IDIOPATHIC RESTRICTIVE CARDIOMYOPATHY IN CHILDREN, STUDY OF 5 PATIENTS

Abstract

 The functional classification of cardiomyopathies includes three types of dilated, hypertrophic and restrictive. Idiopathic RESTRICTIVE CARDIOMYOPATHY is a poorly recognized entity of unknown cause characterized by nondilated, nonhypertrophied ventricles diastolic dysfunction resulting in dilated atria and variable systolic function. RESTRICTIVE CARDIOMYOPATHY is rare in CHILDhood and have poor prognosis. The purpose of this report is to review the clinical and hemodynamic profile and outcome of RESTRICTIVE CARDIOMYOPATHY in a group of five Iranian CHILDren. (Three girls and two boys). The Mean age at diagnosis was 13.2 years (range 5 to 17 years). All patients had evidence of systemic and pulmonary venous congestion. Biatrial enlargement was the most consistent EKG finding and was present in all patients. The most striking echocardiograhic feature was severe biatrial dilation in the presence of normal or near normal ventricular cavity dimensions. Elevation of left and right ventricular end-diastolic pressures, pulmonary artery pressure and right atrial pressure were noted in all patients undergoing cardiac catheterization. Pericardial biopsy was performed to rule out of constrictive pericarditis in two patients. One patient died 10 years after diagnosis.
