Abstract: 

Background: Brucellosis is one of the endemic diseases in Iran that has a worldwide spread and is associated with chronic disabilities in humans. Combination therapy of Brucellosis leads to recovery of symptoms, shortening of the symptomatic intervals, and decrease in the rate of relapse and drug resistance. Considering the use of rifampin in the treatment of tuberculosis, and the necessity for an alternative treatment in regions endemic for both tuberculosis and brucellosis, the aim of this study was to compare the efficiency of the regimen of rifampin-Doxycycline with ciprofloxacin-Doxycycline in relapse of brucellosis.Materials and methods: This randomized controlled trial was performed on 90 patients, older than 17 years old, affected with brucellosis, which were referred to the Infectious Disease Clinics at Arak University of medical sciences between the years 1384-1387. The patients were randomly divided into two groups: the DR groups, receiving 100 mg of Doxycycline twice a day and 300 mg of rifampin Bid daily for eight weeks; and the CD group, receiving 100 mg of Doxycycline plus 500 mg of ciprofloxacin twice a day for eight weeks. The patients were analyzed for the relief of symptoms, drug side effects, and laboratory findings during the treatment.Results: In this study, the rate of relapse in both groups were similar. The relapse was seen in 4.5% and 3.2% of the patients for the DR and CD groups, respectively (P=0.168). The drug side effects were slight in both of groups, with no significant difference, and did not lead to discontinuation of the therapy.Conclusion: According to the same rate of relapse in both CD and DR regimens in the treatment of brucellosis and considering the usage of rifampin in regions with high prevalence of tuberclusis, the CD regimen is recommended as an appropriate one.

Abstract: 

Background and Aim: Brucellosis has widespread prevalence and is one of the endemic diseases in our country, Iran. It is estimated that every year almost 500000 cases of Brucellosis-mostly Brucella Melitensisoccur in the world. Any type of brucellosis demands a specific treatment. Thus, this study was conducted to compare reduction of Brucellosis anti-antibody level in the serum one year after treatment with two regimens, i.e. Streptomycin + Doxycyclin and Rifampin + Doxycyclin.Materials and Methods: This experimental study was performed on 100 patients with Brucellosis based on the clinical findings, wright ³ 1/80 and 2ME ³ 1/80. The cases were randomly divided into two groups, one received Streptomycin + Doxycyclin (50 cases), and the other group was treated by Rifampin + Doxycyclin (50 cases). Then 3, 6 and 12 months after treatment Wright and 2ME tests were performed and their results compared by means of statistics using Chi-Square test.Results: The findings of the study showed that age of the cases ranged between 13 and 78 years and 46% of them were 39-48 years old. Results of the tests revealed no significant differences before treatment ,but immediately after treatment significant differences were observed in the two groups (Wright: P=0.05, 2ME: P=0.005) as in the first group tests tended to be negative more rapidly than in the second group. Wright’s test after 3 months showed significant differences in the two groups (P=0.05) but there were no significant differences at 6 and 12 months following treatment.Conclusion: Regarding the rapidity of negative titers of tests in the first group immediately and 3 months after treatment, it could be inferred that Streptomycin regimen leads to a more effective treatment with rapid recovery which is also accompanied by reduction in the disease anti-antibody level in the serum.

Abstract: 

INTRODUCTION: The purpose of this ex vivo study was to determine the releasing characteristics and doxycycline dentinal diffusion of AH26 sealer-doxycycline combination from apical 3mm of tooth root and apical foramen.MATERIALS & METHODS: One-hundred and two recently extracted single-rooted human teeth were decoronated and prepared with #3 and #4 Gates-Glidden drills and rotary Mtwo files. Smear layer was removed; all surfaces except for apical 3mm of each root were sealed with two coats of nail polish. To quantify the release and diffusion of the doxycycline at different time intervals (30 min, 48 and 72 h) after root canal obturation, the samples were randomly divided into three groups (n=30; 0.5 h, 48 h, 72 h). To evaluate the release of doxycycline from AH26 sealer-doxycycline combination at six concentrations of antibiotic including 0.5%, 1%, 2%, 5%, 10% and 20%; each experimental group was divided into six equal subgroups (n=5). Root canals were filled with gutta-percha and AH26-doxycycline combinations and then were placed in vials containing 1.25mL of phosphate buffer saline solution (PBS). After 30 min, 48 and 72 h, the amount of doxycycline released from specimens into PBS were determined by measuring the absorbance values using UV spectrophotometry at lmax=350 nm. Data were analyzed using two-way ANOVA.RESULTS: The findings of this study revealed that AH26 sealer-doxycycline combination released variable measures of antibiotic at each time interval and in the various concentrations. At 30 min, no statistically significant differences were obtained between the results of subgroups, but at 48 and 72 h these differences were significant (P<0.001). The results also showed that differences between 0.5 h, 48 h and 72 h were significant within subgroups (P<0.01).CONCLUSION: Under the conditions of this ex vivo study, doxycycline can be released from AH26 sealer-antibiotic combination through 3mm of apical root and apical foramen at 30 min, 48 and 72 h after mixing the sealer with doxycycline at concentrations of 0.5% up to 20%.

