Abstract:

Primary appendiceal lymphoma is rare and seldom reported in the literature in this age. It is mostly of B-cell nonHodgkin’s type. Most of the cases in the prior reports were in men and the elderlies. Our case is a unique, according to the age which is 15, Our patient was presented with abdominal pain and signs and symptoms of acute appendicitis. After operation pathology specimen showed an appendicular mass of four centimeter in diameter. Histological and immunohistochemical study showed a diffuse large B-cell lymphoma. He received chemotherapy and now he is in good condition after two years....

Abstract:

Tazkerat-ol Olyaa, the only prose work of Attar, is a collection of quotes and biographies from seventy two great mystical figures. He starts the book by narrating the biography and famous quotes of Imam Jafar Sadeq (the sixth Shiite Imam) and finishes it by the accounts of Hossein Mansoor-e Hallaj’s life. An APPENDIX was added to Tazkerat-ol Olyaa which includes twenty biographies from Ibrahim-e Khavaas to Imam Mohammad Bagher (the fifth Shiite Imam). Some researchers believe that this APPENDIX was added by a person or some persons after Attar and base their argument on the differences in the writing styles and lack of this APPENDIX in the earlier hand-written versions of Tazkerat-ol Olyaa. However, to prove this claim some more proofs are essential. An intertextual comparison between Tazkerat-ol Olyaa and some other old works is a way to identify the writing style used in the first section of Tazkerat-ol Olyaa better and, by the same token, we can then declare whether or not the second section of the work was added later. Tazkerat-ol Olyaa has a deep relationship with its preceding mystical works because Attar has made it clear that he has used some different sources and has used a combinational method to author the biographies. The intertextual study of Tazkerat-ol Olyaa with its predecessors shows that Attar has had a unique and special style in narrating the life of characters in Tazkerat-ol Olyaa. This style which is carefully used in the first section of the book is not observed in the second section. This strengthens the idea that second section was added later by some other person or persons. In this paper, we study the intertextual relations between Tazkerat-ol Olyaa and its sources and provide some examples of these relations. Then, after an analysis of Attar’s method in using the earlier works, we focus on some differences between the first section and the appended section of Tazkerat-ol Olyaa from the intertextual relations’ perspective.

pregnancy. Unlike omentom, cecum and APPENDIX are not likely to Background: Early diagnosis of appendicitis is important during be displaced by the growing uterus, so the concept of position change of the APPENDIX during pregnancy is questionable. Objective: To determine the anatomical location of APPENDIX during pregnancy. Methods: In a prospective study from October 1995 to March 1999, 291 women of reproductive age were evaluated for the location of APPENDIX. They were divided into 3 groups, A: 165 pregnant women (37-40 weeks of gestation) who underwent elective cesarean section, B: 26 pregnant women (19-39 weeks of gestation) with acute appendicitis, who underwent appendectomy, and C: 100 non-pregnant women with acute appendicitis who underwent appendectomy serving as the comparison group. The location of APPENDIX was considered as normal if it fell within ± 2 cm variation from McBurney’s point, otherwise, it was considered as a position change. Results: In group A, 26 out of 165 (15%) and in group B, 6 out of 26 (23%) women had change in the position of APPENDIX. In the control group, 17% had change in position. There were no significant differences between group A and B as compared with the control group (group C). In group B, there was no relation between the height of the fundus and the point of tenderness. Conclusion: The location of APPENDIX does not change significantly during the pregnancy in most women.

Abstract:

We report a rare variant of mucocele of the APPENDIX. An elderly gentleman was admitted with symptoms and signs of acute appendicitis. He underwent laparoscopic converted open appendicectomy. On exploration there were pearls or fish egg like globular material along with jelly in and around the APPENDIX. On examination of the specimen a differential diagnosis of hydatid cyst or pseudomyxoma was considered. On microscopic examination, the globules consisted of eosinophilic laminations of mucin surrounding an amorphous granular core. After histopathological examination, our case was diagnosed as myxoglobulosis of the APPENDIX. Acute appendicitis is a common clinical problem and appendisectomy a regular surgery. The clinician should be aware of this entity of myxoglobulosis, to avoid confusion with pseudomyxoma or hydatid cyst. Presence of typical fish eggs or pearls like globular structures in and around the APPENDIX is suggestive of myxoglobulosis. Appendisectomy is curative treatment for patients with myxoglobulosis of APPENDIX and there are no reports of its recurrence.

Abstract:

Introduction: Primary appendiceal adenocarcinoma is rare (less than 250 cases described in the literature). In this article a case of appendiceal carcinoma with peritoneal seeding is reported.Case: The patient is a 38 years old man presented with pain, tenderness and rebound in the lower quadrant of the abdomen. These findings suggested acute appendicitis. Emergency laparotomy showed inflamed APPENDIX, without adhesion to the surrounding tissue or perforation. Appendectomy was performed. The histologic diagnosis revealed a moderately differentiated appendiceal adenocarcinoma. The tumour had infiltrated up to serouca of the APPENDIX. The patient was reffered to oncology center but due to refusing of patient for further evaluation, six months later he was admitted with dyspnea, abdominal pain and prorusion. Clinical examination and diagnostic work ups showed massive exudative ascites. The patient was admitted to the oncology ward of Vali-e-Asr hospital with the diagnosis of  appendiceal adenocarcinoma with peritoneal involvement. Conclusion: In case of acute appendicitis, the possibility of appendiceal adenocarcinoma should be considered

Abstract:

Primary appendiceal lymphomas are exceedingly rare neoplasms mostly ignored until histological examination of appendices. We report a case of primary appendiceal lymphomas in a 22–year-old man. Appendectomy was performed under diagnosis of acute appendicitis. Macroscopically a fleshy intramural mass in middle third portion of APPENDIX was observed. Microscopic examination revealed a diffuse large B-cell lymphoma inducing acute appendicitis. Further medical workup of the patient was unremarkable and the disease was restricted to APPENDIX. After initial simple appendectomy, the patient underwent adjuvant chemotherapy and was healthy after two years. This case emphasized great value of detailed histological examination of all appendices.
