Introduction: CONCHA bullosa is the most common variation of the middle turbinate of the paranasal sinuses. When it causes nasal obstruction, osteomeatal unit dysfunction, or rarely chronic infection, surgery is required. Case Report: We present a fungal infection of CONCHA bullosa, which is a rare indication for surgery of the CONCHA bullosa. A 59-year-old female patient presented with hemifacial pain on the right side, which had lasted for 2 months. There were no pathological findings in her endoscopic nasal examination. Advanced examination by paranasal computed tomography (CT) revealed bilateral CONCHA bullosa variation and soft tissue density in the right CONCHA bullosa. As the biopsy taken from CONCHA bullosa demonstrated fungal hyphae, endoscopic surgical treatment was performed. Conclusion: We stress the importance of the CT in hemifacial pain by this rare case report, in which endoscopic nasal examination was normal. Fungal infection in the CONCHA bullosa is rare, and infected CONCHA bullosa is a pathology to be considered in the differential diagnosis in patients with complaints of hemicranial headache.

Dermoid cysts are congenital anomalies derived from ectoderm. They are rare in head and neck region and especially in pinna. We presented an 8-year-old girl with a dermoid cyst in the CONCHAl region of auricle which was treated with complete surgical resection.

مقدمه: انسداد مجرای خروجی سینوس ها سبب تجمع ترشحات موکوسی اپی تلیوم تنفسی در داخل آنها شده که موکوسل سینوس نامیده می شود.معرفی بیمار: گزارش حاضر یک مورد موکوسل کونکابولوزا در دختر جوان 18 ساله ای است که بدلیل انسداد نسبی سمت راست حفره بینی از 8 ماه قبل و انسداد کامل طی 2 ماه اخیر مراجعه نموده است. بیمار با تشخیص احتمالی موکوسل کونکابولوزا تحت عمل جراحی قرار گرفت.نتیجه نهایی: بررسی پاتولوژی از نمونه برداشت شده حین عمل موید موکوسل بود. بعد از عمل هیچگونه عارضه ای گزارش نشد. 2 ماه پس از عمل در معاینه مجدد بیمار بینی باز بوده و عارضه ای مشاهده نشد.

Introduction: CONCHA bullosa is a usual anatomical variation of the nose and paranasal sinuses that can compromise their anatomy and function and cause such symptoms as headache, rhinosinusitis, and nasal congestion. Septal perforation is the defect of the septum that brings nasal cavities in communication and is usually caused by trauma. No septal perforation has been reported due to CONCHA bullosa. Case Report: The case of this report was a 15-year-old female who presented with long-term nasal obstruction due to a huge CONCHA bullosa in her right nasal cavity which perforated nasal septum. She lacked a history of any nasal septum surgery, drug abuse, cocaine, or long-term nasal spray. Endoscopic surgery was performed and our patient’, s symptoms rapidly diminished after the surgery. Negative results were obtained for tuberculosis, Wegener’, s granulomatosis disease, and malignancy. Conclusion: We believe that the CONCHA bullosa presented in this case might have caused chondrocyte apoptosis due to mechanical pressure and led to septal perforation. Although uncommon, providers should be aware of the possibility of septal perforation in cases with CONCHA bullosa when planning to perform surgery on patients with similar pathology.

Objectives CONCHA bullosa is the most common anatomical variation of the middle turbinate, which can be associated with symptoms such as headache, nasal congestion, and loss of smell. This study aimed to evaluate the frequency of CONCHA bullosa and its association with osteomeatal anatomical variations in an Iranian population using cone-beam computed tomography (CBCT). Methods This retrospective study evaluated CBCT images of 99 patients. Presence of CONCHA bullosa, nasal septal deviation and paradoxical middle CONCHA was evaluated. The frequency of these anatomical variations and their associations were analyzed. Results The study included CBCT scans of 60 women (60. 6%) and 39 men (39. 4%). According to the results, 57 cases (57. 6%) of nasal septal deviation were observed. CONCHA bullosa was present in 51. 5% of the cases (51 cases). Fifty-one patients (51. 5%) had evidence of CONCHA bullosa located in the right CONCHA (23. 5%), involving the left CONCHA (39. 2%), and bilaterally distributed (37. 2%). There was no significant difference between males and females regarding the presence of CONCHA bullosa (P=0. 092). The relationship between the presence of CONCHA bullosa and nasal septal deviation was not statistically significant (P=0. 796). Conclusion The presence of nasal septal deviation and CONCHA bullosa in this study seemed to be in accordance with prior studies. The presence of CONCHA bullosa was not significantly associated with the presence or laterality of nasal septal deviation.

Backgroud/Objectives: In recent years endoscopic sinus surgery has been established as a successful treatment of chronic sinusitis and more attention has been given to CT evaluation of the paranasal sinuses. CONCHA bullosa is defined as aeration of the middle turbinate. It is one of the most important anatomic variants that may cause inflammatory sinus disease by narrowing of the middle meatus. The incidence of CONCHA bullosa has been reported from 4-80% in different studies. The goal of this study is to determine the incidence of CONCHA bullosa and a few other significant sinonasal anatomic variants in symptomatic patients and to correlate with the presence of inflammatory sinus disease as detected on CT. Materials and Methods: Coronal CT scans of the sinuses were evaluated in 174 consecutive symptomatic patients. The symptoms included postnasal discharge and tenderness over the facial sinuses during the past two weeks prior to examination. Results: The mean age of patients was 30.6 years (from 7 to 66 years); and there were 95 (54.6%) males and 79 (45.4%) females. CONCHA bullosa was present in 113 (64.9%) patients. It was bilateral in 45 (39.8%) and unilateral in 68 (60.2%) patients. In cases with unilateral CONCHA bullosa, the left to right ratio was 1.7 to 1. Inflammatory disease of the sinuses was identified in 81 (71.7%) patients with CONCHA bullosa and 35 (57.7%) patients without CONCHA bullosa. The incidence of inflammatory disease of the sinuses was higher when there was coexistence of CONCHA bullosa and deviation of the nasal septum. The most common symptom was headache (71.8%), and the least common was fever (27.9%). Tenderness over the frontal and maxillary sinuses, postnasal drip and fever were more common in patients with CONCHA bullosa. Conclusion: CONCHA bullosa is associated with higher incidence of inflammatory sinus disease. With simultaneous CONCHA bullosa and nasal septal deviation, the incidence of inflammatory disease increases.

