TRANSPUPILLARY THERMOTHERAPY FOR THE TREATMENT OF OCCULT SUBFOVEAL CHOROIDAL NEOVASCULARIZATION IN AGE RELATED MACULAR DEGENERATION

Abstract

 Objective: To assess the efficacy of transpupillary thermotherapy (TTT) for the treatment of OCCULT CHOROIDAL NEOVASCULARIZATION (CNV) in patients with AGE-RELATED MACULAR DEGENERATION (ARMD).Participants: Twenty-three eyes of 22 consecutive patients with occult CNV that underwent TTT, were studied prospectively.Design: Interventional, non-comparative, prospective, self-controlled, case-series (before-after design).Methods: Twenty-three eyes with angiographically defined CNV underwent TIT with diode laser (810 nm). Complete eye examination and fluorescein angiography were performed before and after thermotherapy.Laser beam size ranged from 1.2 to 3.0 mm and power setting between 280-640 mW.Treatment was given in one area for 1 minute, the end point being no visible change or very slight graying of the retina. Outcome measures included visual acuity changes and angiographic and fundus examination findings. Patients were followed 1, 2, 4 and 6-12 months after treatment.Results: Enrolled eyes with occult CNV underwent TTT and followed for a mean of 7.1 months (range 6-12). Mean visual acuity before treatment was 20.320 (range 20.800 - 20.50) and improved significantly after TTT to 20.180 (range 20.400 - 20.40) (P=0.031).At the final visit, 6-12 months post- TTT, visual acuity remained stable in 13 eyes (56.6%) (±1 Line variation), improved more than one line in 9 eyes (39.1%) and dropped two lines in one eye (4.3%). Recurrence of CNV occurred in 4 eyes (17.4%) 4-6 months after treatment. No serious complication occurred in patients.Conclusion: TTT is an effective treatment for occult CNV which may maintain or improve visual function and cause CNV closure in the majority of patients.