Considering the use of rifampin in the treatment of tuberculosis, and the necessity for an alternative treatment in regions endemic for both tuberculosis and brucellosis, in this study we compared the efficiency of the WHO's standard regimen of rifampin-doxycycline (RD) versus ciprofloxacin-doxycycline (CD) for the treatment of brucellosis.Materials and Methods: This randomized controlled trial was performed on 90 patients, affected with brucellosis, who were referred to the Infectious Disease Clinics at Arak University of medical sciences. The patients were randomly divided into two groups: the DR group, received 100 mg of Doxycycline twice a day and 300 mg of rifampin Bid daily for eight weeks; and the CD group, received 100 mg of doxycycline plus 500 mg of ciprofloxacin twice a day for eight weeks. The patients were analyzed for the relief of symptoms, drug side effects, and laboratory findings during the treatment.Results: In this study, the rate of symptom relief and laboratory findings in both groups were similar. The relief of symptoms was seen in 93.2% and 83.9% of the patients for the DR and CD groups, respectively (P=0.182). The side effects of the drug were not significant in either groups, with no significant difference, and did not lead to discontinuation of the therapy.Conclusion: Due to the similar efficacy of CD and DR regimens in the treatment of brucellosis and considering the usage of rifampin in regions with high prevalence of tuberclusis, the CD regimen is recommended as an appropriate treatment.

Abstract: 

The aim of the study was to estimate the impact of antibiotic doxycycline on the spring development of the bee colonies, worker bees’ bodies chemical composition, development of hypopharyngeal glands and some physicochemical parameters of honey. The experiment was conducted during the spring stimulating feeding at apiary of Research Center of Stockbreeding and Agriculture, Smolyan, Bulgaria. Two groups of bee colonies were formed – control and experimental. The control group was fed with sugar solution (sugar/water 1: 1) and experimental group with the addition of antibiotic doxycycline at a dose of 500 mg/L in sugar solution. Significantly lower values were obtained for the strength of the bee colonies and the amount of sealed worker bee brood fed with doxycycline than the control group. Strong correlations were found between the amount of sealed worker bee brood and the quantity of bee pollen and the quantity of honey and bee pollen in the both groups. From this study, it was concluded that development of hypo-pharyngeal glands was not positively affected after application of antibiotic doxycycline at a dose of 500 mg/L. Based on the results obtained there is no difference in physicochemical parameters of honey samples from the control and experimental group. In all cases testing for doxycycline residues in honey samples from the experimental group was relevant and high. Therefore, the authors do not recommend doxycycline in the beekeeping.

Abstract: 

Background: Acne vulgaris, is a chronic inflammatory disease of the pilosebaceous units. Genetic, environmental factors and nutrition influence on this disease. Recent studies indicate an essential role of nutrition in the development of acne, particularly with the effect on oxidative stress and inflammatory processes. Many studies have reported lower incidence of acne in individuals who receive diets containing large amounts of omega-3. The aim of this study was to determine the probable therapeutic effects of omega-3 for the treatment of acne.Materials and Methods: In this study, 60 patients with moderate to severe facial acne were randomly divided into two treatment groups: 1) 100 mg doxycycline daily and one placebo capsule Omega -3 2) doxycycline 100 mg daily and one capsule 1000 mg Omega -3. Acne severity was determined based on GAGS system at the beginning of treatment and at the end of the sixth and twelfth weeks of treatment, with photographs obtained from each patient.Results: There was a significant reduction in acne severity score (GAGS) at the end of sixth and twelfth weeks in the Omega-3 group in comparison to control group that received placebo.Conclusion: Omega-3 fatty acids can improve moderate to severe inflammatory acne, probably due to their anti-inflammatory properties.
