Introduction: Medication, especially injection drugs are one of the complex situations and high-risk medications in clinical processes that nurses face. Risk management is one of the recommended method to identify and prevent adverse events from drug errors. The purpose of this study was to evaluate the impact of risk management programs on Preparation of electrolyte concentration skills by nursing staff.Materials and methods: In this experimental study, 50 nurses working in emergency departments that were involved in the process of medication, were selected through census in 2014, then randomly divided into control and test groups. The control group received the routine of the hospital and in the experimental group risk management program was implemented. Preparation of electrolyte concentration skills was examined by valid and reliable researcher check list. Data were collected in pre and post-test with SPSS software, version 21 descriptive and inferential statistics were analyzed. Results: nurses' skill in preparing the condensed electrolyte, before the intervention and control groups, was respectively 96.16±3.2 and 17.17±0.2, and in two weeks after the implementation of risk management programs was respectively 27.27±8.1 and 83.17±2.2. In the pre-test, independent t test, a significant difference was observed between the two groups in the nursing skills (P>0.05). While in the post-test, significant differences in scores between the experimental and control groups were nursing skills (P<0.001). Paired t-test, before and after the intervention, the experimental group showed a significant difference (P<0.001).Discussion and Conclusion: The results showed the effectiveness of the risk management program. Implementation of risk management programs as a way to improve nurses' skills in preparing the condensed electrolytes is recommended.

Abstract:

Introduction: Dialysis is of the important ways in the treatment of the acute and chronic renal failure patients and the urea reduction ratio is used to determine the amount the removal of waste generated by of dialysis. Therefore, the present study has been performed to investigate the effect of priming with recirculation of normal saline method on urea reduction ratio among hemodialysis patients.Material and Methods: This is a randomized clinical trial study that was performed in 522 Military Hospital in Tabriz, Iran in November 2014. A total of 44 hemodialysis patients were recruited by convenience sampling method and randomly divided in two groups. The dialysis machine, in intervention group, with the method of priming with recirculation of normal saline for a month and in the control group the routine method was primed. Before and after intervention, blood sampling was determined to calculate the blood urea nitrogen and urea reduction ratio. Data were analyzed by SPSS version 20 statistical software. Results: The mean and standard deviation of URR was respectively 58.45±6.23 and 69.04±6.24 before and after the intervention in the intervention group which showed a significant increase after the intervention (P<0.001). The mean and standard deviation of URR was respectively 58.27±11.38 and 59.22±7.59 before and after the intervention in the control group there was no significant differences compared to before the intervention (P=0.753).Discussion & Conclusion: The study results showed that can be used from the method primed with saline recirculation dialysis method to increase the urea reduction ratio and with a proper dialysis prevented from many of the complications and with preventing readmission, while saving the cost of health care, providing a better quality of life for these patients.

Abstract:

Introduction: Hospitalization in critical care unit is a stressful issue for patients and families due to the presence of unfamiliar equipment and procedures in the unit. This confusion and unfamiliarity leads to disturbance and even physiological and mental problems in the patients. The main objective of this work is to examine effect of family- centered orientation program on prevention of delirium prevalence in patients with coronary artery disease in CCU.Materials and Methods: In this quasi-experimental work in 2013 in Zanjan, in 3 months, 80 inpatients from CCU meeting the criteria of entering to the research, were selected based on sample and convenience sampling in two test and standard groups. For test group, family- centered orientation program was performed. The samples in both groups were examined in terms of delirium problem using the mini mental status examination (MMSE) instrument. Then, the obtained data were analyzed using the t-test and fisher. Results: Data analysis indicated that delirium incidence in the test group indicated a significant statistical decrease using the T-test with p= 0.002; however this difference was not clinically significant.Discussion & Conclusion: due to the benefits of family- centered orientation programs to decrease of delirium, using of orientation program is recommended but due to the lack of clinically significant about delirium, large samples and several evaluations of patients seems necessary for studies designs.
