Abstract: 

Background: Platelet-rich plasma (PRP) is an autologous product having a high concentration of growth factors well known to stimulate different tissues healing. This study was made to determine the effect of PRP on healing of femoral cancellous bone defect in rabbit.Materials and Methods: The experiment was conducted on 16 male adult New Zealand white rabbits. The animals were divided into the control group and the treated group including 8 rabbits in each group. Under general anesthesia, using a dental drill, a hole 4 mm in diameter 5 mm in depth was made in the inner aspect of the medial condyle of left femor. In the treated group the created hole was filled with autologous PRP. In the control group, the defect was left untreated. The bone healing was evaluated histopathologically at the end of the first and the second month post surgery. In addition, the samples were assessed histomorphometrically 2 month post surgery. Results: In the control group, the defect seemed to be filled with adipose tissue including sparse hematogenic cells 1 month after surgery. A poor osteogenic activity and some osteoblasts were visible two month post surgery. In the treated group, within 1 month, a mass of osteoblasts as well as neoformed vessels occupied the gap 1 month after surgery. One month later we observed increased young bone trabeculas in addition to a more compact bone neoformation.Conclusion: We concluded that PRP with some characteristics such as ostegoenesis and osteoinduction provide a more rapid regeneration of bone defects.

Abstract: 

Background: This study proposed to examine the possibility of homing of bone marrow derived-mesenchymal stem cells after allograft transplantation in irradiated and healthy bone marrows.Materials and Methods: 65 rats were divided into 13 groups (1 healthy and 12 irradiated). Mesenchymal stem cells were isolated from femoral bone marrow of male rats. The cells were cultured under morphological observations, differentiation tests and immunocytochemistry staining. Then, the mesenchymal cells were injected to the tail vein of the healthy and irradiated (7Gy gamma ray) female rats. During defined time gaps after injection, the transplanted rats were killed and their isolated bone marrow was cultured. DNA of the cultured cells was extracted and was amplified by PCR with specific primers for chromosome Y (SRY). Finally, the PCR products were analyzed on gel electrophoresis. Results: Morphological observations and, differentiation and immunocytochemistry tests conformd that the isolated cells were mesenchymal stem cells. PCR results for the healthy and irradiated rats in all time gaps after transplantation were negative indicating that male mesenchymal stem cells were failed to home in bone marrow of transplanted female rats. Conclusion: We concluded that due to low number of the injected mesenchymal stem cells or trapping of the cells in other organs Implantation the cells in bone marrow was not considerably detectable.

Abstract: 

Background: Regarding the current trend of turning attention towards the clinical application of herbal drugs in the healing of burn wounds, the present study was designed to evaluate the effect of Arnebia leaf on the healing process of second degree burn wounds in rat.Materials & Methods: After the production of a second degree burn covering 10% of total body surface area on 40 adult albino rats (weighing 220±30 g.) animals were divided into 4 groups: treatment group using herbs containing Arnebia leaf; treatment group using herbs containing no Arnebia leaf; silver sulphadiazine (SSD) treatment group and normal saline treated group. All groups were treated and controlled once a day up to the 30th day that they decapitated for histological study of healing process (e.g. epidermal area density, blood vessel density and number in the area). Data analyses were done using one-way ANOVA and post-hoc Tukey tests. Results: Morphometric studies showed that herbal treatment group containing Arnebia leaf compared to herbal treatment group with no Arnebia leaf and SSD group for epidermal area density had a significant lower (P<0.0001 and P<0.002, respectively) and compared to no Arnebia leaf group a significane higher number and density of blood vessels (P<0.001 and P=0.004, respectively).Conclusion: This study shows that Arnebia leaf did not have any effect on healing of second degree burn wounds in rat's. More study were recommended for evaluating the effects of other parts of the plant on burn healing.

Abstract: 

Background: Purity of cultured Schwann cell is very important in patients’ outcome. The purpose of this study was evaluation of cell purity in a culture without nerve growth factors and fetal bovine serum.Materials and Methods: In this experimental study, for culture of human Schwann cells, nerve growth factors and fetal bovine serum were replaced by human autologous serum. Obtaining consent from the close relative, nerve grafts were harvested and transported to processing unit where they were cultured in DMEM upon aseptic condition. Then the cultured cells were evaluated with S100 antibody staining for both morphology and purity. Results: Cell purity range was from 97% to 99% (mean=98.11±0.782%). Cell count was 14055.56±2480.479 per microliter. There was not significant correlation between cell purity with either the culture period or the age of donors (P>0.05). The Spearman correlation coefficient for the cell purity with the culture period and the age of donors was -0.21 and -0.09, respectively.Conclusion: We found that the replacement of nerve growth factors and fetal bovine serum with human autologous serum improves the cultured Schwann cells for clinical use with more safety and minimum reagents.

Abstract: 

Background: Trauma is the leading cause of mortality and its unfavorable outcomes among the population under 50 years. This study was conducted to assess the outcome of traumatic brain injuries and its related factors among the patients hospitalized in intensive care unit (ICU).Materials and Methods: This descriptive study was carried out on 95 patients hospitalized with traumatic brain injuries in ICU wards of Kashan hospitals. A check-list was used for data gathering, consisted of demographic and clinical data (e.g. characteristics of accident, trauma outcome and the level of consciousness). The level of consciousness was evaluated using Glasgow Coma Score (GCS) on admission and 6 hours post-admission in ICU. Results: The average age of patients was 27.6±13.9 years. The average length of hospitalization in ICU (12.3±13.9 days) showed a significant relationship with GCS. Sixtypercent of patients showed desirable outcomes with a mortality of 23.2%. The severity of head trauma on admission and 6 hours post-admission had a significant relation to trauma outcome (P=0.0001). Logistic regression showed that GCS evaluation 6 hours postadmission could identify the outcome of trauma (P=0.007). Conclusion: The evaluation of GCS 6 hours post-admission to ICU was among the most important variables in identifying outcomes after head trauma. To evaluate and predict the outcomes of head trauma, the data related to the GCS of the ICU patients should be recorded carefully.

