CARPAL JOINT ARTHRODESIS INDUCTION USING CROSS PINS AND BONE MARROW IN DOGS

Abstract

 To evaluate osteogenic property of BONE MARROW for CARP ARTHRODESIS, five healthy mixed breed DOGs with an average age of 5.40±3.78 years and weighing between 23.62±4.34 kg were used. After skin incision in dorsal surface of right CARPal area and exposure of articular surfaces, complete removal of articular cartilage to depth of bleeding subchondral bone of all joint surfaces (radio-carpal, intracarpal and CARPo-metacarpal joints) was achieved by curettage. In order to perform CARPal fixation, two pins in cross manner were introduced in the CARPal joint via metacarpal bones to the radius and ulna. To promote callus formation, derived autologous BONE MARROW from the head of femur was added into and around the contact surfaces of the joints intraoperatively and casting from digits to elbow joint was used for further immobilization. All animals were followed radiographically at 3, 5, 7, 9, 11, 13, 17 and 24 weeks after surgery. The results revealed new bone formation at 3 week and showed continuously increase in radiographs. In four DOGs, complete ARTHRODESIS was achieved at 17 week and in one DOG the outcome was not favorable due to infection. On the basis of these results, it can be concluded that using CROSS PINs and BONE MARROW have provided complete ARTHRODESIS in 80 percent of the cases. Although BONE MARROW is not yet a standard treatment to stimulate ARTHRODESIS, in this experimental study BONE MARROW transplantation has been used successfully to promote ARTHRODESIS in the CARPal joint.
