چکیده: 

دررفتگی کشکک یکی از مشکلات رایج ارتوپدی در سگ هاست. هم سگ های نژاد کوچک و هم سگ های نژاد بزرگ می توانند درگیر بشوند، همچنین این بیماری در گربه ها نیز ممکن است دیده بشود. دررفتگی به سمت میانی نسبت به سمت جانبی رایج تر است و معمولا در سگ های نژاد کوچک تشخیص داده می شود. دررفتگی کشکک بر اساس شدت تغییرات اتفاق افتاده، به 4 گرید مختلف تقسیم بندی می شود. دررفتگی کشکک یک اختلال مادرزادی یا وابسته به رشد است، اما می تواند بطور ثانویه در اثر ضربه ( که منجر به پاره شدن یا کش آمدن کپسول مفصل و بافت های اطرافی و بی ثباتی رانی-کشککی حاصله) نیز رخ بدهد. تشخیص بر اساس شواهد بالینی بی ثباتی کشکک است، اما تصویربرداری تشخیصی نیز برای بررسی میزان بدشکلی های استخوانی و همچنین تعیین مناسب ترین روش درمان نیاز است. علائم بالینی می تواند از یک سگ تا سگ دیگر تفاوت داشته باشد و صرفا تا حدودی مرتبط با میزان بد شکلی های استخوانی همزمان می باشد. لنگش می تواند پیوسته یا متناوب باشد. درمان جراحی شامل تکنیک هایی است که در بافت نرم و بافت استخوانی قابل انجام می باشد، اما در بیشتر کیس ها ترکیبی از هر دو روش برای اصلاح دررفتگی بکار برده می گردد. نرخ مشکلات بعد از عمل معمولا پایین است و از جمله متداول ترین مشکلات بعد از عمل دررفتگی مجدد و همچنین مشکلات مربوط به ایمپلنت می باشد. پیش آگهی درمان بطور کلی مطلوب است و در عمده بیماران، بعد از عمل، اندام به فعالیت طبیعی خود باز میگردد. این مقاله به توصیف دررفتگی کشکک در سگ ها، شامل تظاهر بالینی، تشخیص و گزینه های درمانی موجود می پردازد.

Purpose: To report the computed tomography features of a case with complete luxation of the globe after a road traffic accident. Case Report: A 35-year-old male presented with pain, loss of vision, and bleeding from the left eye 48 hr after a road traffic accident. The ophthalmic examination of the left upper and lower eyelids showed edema with subcutaneous hematoma, crepitus, and complete blepharoptosis. On retracting the eyelids, the left eyeball was not visible and the patient was not able to perceive light. The left temporal region appeared filled with a soft, palpable globular structure situated beneath the temporalis muscle. A noncontrast computed tomography (NCCT) of the head and orbits showed a comminuted and displaced fracture of the floor, medial, and lateral orbital walls in addition to a displaced tripod fracture of the left zygomatic bone. The intact left eyeball was seen below the temporalis muscle without any optic nerve or extraocular muscle attachment. The virtual reality reconstruction highlighted a contributory supero-temporal defect in the bony orbit, which appeared large enough to accommodate the intact eyeball. Conclusion: The computed tomography of the orbits provided a detailed location of the luxated eyeball and provided guidance in further management of the case.

EPIDEMIOLOGIC STUDIES HAVE SHOWN THAT THE INCIDENCE OF DENTAL TRAUMA RANGES FROM 25% TO-58% AND THE MOST COMMON AGE GROUP AFFECTED IS CHILDREN 8 TO 12 YEARS. Even though globe luxation is a quite rare eye problem observed in the pediatric emergency department, it is quite vital since it causes loss of vision. An 8‑, month‑, old male patient presented with the complaint of spontaneous eye protrusion. Eye manipulations that would increase intraocular pressure were avoided and the eye was quickly covered with a dressing to keep the eye wet. The eyeball was placed back performing lateral canthotomy. Unlike the other cases in the literature, the present case is a case of spontaneous and recurrent globe luxation in an infant with a syndromic facial appearance. Early diagnosis and management of the cases will prevent the development of complications.

Accurate diagnosis, immediate care and proper treatment planning are important factors for the successful treatment of dental traumatic injuries. In extrusive luxation, postponement in treatment may lead to the need for new strategies for the resolution of unwanted consequences. The present case report describes an unusual condition of delayed treatment regarding extrusive luxation of two anterior teeth treated by intentional replantation. An 18-year-old female patient attended the Dental School for the treatment of a traumatic injury to the anterior teeth 5 days after a fall from a bicycle. Clinically, teeth #21 and #22 were extruded in incisal edges for 5 mm from their neighboring teeth, had edematous gingiva, showed grade II mobility and were painful to percussion. Radiographically, the roots were intact and the periodontal ligament space was thickened along its entire length. As immediate repositioning could not be performed, intentional replantation was recommended. After detaching the periodontal ligament, the teeth were extracted and the alveolus was curetted and irrigated with saline solution to remove the already-formed clot. After replantation, the teeth were restrained and the patient was medicated with antibiotics as well as analgesics. After 10 days, the splint was removed, the root canals were instrumented and then, filled with calcium hydroxide. After 30 days, they were completely obturated using gutta-percha. The 5-year follow-up showed root integrity, absence of mobility and normal periodontium. The outcomes of the current case report revealed that when intentional replantation was properly conducted, it could be considered an option for the treatment of extrusive luxation where the teeth were not to be immediately repositioned.

Globe luxation resulting from traumatic incidents is an uncommon occurrence, with bilateral cases being particularly rare. This report presents a rare case of severe sinonasal trauma with bilateral globe luxation following a traffic accident. A 15-year-old boy was admitted with multiple traumas. He was riding a vehicle without helmet. A notable brain pulsation was observed in a wide and deep cut at the glabella. Bilateral globes were completely dislocated without perforation, and both pupils were dilated and nonreactive. Computed tomography scan showed multiple fractures of ethmoidal and nasal bones, in addition to the medial wall of the left orbit. The globe repositioning was performed after the decompression of the orbital wall. The patient remained stable after four days, with no occurrence of meningitis or cellulitis. However, complete optic nerve avulsion resulted in no light perception. Bilateral tarsorrhaphy was conducted to safeguard the globes. In conclusion, managing life-threatening conditions stakes precedence in cases of bilateral optic nerve avulsion.

This case report documents the clinical approach adopted for two maxillary incisors with intrusion and lateral luxation trauma. The proposed procedures involved reposition of both teeth, one of them with surgical extrusion and endodontic procedure for both, with 10 years of follow-up.
