Abstract: 

Background and Aim: Diagnosis is an implement of organizational development. The main objective of this study was to diagnosis the TUMS by using Six Box Model of the Weisbord to identify in which areas of this model Purposes, Structure, Leadership, Rewards, Relationships, Helpful Mechanisms and Attitude toward Change is more vulnerable.Materials and Methods: The research was descriptive and the statistical population included five hospital staff subset of TUMS. Out of 280 questionnaires we received 220 completed ones. For data gathering we used standard questionnaire of the Weisbord which was estimated as both reliable and valid. By the use of Cronbach alpha coefficient, reliability coefficients was obtained equal to 0.85. Also, questionnaire was confirmed by 5 faculty members in terms of nominal and content validity. The data were analyzed by SPSS software.Results: All the variables except rewarding and Attitude toward Change had significant changes p<0.05 thus the H0 for Purposes, Structure, Leadership, Relationships and Helpful Mechanisms were rejected. These variables stay in acceptable condition. However there was a significant difference between Relationships and rewarding; considering their T-Value it could be concluded that Relationships situation was desirable (T=3.62) but rewarding situation was undesirable (T=5.70(.Conclusion: The findings conclude that TUMS, currently the most appropriate situation was in communication and the most inappropriate situation was rewarding. Also other aspects are between these two conditions. Also, rewarding area needs planning to increase employee satisfaction.

Abstract: 

Background and Aim: More than 80% of patients need to take x-rays during their diagnosis and treatment stages. Due to the biological effects of ionizing radiation, observance of radiation protection principles are necessary. The observance of radiation protection principles in intensive care units at Imam Reza hospital of Kermanshah in 2014 was evaluated in this study.Materials and Methods: In this cross-sectional study which was performed by census method, performance of 38 radiographers working in target center was investigated regarding the following of the principles of radiation protection in respect of patients, nurses, radiographers, and other clients in intensive care units. Data collection tool included a questionnaire and a checklist of 31 principles of radiation protection.Results: The overall level of observance of radiation protection principles in intensive care units was at an appropriate level (94.7%) in 36 radiographies. The highest and lowest levels of principle observance of radiographers were in respect to themselves (94.7%) and nurses in the intensive care units and other clients (26.3%), respectively.Conclusion: According to results in this study, radiation protection principles observance by radiographers during portable radiography examinations respect of themselves is appropriate but respect of other nurses is not satisfactory. It seems that it can be improved to 100% of the optimum level by equipping intensive care units to protective equipment, setting some obligations to use them, and controlling and continuous monitoring of protective measures of radiographers.

Abstract: 

Background and Aim: The objective of this study was to design electronic card of health for schizophrenic patients to better manage their clinical information. Materials and Methods: The study was conducted year 2014. The population of the study included 40 psychiatrists. The information, based on minimal clinical and demographic data, was gathered through field study and survey using a questionnaire. Referential and inferential statistics techniques were used to analyze the data. To schizophrenic patients' electronic health cards, various software were studied to design database, card and card reader.Results: According to the results and observed frequencies, it was confirmed that the components of demographic and clinical information be inserted in the electronic card of health. The project includes hardware of card reader machine, card and visual studio software. Microsoft SQL Server, 2008, was applied to design database.Conclusion: The electronic card of health for schizophrenic patients helps the treatment team to provide effective health care as well as medical records and hospital admission's staff in better management of patient information. It also reduces the problems of relatives and family members of the patients in the medical centers and facilitates, and the process of treatment of schizophrenic patients significantly. Using schizophrenia patient’s electronic health card, hospital in better management of patient information puts in good condition.

Abstract: 

Background and Aim: Job Burnout syndrome consists of emotional exhaustion, depersonalization, and reduced personal accomplishment. The health system personnel to deal with stress because of physical, mental, and emotional intensity are at risk of burnout.The aim of this study was to determine the relationship between burnout and general health of the financial staff hospitals of Tehran.Materials and Methods: In this cross-sectional study, the sample size consisted of 150 financial administrative personnel employed by university hospitals of Tehran University of Medical Sciences which were selected by random sampling. The instrument was included demographic information, general health questionnaire and the Maslach Burnout questionnaire, which was already confirmed for its validity and reliability.Results: The results showed a significant correlation (p<0.001) between burnout and general health therefore public health was reduced with increased burnout. There was also a significant relationship (p<0.05) between burnout and satisfaction with work and partners. But there was no significant relationship between age, gender, work experience, marital status and income.Conclusion: Regarding the significant relationship between burnout and public health in this study, the use of improved working conditions and the promotion of mental health personnel is recommended.

