With the advent of increasing molecular characterization of individual differences, it has become ever more important to understand how we might wisely individualize our approach to hormone use. The need to individualize, in turn, has spurred an interest in determining and understanding more about how ESTROGENS differ. From the clinical point of view, we know that not all women are the same and we know that not all ESTROGENS are the same. But which of the differences between women or hormones are the ones that really matter when making clinical decisions about birth control or menopause management?When transdermal estradiol formulations for menopause management became available in the early 1990s, the dogma that all ESTROGENS were equivalent was called into question. It was recognized that the transdermal route of administration of estradiol had less hepatic impact than the oral route and that this might confer benefits, including a reduced risk of venous thromboembolism. This line of reasoning seemed to make sense until a recent report (Cole et al. Obstet Gynecol 2007; 109: 339-346) showed that the transdermal contraceptive patch leads to higher rates of venous thromboembolism than oral contraceptives. Taken together, these observations beg the question as to whether route of administration of ESTROGENS does indeed matter.To dissect these findings, we must consider the pharmacologic attributes of different estrogen preparations and the physiologic mechanisms that are influenced by type, route of administration, and dosage of estrogen formulations. The goal is to illustrate the concept that not ESTROGENS are the same and to highlight our knowledge gaps. We will likely be afforded a more clinically relevant perspective about the clinical differences between ESTROGENS that are more chemically similar as we learn more about how each differentially harnesses intracellular machinery, such as protein chaperones.

Clomiphene citrate (CC) is a nonsteroidol estrogen agonist/antagonist, effective in inducing ovulation in anovulatory women. Over the years the lower-than-expected pregnancy rate has mainly been attributed to a prolonged anti-estrogenic effect on the endometrium. CC has been shown to have predominantly estrogen receptor (ER) antagonistic effects in the uterus. PhytoESTROGENS (PE) possess estrogenic activity and are of increasing interest for their possible influence on the physiology of the reproductive tract. Several reports have suggested that some of the effects of PE are mediated by their ability to bind the ER. In a first study, we indicated that a prolonged treatment with PE resulted in an increased incidence of endometrial effects. This means that they bind to ERa and ERp and act as an agonist. Therefore, while CC has predominantly ER antagonist activity in the uterus, specifically in the endometrium, PE has an agonistic activity. We showed that the addition of a high dose of PE to CC-treated patients resulted in endometrial proliferation, reversing the deleterious effects of CC on endometrium and contributing to higher pregnancy rate. Progesterone supplementation of the luteal phase is routinely prescribed to women undergoing IVF, but the therapeutic need to support the luteal phase with ESTROGENS remains debatable. According to above considerations, we proposed an improvement of implantation in luteal phase PE-supplemented IVF cycles. Results seem to emphasize a positive action of PE at a high dosage on the outcome of IVF-ET when a down-regulation protocol was used. Our findings suggest new avenues for future fertility research and treatment with PE, and strengthen the importance of investigating the features of estrogenic action on endometrium before implantation.

THE ESTABLISHED ROLE OF ESTROGEN IN THE DEVELOPMENT AND PROGRESSION OF BREAST CANCER RAISES QUESTIONS CONCERNING A POTENTIAL CONTRIBUTION FROM THE MANY CHEMICALS IN THE ENVIRONMENT WHICH CAN ENTER THE HUMAN BREAST AND WHICH HAVE ESTROGENIC ACTIVITY. ALTHOUGH BREAST CANCER MAY NOT BE A NEW DISEASE, THE INCIDENCE OF FEMALE BREAST CANCER HAS RISEN WORLDWIDE IN UNPRECEDENTED TERMS IN RECENT DECADES. EPIDEMIOLOGICAL STUDIES SHOW THAT 90% OF BREAST CANCERS ARE ENVIRONMENTAL IN ORIGIN. THE MAIN INFLUENCE IN THE DEVELOPMENT OF BREAST CANCER REMAINS LIFETIME EXPOSURE TO ESTROGEN.A RANGE OF ORGAN CHLORINE PESTICIDES AND POLYCHLORINATED BIPHENYLS POSSESS ESTROGEN-MIMICKING PROPERTIES AND HAVE BEEN MEASURED IN HUMAN BREAST ADIPOSE TISSUE AND IN HUMAN MILK. THESE ENTER THE BREAST FROM VARIED ENVIRONMENTAL CONTAMINATION OF FOOD, WATER AND AIR, AND DUE TO THEIR LIPOPHILIC PROPERTIES CAN ACCUMULATE IN BREAST FAT. HOWEVER, IT IS EMERGING THAT THE BREAST IS ALSO EXPOSED TO A RANGE OF ESTROGENIC CHEMICALS APPLIED AS COSMETICS TO THE UNDERARM AND BREAST AREA. THESE COSMETICS ARE LEFT ON THE SKIN IN THE APPROPRIATE AREA, ALLOWING A MORE DIRECT DERMAL ABSORPTION ROUTE FOR BREAST EXPOSURE TO ESTROGENIC CHEMICALS AND ALLOWING ABSORBED CHEMICALS TO ESCAPE SYSTEMIC METABOLISM. THIS REVIEW CONSIDERS EVIDENCE IN SUPPORT OF A FUNCTIONAL ROLE FOR THE COMBINED INTERACTIONS OF MECHANISMS OF DISRUPTING EFFECTS OF ENVIRONMENTAL HORMONE AND NATURAL HORMONE, COSMETIC CHEMICALS WITH ENVIRONMENTAL ESTROGENS, PHARMACOLOGICAL ESTROGENS, PHYTO-ESTROGENS AND PHYSIOLOGICAL ESTROGENS IN THE RISING INCIDENCE OF BREAST CANCER.

The mammalian testis is a complex organ that serves two important functions, synthesis of steroids, with significant amount of estrogenic hormones produced and production of spermatozoa. Estrogen receptors (ERs) are expressed in cells of the testis as well as the epididymal epithelium. We have demonstrated that estrogen receptor expression is higher in reproductive tissues as compared to non-reproductive tissues. In addition to this, western blot analysis showed a signal for estrogen receptor.

Introduction: Environmental ESTROGENS are the artificial chemical compounds found in our environment. These materials enter human body and the other creatures through various routes and they interfere with endocrine system. The materials have harmful effect on the health of creatures by increase or decrease the activity of this system. The goal of this study is determining the amount of environmental ESTROGENS and the type of their application in the cosmetics.Method: In this study a list of primary compounds of cosmetic cream, aromatics and soaps is preparationed and this primary compounds are evaluated their estrogenic property.Results: 63% of creams, 26% of aromatics have estrogentic property. In the creams the most amounts is in the hair cream that is 76% and the least amount is in the layer remover cream that is 47% for estrogenic property. In the aromatics 46% of sprays have estrogenic compounds. Totally 56.9% of the investigated compounds have estrogenic property.Conclusion: This study has done for more recognition of materials having environmental ESTROGENS that the human expose to them. Because of more and better recognition of materials having environmental estrogen is the first step to limit the contact of human to these potentially dangerous compounds.