Background: Rhinogenic origin is an important source for headache, which may be treated by medical or endoscopic intervention. An aim of this study was to clarify whether the surgical or medical intervention is superior.Materials and Methods: In this randomized double blind clinical trial study, 44 patients (19 male and 25 female) with periorbital or frontal pain were enrolled. Patients were divided into 2 groups of surgical or medical intervention randomly. Medical group received 3 courses of 1.5 months 125 mg per puff, fluticason nasal spray (2 puffs Q 24 hours in each side), and oral Pseudoephedrin 30 mg Q 8 hours with 2 weeks intervals. Surgical group underwent turbinoplasty with functional endoscopic sinus surgery approach. Duration (per hour), frequency (per week) and severities of the headaches were measured by Visual Analog Scale (VAS) before treatment, and at 1.5, 3 and 6 months after institution of treatment by an examiner, who was unaware of the patients’ treatment plan.Results: Before treatment, chronicity (P =0.980), severity (P=0.742), frequency (P=0.730), and duration (P=0.603) of the headaches were not significantly different. The severities of the headaches in surgical group were significantly lower at 1.5, 3 and 6 months (P<0.001), also the frequencies and the durations of the headaches were significantly lower at 6 months after an institution of treatment compared to medical group (P=0.027, P=0.008, respectively).Conclusion: Turbinoplasty in chonca bullusa patients is an acceptable and a simple procedure for relieving pain in rhinogenic headaches, compared with medical treatment.

The chondrocutaneous sandwich of the external ear has a delicate and complicated design, making partial ear reconstruction a challenging task for plastic surgeons. In this study, 12 patients who had surgery utilizing the CONCHAl rotational flap and grafting approach were monitored for 1 year after surgery. The upper third of ear deformities have been repaired using a cartilaginous composite flap. Following the procedure, a year later, there were no signs of a seroma, infection, hematoma, deformity, or flap ischemia. All patients had pleasing cosmetic outcomes. The findings of this study suggest that full-thickness upper third auricular abnormalities be repaired utilizing this combination method.

Background: The ostiomeatal complex (OMC) is not a separate anatomical structure although it is a functional unit of structures, including the middle meatus, uncinate process, infundibulum, maxillary sinus ostium, ethmoidal bulla, anterior ethmoid sinus ostium, and frontal recess. CONCHA bullosa is the pneumatization of the CONCHA, which is one of the most common anatomical variations in the middle turbinate. Methods: This study was conducted using the cone-beam computed tomography (CBCT) images of 172 patients in the archives of the Department of Oral and Maxillofacial Radiology, Dentistry School, Ahvaz Jundishapur. Patient information including age and gender, presence or absence of CONCHA bullosa, the involved side (left or right), and its type (i. e., extensive, lamellar, and bulbous) were collected in the information form. Finally, the chi-square test (with SPSS, version 22) was used to analyze the data, and P value less than 0. 05 was considered statistically significant. Results: Patients with and without CONCHA bullosa were 39. 1 and 41. 7 years, respectively, but it was no significant difference in terms of age (P = 0. 321). Out of 52 patients with CONCHA bullosa, 19 (36. 5%) cases were males and 33 (63. 5%) of them were females. The prevalence of CONCHA bullosa was higher for the bilateral side (20 patients, 38. 5%, P = 0. 000). The prevalence of bulbuls-and lamellar-shape was nearly the same (32. 7% and 30. 8%, respectively). Eventually, the extensive shape with 36. 5% was more frequent for the shape of CONCHA bullosa (P = 0. 000). Conclusions: The prevalence of CONCHA bullosa was high. There was no significant difference in terms of age (P = 0. 321) and gender (P = 0. 058) of patients with CONCHA bullosa. The extensive type and the bilateral appearance of CONCHA bullosa were more significant (P = 0. 000).

Introduction: Although ectopic eruptions of teeth into the nasal cavity have been previously reported, the eruption of a tooth into the turbinates of the nose is quite rare. This report describes a case in which a tooth ectopically erupted into the inferior turbinate of the nose. Signs and symptoms of the occurrence, as well as its diagnosis and treatment, are also delineated in this case report.Case Presentation: A 32-year-oldman with swelling in the left paranasal area of the face was referred to the department of Oral and maxillofacial surgery for diagnosis and treatment. A diagnostic workup resulted in the diagnosis of an ectopic tooth eruption. The tooth was removed via the use of a transoral approach and without any complications.Conclusions: This case report alerts surgeons to consider the possibility of an eruption of a tooth into the nasal cavity or inferior turbinate of the nose when panoramic radiography reveals an impacted canine tooth located high in the maxilla.
