Abstract:

Background: Previous literatures have shown a Transient ischemic attack (TIA) mimic rate of 9-31%. We aimed to ascertain the proportion of stroke mimics amongst suspected TIA patients. Methods: A prospective observational study was performed in Ghaem Hospital, Mashhad, Iran during 2012-2013. Consecutive TIA patients were identified in a stroke center. The initial diagnosis of TIA was made by the resident of neurology and final diagnosis of true TIA versus TIA mimics was made after 3 months follow-up by stroke subspecialist. Results: A total of 310 patients were assessed during a 3-month period of which 182 (58.7%) subjects were male and 128 (41.3%) were female. Ten percent of the patients was categorized as a TIA mimic. The presence of hypertension, aphasia, duration of symptoms, and increased age was the strongest predictor of a true TIA. Migraine was the most common etiology of stroke mimic in our study. Conclusion: It seems that many signs and symptoms have low diagnostic usefulness for discrimination of true TIA from non-cerebrovascular events and predictive usefulness of any sign or symptom should be interpreted by a stroke neurologist.

Abstract:

The purpose of this paper is to analyze the flow field structure in Transient state and performance of secondary injection system for thrust vectoring in divergent section of a two-dimensional nozzle. Secondary injection for thrust vectoring in a two-dimensional nozzle is studied by solving threedimensional Reynolds-averaged equations by means of fluent solver. Spalart-Allmaras model was used to model the fluid behavior near the walls. Density-based solver and explicit formulation are employed in the computational model. Results show that the solution of interfered field in the Transient-state is more accurate than steady-state, especially in the initial injection. In addition, various testing showed that the maximum side force would be in the injection angle of twenty degrees and with increasing pressure ratio, we have more side force. At the end it was observed that by sketching the exit gases deviation according to time, we could be informed of desired secondary injection time in order to achieve required deviation around pitch axis, and the required force to achieve desired deviation angle. The innovation of this paper is the solution of interfered field in Transient state, and of course the injection from the optimal point.

Abstract:

Introduction: Hydatid disease has a high incidence in the countries of the temperate zone such as Iran. Only few cases in the head and neck region have been reported in literature. We described a rare case of infratemporal fossa echinococcus granulosus in a thirteen-year-old girl from an endemic area (Dashte Moghan) in Iran.Clinical findings: A thirteen-year-old girl with a right sided frozen eye and facial swelling was admitted in our hospital. In the obtained CT scan there was a cyst in the right infratemporal area. She had a past history of brain hydatid cyst. The cyst was resected and the histological diagnosis was hydatid cyst. The liver was not affected in this patient. Conclusion: Infratemporal fossa is a rare location for hydatid disease and we should consider the diagnosis for any cystic lesion of the infratemporal fossa in an endemic area.

Abstract:

Nowadays, to having greater protection of the environment and due to the pollution caused by using fossil fuels and the limitation of these resources, the global orientation towards is renewable energy sources. As a sustainable form of energy, solar energy is really important, because that is almost the cheapest source of energy production in areas with high solar radiation potential; so global demanding of electricity generation can be supplied with using this technology. There is possibility of direct or indirect lightning strike on photovoltaic system equipment due to the installation of this equipment outdoors or on rooftops. In this paper, the Transient overvoltage crated by direct and indirect lightning strike to the photovoltaic frame in different parts of the photovoltaic system is investigated by an accurate modeling of the frame, pillars and ground of a photovoltaic system in EMTP-RV.

Abstract:

The prediction methods for steady-state performance of switched reluctance motors (SRMs) have been in place for some time; however, little attention has been paid to the Transient analysis of the machine. This paper reports the effects of various faults, loading and unloading, as well as starting on the Transient behavior of SRM. Results are presented to illustrate that faulty conditions will not produce serious problems in the operation of SRMs.

Abstract:

Mumps is a contagious viral disease that presents as unilateral or bilateral parotid enlargement. The complications of the disease is widespred involvement of different organs from central nervous system to pancreas. This is a report of a 10 year old boy who was admitted due to mumps meningo encephalitis. Brain CT scan was done due to persistence CNS presentations, which showed an obvious hydrocephalus. The signs and symptoms relieved acetazolamid and dexametazone. The consciousness became normal and hydrocephaly also decreased.

Abstract:

Epstein-Barr virus (EBV) is one of the most important causes of hemophagocytic syndrome. We report a 76-year-old man who presented with pneumonia like symptoms and pleural effusion following upper respiratory tract infection. He underwent thoracentesis and pleural fluid cytology revealed large number of histiocytic macrophages that had phagocytosed RBCs and other inflammatory cells like lymphocytes and neutrophils. Pleural fluid analysis showed Epstein-Barr virus Antigen (EBNA) as the causing agent and corticosteroid therapy was initiated. A few days later, pleural effusion subsided and further cytologic examinations revealed no trace of hemophagocytosis.
