tests, HBsAg , HBEAG , α fetoprotein were tested every 6 months. Patients with CAH, and cirrhosis were excluded in the first visit. Data were analyzed by coxrgression model and kaplan - Meier methods. A total of 221 cases, 58.8%were male and 41.2% were female. Mean age ± SD was 29 ± 11 years. Mean years of FOLLOW UP was 4.5 ± 2.6 years. HBEAG was positive in 22 cases. Clearance of HBsAg and HBEAG were obderved in 24 cases (12.1%) and 10 cases (45.5%) in HBEAG negative and HBEAG positive cases (Annual negativation of 2.7% and 10.5% respectively). Negativation of HBEAG as compared to HBsAg was 4/35 fold.(P= 0.0001). Progression to CAB were observed in 18 (11%) and (13.5%) in HBEAG negative and HBEAG positive individuals (A mean annual rate of 2.1% and 3.2% respectively). Progression to CAH in HBEAG positive patients as compared to HBEAG negative patients was 2.1fold (P= 0.22) . α fetoprotein was increased mildly in 5 cases (15-45). Liver cancer was not seen. This study showed that negativation of viral markers and progression to CAH were observed in our cases, but progression to cirrhosis and liver cancer are rare in this perioid of times FOLLOW UP of these cases are necessary.
