Abstract:

PROGESTERONE induced dermatitis is a rare disorder. It typically occurs in females due to an autoimmune phenomenon to endogenous PROGESTERONE production, but can also be caused by exogenous intake of a synthetic progestin. Here in, we present a case of autoimmune PROGESTERONE anaphylaxis (AIPA) observed in an adolescent female. The patient is an 18-year-old Caucasian female with no significant past medical history and no prior exogenous hormone use, who presented to her primary care physician complaining of cyclic skin eruptions with dyspnea, cough and respiratory distress. She noted that her symptoms occurred monthly, just prior to her menses. An intradermal skin test using 0.1 cml of PROGESTERONE was performed. The patient developed a 15mm wheal after 15 minutes, confirming the diagnosis of AIPA. The patient was started on a continuous regimen of an oral conjugated estrogen (0.625mg). The skin eruptions and respiratory symptoms have not returned since the initiation of this therapy.Autoimmune PROGESTERONE dermatitis manifests via the occurrence of cyclic skin eruptions. Women with the disorder commonly present with dermatologic lesions in the luteal phase of the menstrual cycle, if there are any other organ involvement in addition to skin (e.g. lung, GI) the reaction should be called as autoimmune PROGESTERONE anaphylaxis. Diagnosis of AIPA is confirmed by performing a skin allergen test using PROGESTERONE.

Abstract:

The aim of this study was to evaluate the effect of replacing PROGESTERONE injections by PROGESTERONE vaginal inserts on productive and reproductive parameters of lactating cows with reproductive problems after prolonged periods of heat stress. Fifteen Holstein cows were randomly assigned to one of three treatments: 1) treated with two PROGESTERONE vaginal inserts (P4-INS); 2) treated with PROGESTERONE and estradiol via injections (P4-INJ) and 3) control, cows with gestation and parturition normally. All cows were subjected to a Presynch-Co-Synch timed artificial insemination protocol and then inseminated at 71 d in milk. Concentration of P4 in (P4-INS) and (P4-INJ) cows during the first 7 d of induction of lactation was similar (P>0.05) and then increased linearly in both groups. Adjusted milk yield, total milk yield, and average daily milk yield were similar (P>0.05) in treated cows, but cows from (P4-INJ) group produced less milk (P<0.05) than control cows. Days in milk and peak milk yield were similar (P>0.05) among all groups. Per-centage of milk fat and protein, as well as days to peak milk yield were higher (P<0.05) in both treated groups compared to control. Body condition score, service per conception, conception to first service, total conception, days open and culling rate at the end of lactation were similar (P>0.05) among treatments. PROGESTERONE administration by injection or by vaginal insertion resulted in similar production and reproduction parameters of dairy cattle with induced lactation.

Abstract:

Background: Supplementation of luteal phase with PROGESTERONE is prescribed for women undergoing routine IVF treatment. Objective: The objective of this study was to compare the efficacy of three types of PROGESTERONE on biochemical, clinical and ongoing pregnancy rates and abortion and live birth rates.Materials and Methods: A prospective randomized study was performed at Royan Institute between March 2008 and March 2009 in women under 40 years old, who use GnRH analog down-regulation. One hundred eighty six patients in three groups were received PROGESTERONE in oil (100 mg, IM daily), intravaginal PROGESTERONE (400 mg, twice daily) and 17-a hydroxyprogestrone caproate (375mg, every three days), respectively. Results: Final statistical analysis after withdrawal of some patients was performed in 50, 50 and 53 patients in group 1, 2 and 3 respectively. No differences between the groups were found in baseline characteristics. No statistical significance different was discovered for biochemical, clinical and ongoing pregnancies. Although the abortion rate was statistically higher in group 1 (p=0.025) the live birth rate was not statistically significant between the three groups. Conclusion: The effects of three types of PROGESTERONE were similar on pregnancies rate. We suggest the use of intravaginal PROGESTERONE during the luteal phase in patients undergoing an IVF-ET program because of the low numbers of abortions, and high ongoing pregnancy rates.
