THE EFFECTS OF EXTRACORPOREAL SHOCK WAVE LITHOTRIPSY (ESWL) ON RENAL AND LIVER FUNCTIONS

Abstract

 Background and Objectives: ESWL is the safest method for treatment of urolithiasis. However, recent studies have shown that KIDNEY and adjacent organs may be affected by this method of therapy. Therefore, we evaluated effects of shock wave in the patients who underwent ESWL. Materials and Methods: A total of 60 patients with non-obstructive stone of upper urinary tract were randomly selected for this clinical trial study. Effects of shockwave on LIVER and renal functions were evaluated by measurement of serum creatinine, creatinine clearance, twelve hour urinary protein, and serum LDH, SGOT and SGPT. To analyze data SPSS 9.2 programme and paired sample t-test were used. Results: No significant changes were observed in serum creatinine or glomerular filtration rate (GFR) (P=0. 1). On the other hand, there was significant proteinuria with hematuria after ESWL (P<0.001). With increasing of total given energy, levels of serum LDH, SGOT and SGPT significantly raised (P<0.001), but serious clinical changes were not observed. Conclusion: After ESWL there was some damage on KIDNEY and LIVER cells but these effects were not serious; however, more research is required in this field. In relationship to these effects especially on right side urolithiasis and high total given energy, those who need multiple repeated sessions of ESWL or treatment of a long duration must be cautious about the COMPLICATIONS.
