Abstract: 

Introduction: Among women with breast cancer in Iran, Up to 50 per cent of patients aged less than 50 years. More than 70 per cent of these women experienced an earlier menopause after chemotherapy. Our main aim of this study was effect of citalopram together with aspirin on CIA symptoms in premenopausal women.Methods: In this randomized clinical trial study we allocated individuals randomly to treatment (Citalopram together with Aspirin) and control (placebo together with Aspirin) groups, 28 and 32 patients in the treatment and control group respectively. We selected patients who referred to Breast Cancer Research Center and Shohada Esfahan hospital. To assess functional, practical, emotional, social and hot flashes symptom we used FACR-G questionnaire. As well as we employed Student’s T-test and Wilcoxon signed-rank test to evaluate the effect of treatment on these patients.Results: The mean of age were 45.03 5.1 and 44.7 5.3 in treatment and control groups respectively. Exponentially reduction of the menopausal hot flashes symptom was statistically significant in both groups (p-value<0.0001). In this study there is no significant difference in terms of practical and social parameters (p-value 0.05). Increase of emotional states (p-value=0.2) and physical health (p-value=0.04) among treatment group were significant after using citalopram and aspirin.Conclusion: Our result demonstrated that using citalopram together with aspirin in breast cancer patients with chemical menopause markedly decrease the hot flash levels causing chemotherapy. Also we saw the positive effects of this treatment on emotional and physical health. Thus, our treatment suggest to medical specialist to improve the hot flash symptom.

Abstract: 

Introduction: Breast cancer is the second cause of cancer death in women in the world. Lymphedema is also a chronic and debilitating condition for breast cancer survivors. The main aim of this study was to determine the frequency of patients suffering from upper extremity musculoskeletal problems who are known case of Breast Cancer Related Lymphedema (BCRL).Methods: In this cross-sectional study, thirty women with lymphedema associated with breast cancer surgery were assigned by convenience sampling.To determine upper extremity musculoskeletal problems (Biceps tendonitis, Rotator cuff& Shoulder instability, Impingement Syndrome) a data form was applied including patient’s history? from medical records and also by asking themselves, and orthopedic tests performing by therapist contain six tests in shoulder belt for shoulder pathology, 3 test in elbow for tennis elbow, 3 test in wrist for carpal tunnel syndrome and 2 test in thumb for De Queravin syndrome. The patients were counted as affected, even with one positive test.Results: The results suggest that a 70% incidence of shoulder pathology, 43.3% carpal tunnel syndrome, 20% De Queravin syndrome and 13.7% tennis elbow in the studied patients.Conclusion: Evaluation of musculoskeletal pains can improve the process of treatment and quantity of life for the patients with breast cancer and lymphedema and also can helpful for them, their families and medical staff by reducing the complications of main illness. Our study showed high frequency of musculoskeletal problems in lymphedema patients, especially in shoulder joint and wrist joint. So early diagnosis of these problems in lymphedema patients can increase rhe quality life of them and it seems too important.

Abstract: 

Introduction: Using the information technology in medical centers, today as one of the efficient ways to improve the work processes. Effective design of health information technology (HIT) for patient-oriented service requires consideration of workflow from patient’s perspective, termed ‘patient-oriented workflow’.The purpose of this paper, is documentation of workflow diagram for a Breast Disease Clinic affiliated to Breast Cancer research Center in 2013, and analyzing it in term delays, rush points and human errors in work processes. Then using the facilities and services will be offered ways to improve processes.Methods: This research is a descriptive and observational research. Data collected by using previous researches, systematic interview and continuous observation. Using the results of interviews and observations we plotted flowchart of the center was plotted and then the current workflow problems using the scientific with experts in the field of medical informatics identified and solutions were presented. In the last phase the center’s optimal flowchart was drawn.Results: Problems in breast disease clinic workflow consist of: People rush at presence or by phone scheduling, clinic and radiology services waiting room, Delay in retrieving patient’s records, Delay in presenting lab and radiology services reports and Data entry redundancy and waste of time and energy raised from.To solve above-mentioned problems, we have suggested some health information technology systems, such as Online scheduling services, Electronic Medical Records (EMR) system, Picture Archiving and Communication System (PACS), Online Lab Reporting system, etc.Conclusion: Comparing current flowchart of the center and suggested optimal Chart shows that work processes, by implementation information systems, are facilitated and accelerated. By omitting or shortening processes such as formation and retrieval of records, getting time and report in addition to the accuracy of the services provided, save a lot of time and money for the center as well as the patient.

