Abstract

Introduction and aim: The estimated rate of poison exposures related deaths is 7% of mortality at all. Intended POISONINGs are influenced to a large extent by the population's socioeconomic and cultural status. It is clearly important to have guidelines for assessment, appropriate management and possible modes of prevention for intended POISONINGs, but first, the magnitudes and patterns of this problem have to be known. Objective: To find out the magnitude, patterns and characteristics associated with intended POISONINGs in Birjand.Material and methods: A cross sectional study was performed on all cases that were admitted with intended POISONINGs in 2 referral centers in Birjand during 2001. Various parameters in relation to agent of POISONING, and patients were considered for statistical analysis.Results: Out of the 602 cases that had poison exposures, 222 (36.8%) harmful intention (DSH/Suicide), 85 (14.8%) harmless intention (abuse, misuse, recreational) were seen. The prevalence of harmful intention was seen to be higher in the 15-24 years age group, in females, in unmarried and in spring season. Benzodiazepines were the most common agent of POISONING in this group. About 80% of the cases had trivial POISONING and were discharged in the less than 6-12 hours.In contrast to harmful intention, the prevalence of harmless intention was seen more in the men than in women, and the tricycles antidepressants were the most common agent caused POISONING. A significant different was noted between the type of POISONING and patients education (P=0) in harmful intention cases. POISONING was more common with DRUGS in educated persons and with chemicals agent in uneducated persons.About 80% of the cases in harmful intention and 78% in harmless intention had a trivial POISONING and were discharged in the less than 6-12 hours.Three deaths occurred in harmful intention and one death in harmless intention POISONING 1%-2% of intentional POISONING at all.Conclusion: Female sex, age of 15- 24, spring season, were the risk factors for DSH/suicide. Because the act of DSH may reveal internal feeling of distress and because that as many as 10%-14% of those DSH may die by suicide, there should be specific attention to psychosocial precipitant factors for DSH to forestalling suicide particularly in teenage girls.