Abstract: 

Background and Aim: In attention to pressing need to cost containment and service efficiency, laboratory services serve as a central locus for controlling health care expenditure. This study intents to investigate proportion of laboratory utilization in Iran to pave the way for future interventions.Materials and Methods: A cross-sectional study was conducted to investigate the proportion for laboratory utilization through the retrospective analysis of 384 medical records at the tertiary care Kashan University Hospital Shahid Beheshti. To pave the way for future intervention, over utilization tests were classified into two categories, inappropriate and resulted in laboratory errors.Results: Nine thousand five hundred forty one laboratory tests were ordered, 2522 (26.40%) of the tests were inappropriate and 143 (1.5%) of the over utilization tests have occurred due to laboratory testing cycle errors. According to reviewers’ judegment relevancy to medical intervention was considered as the most accepted appropriateness criteria 5012 (52.53%) for laboratory tests.Conclusion: Since, the laboratory tests over utilization is multifactorial, forming a multidisciplinary team including physicians, nurses, laboratory staff, medical education experts, health care management and health information technology staffs in terms of Utilization Committee to develop more targeted strategy based on root cause analysis of over utilization behavior seems necessary.

Abstract: 

Background and Aim: Growing the number of private institutions offering health care and fierce competitive market conditions in the private hospitals industry cause to pressure these hospitals to provide high-quality services. The purpose of this study was to examine the effect of service quality on patient satisfaction in private hospitals of the Guilan province.Materials and Methods: Based on standard model of Buyukozkan et al (2011) a new dimension professionalism to SERVQUAL model was added that has used six dimensions including tangibles, reliability, responsiveness, assurance, empathy and professionalism as the dimensions of service quality. The number of samples was 360 patients hospitalized in four private hospitals and a standard questionnaire was used to assess the quality of services. Pearson Correlation Coefficient and Simple and Multiple Linear Regression was used to analyze the data and then by using ANOVA and Duncan’s and by Fuzzy Analytical Hierarchy Process technique, ranking of private hospitals was performed.Results: Four dimensions: tangibles, responsiveness, empathy and professionalism on patient satisfaction have been found to be effective while Components of trust and assurance in private hospitals of the Guilan province had no significance effect. Conclusion: Based on the effects of service quality dimensions except trust and assurance recommend that hospitals can increase patient satisfaction if they pay special attention to patient in empathy, updating facilities and technology in tangibles, fast response to patient in responsiveness, employing professional and skilled human resources in professionalism dimensions.

Abstract: 

Background and Aim: Chronic myeloid leukemia (CML) is a myeloprolifrative neoplasm that is characterized by an expansion of myeloid, erythroid cells and platelets in peripheral blood and myeloid hyperplasia in bone marrow. Secreted frizzled-related protein family is a negative regulator of the Wnt signaling pathway that suppresses this signaling pathway in healthy individuals. Aberrant regulation of the Wnt signaling pathway is a prevalent theme in cancer biology, and methylation in promoter of SFRP family has been shown to cause uncontrolled cell proliferation in cancer. Chronic myeloid leukemia was the first malignancy in which the important role of Wnt signaling pathway has been described. In the present study, we examined the methylation status of SFRP1 and SFRP2 genes in patients with CML.Materials and Methods: Blood samples were obtained from 25 healthy individuals and 33 patients whit chronic meyloied leukemia (23 male, 10 female) Then Isolated DNA was treated with sodium bisulfite and analyzed by methylation-specific polymerase chain reaction (MSP) with primers specific for methylated and unmethylated promoter sequences of the SFRP1 & -2 genes. We used Mann-Whitney u-tests to investigate the correlation between SFRP-1 and SFRP-2 genes hypermethylation and clinical parameters. Results: In CML patient hypermethyleation frequency of SFRP-1 and SFRP-2 genes were 16.1℅ and 27.2% respectively. In control group SFRP-1 and SFRP-2 genes were unmethylated.Conclusion: The present study showed that, methylation of SFRP genes also occurs in CML like other solid tumors. Therefore, the methylation of these genes may play a role in the initiation of malignant disease.

Abstract: 

Background and Aim: Infertility is a major problem of modern medicine as it affects almost 20% of reproductive-aged couples. The cause of this problem is attributed to the male partner in nearly 40%-50% of these cases. The role of herpes simplex virus (HSV) in male infertility has been investigated using the sensitive methods. The aim of this study was to determine the prevalence of HSV-1, 2 DNA in the semen, blood and urine of idiopathic infertile men and its association with altered semen parameters.Materials and Methods: A total of 150 semen, blood and urine samples from infertile men were collected in the Shariati hospital, Tehran (2012-2014). Sample analysis and diagnostic PCR using specific primers was performed for detection of HSV-1, 2 DNA in the specimens.Results: Analysis showed six groups of infertile men, including HSV-1, 2 positive and negative groups in semen, blood and urine samples. HSV-1, 2 DNA was detected in 38 (18.66%) semens, (6.66%) 10 in blood and 2 (1.33%) in urine. Only HSV-1, 2 positive samples of semen had abnormal semen parameters. Conclusion: Using a powerful molecular method, we detected a high prevalence of HSV-1, 2 DNA in the semen of asymptomatic infertile patients. Although HSV-1, 2 infections were not associated with motility and morphology defects of the sperms, it was related with decreased sperm count in the semen fluid. In addition there was not a significant role for detection of HSV-1, 2 DNA in blood and urine samples of infertile men and abnormal semen parameters.