Abstract: 

Introduction: Breast cancer is the most common women's cancer and as chronic condition with psychological complications such as death anxiety. The aim of this study is evaluate the incidence of death anxiety in women with breast cancer in 2015 in one of the public hospitals of Kermanshah.Methods: This is a descriptive study and we use a two part questionnaire that included demographic characteristics in first part and the second part was Templer Death Anxiety questionnaire contains 15 questions Yes and No. 48 female patients with breast cancer in one of the public hospitals in Kermanshah were selected randomly and data were analyzed using SPSS version 22.Results: The age range of patients was between 29 to 67 years that 93.4% of the patients were married. 79.2% of patients have high death anxiety, that the highest and lowest scores for death anxiety were 15 and 2 and the average scores of the patients was 9.04 with standard deviation of 3.71. Middle-aged people also had the highest death anxiety and as well as patients with poor education and weak economic situation also had higher death anxiety.Conclusion: The results show that there is high Score of death anxiety in studied population which may be due to lack of adequate training to deal with death and death anxiety awareness in patients with cancer in the department of oncology and radiotherapy. According to the results, it is suggested, while more attention to mental health in the same population to improve the quality of mental health of these patients and broader studies done in this issue.

Abstract: 

Introduction: Breast cancer is one of the most common types of cancer among women around the world that like other chronic diseases reduce the psychological well-being. Maintaining and promoting the psychological well-being are in need of psychological intervention in this area. Study the effects of cognitive behavioral group therapy model-based 8-stage Cash to improve psychological well-being of women with breast cancer under Cancer Research Center, Shahid Beheshti University of Medical Sciences has examined.Methods: Method of quasi-experimental study with pretest-posttest control group. For this purpose, 30 patients with cancer and sampling were selected randomly were assigned to two groups of control. All 30 patients completed the questionnaire RPHQ the Psychological well-being. 8 sessions of 90-minute experimental group cognitive-behavioral intervention model based on the 8-stage Cash participated and the control group received no intervention. In the end, both groups were tested again. After collecting the questionnaires, data obtained using analysis of covariance and analyzed by SPSS software version 20 was.Results: Data obtained using single-parameter analysis of covariance showed that cognitive-behavioral therapy-based 8-step pattern Cash improving psychological well-being after the test phase has been statistically significant effect (p<0.005) and its effects to the follow-up stage remains.Conclusion: The results showed that cognitive-behavioral intervention model based on the 8-stage Cash through challenging irrational beliefs about their patients and improve psychological well-being of these beliefs are correct. Therefore, in line with current medical treatments, this intervention can be used to enhance psychological well-being of breast cancer patients.

Abstract: 

Introduction: Phyllodes tumors of the breast (PTs) are fibroepithelial neoplasms that account for less than 1% of overall breast neoplasm. PTs are palpable mass, they growth rapidly and tumor size is greater than 2 cm at diagnose time. The scope of this tumor can be benign tumors to malignant sarcoma. Because of the low prevalence of this type of tumors, limited studies have been conducted in evaluating clinicophatologic characteristics of PT. the aim of this study was to report diagnosed cases of phyllodes tumor.Methods: in this retrospective study, information was identified from the database of laboratory of breast cancer research center. The inclution criterion was patients who diagnosed with PT. The patient’s demographic, histopathological and therapeutic characteristics were evaluated. Data were analyzed by descriptive statistic (mean and standard deviation) using spss 19.Results: a total of 16 patients were found with PTs in breast cancer research center between 2011 to 2015. The mean age of patients was 43±10.78 (range 21-58) and all the patients were female. The chief complaint of patient was mass in 12 patients (75%), pain in 2 patient (12.5%) and pain and mass at the same time in 2 patients (12.5%). The right and left breast were affected 12 patients (75%) and 4 patients (25%) respectively. The mean size of the tumor was 4.7 cm (range 1 to 10 cm). post operative histology was fibroadenoma phyllodes in 5 patients (33.3%), benign phyllodes in 5 patients (33.3%), malignant phyllodes in 3 patients (20%), fibroadenoma in 1 patients (6.7%) and dysplastic carcinoma in 1 patients (6.7%). Radiation therapy was performed in 4 patient post operations. The mean of follow up time was 13 months.Conclusion: Phyllodes tumors were benign or malignant masses and they are potentially invasive. Preferred treatment is wide local excision with a margin of at least 1 cm. Radiation therapy can prevent recurrence.