Abstract: 

Background: Quality control of drinking water is important for healthy consumers. Having a high total dissolved solid (TDS) in water causes a salty taste. Reverse osmosis is one among the membrane processing methods currently used for lowering TDS in water. As during the recent years several desalination plants were set into operation by private sectors in Kashan, the study was carried out for the evaluation of the quality control of inlet and outlet water of the mentioned plants.Materials and Methods: This cross-sectional descriptive study was carried out using samples taken from the inlet and outlet water of plants during a six-month period in summer and winter seasons of the year 2008. A total number of 128 samples taken from 16 desalination plants were analyzed for water chemical quality.Result: The average concentration of chemical parameters in treated water were: TDS= 245, total hardness=118, nitrate=2.5 mg/l, fluoride=0.2 mg/l, sulfate=24 mg/l, chloride=63 mg/l and pH range = 6.8-7.Conclusion: Based on findings on desalinated water, except for fluoride, other parameters were in accordance to the optimum concentration for drinking water. For the prevention of dental caries, addition of fluoride should be considered in future program. While pH of outlet water was relatively decreased with respect to the raw water, but none of them were compatible with drinking water standards.

Abstract: 

Background: While the arteriopuncture is a painful and relatively risky procedure and may be complicated by some problems, venopuncture required for assessing the venous blood gases (VBG) is an easier procedure with fewer complications. Considering the mentioned notion, substituting the VBG values for of ABG values can prevent such complications. This study was carried out with the aim of determining whether the VBG values can be replaced for ABG values in the management of mechanically ventilated patients admitted in ICU wards.Materials and Methods: This study was carried out on 102 patients admitted in ICU ward of Shahid Beheshti. After performing the Allen test, ABG was taken from radial artery of one hand and immediately VBG from the brachiocephalic vein of the other. The ABG and VBG indexes were calculated using a GEM3000 system. After the data collection and calculation of mean and standard deviation of all indexes, two methods were compared using paired t-test, subsequent correlation ratio and the linear regression model. Results: Among the 102 hospitalized patients 70 were males and the remaining 32 females. The results of comparing the ABG and VBG indexes in terms of Pearson-correlation coefficient and the relevant significant level were: pH (0.801, P<0.001); PCO2 (0.835, P<0.001); HCO3 (0.768, P=0.369); PO2 (0.287, P<0.001); SaO2 (0.317, P<0.001); BE (0.810, P<0.011).Conclusion: While there was a significant correlation between the ABG and VBG indexes, but with the consideration of the power of correlation, the substitution of VBG for ABG is not recommended for mechanically ventilated patients.

Abstract: 

Background: The time spend for prehospital action and the type of seeking treatment behaviors of MI patients are important. The present study was conducted to answer seeking treatment behaviors at patients with myocardial infarction (MI) in Kashan Shahid Beheshti Hospital.Materials and Methods: A cross-sectional case study was conducted on 280 patients with MI during 2006-7. Data were gathered using a questionnaire including 7 questions related to the demographic characteristics of subjects, the time of manifestation of symptoms, the hospitalization history, the initial perception of symptoms, the patient's seeking treatment behaviors and methods using to come to the hospital. The patient's behaviors were categorized in two groups of appropriate and inappropriate.Results: From a total of 280 samples 73.8% were men (mean age: 61.37±13.34 years) and 26.4% were female (mean age: 67.21±9.97 years). The most revealed behaviors were rest (42.5%), self medication (34.3%) and calling the Emergency Medical Service (22.85%), using sublingual TNG (20.5%), respectively. Only 107 patients had appropriate seeking treatment behaviors. On the other hand, 60 patients displayed inappropriate treatment behaviors. A combination of appropriate and inappropriate seeking treatment behaviors have also reported by 113 patients. Conclusion: A considerable number of the patients with MI demonstrated inappropriate seeking treatment behavior. Public education would be helpful to reduce the prevalence of the inappropriate behaviors.

Abstract: 

Background: Professional support has frequently been mentioned as one of the basic prerequisite for nurses' job satisfaction and, creativity and quality in nursing care. However, perception of a professional support to this job has not been studied adequately. The present study was made to clarify the concept and the process to professional support in nursing. Materials and Methods: this qualitative study was conducted using the grounded theory method during 2005-9. The research question was that of what experiences and perceptions the nurses have of the concept of professional support, 29 nurses were interviewed semistructurally in several educational hospitals in Tehran and Kashan. Data were analyzed using Straus and Corbin's method.Results: Seven categories were emerged from data including the concept of support, feeling of being sheltrles, decreased self-confidence, and decreased quality of care, avoidance, decreased professional unity and legitimate support. Participants defined professional support as any material or spiritual backing to strengthen their capabilities in nursing care. They believed that professional support increases capabilities of nurses to make clinical decisions. However, feeling to be unsupported made them to be indifferent, low in self-expression, distrusted them to their managers, powerlessness and shortcoming in conflicts. Lack of legitimate support decreases the nurses' decision-making power and made them to avoid of autonomous intervention. Nurse managers had also a sense of lacking legitimate support. Conclusions: Althogh, the professional support enable nurses to offer a good care for their clients, however, sometimes they feel left unsupported. Developing the legitimate support may decrease the nurses feeling of being unsupported.

Abstract: 

Background: Citation analysis is one of the quantitative methods in assessment of scientific journals. The present study evaluate citation and collaboration rate for Feyz, Journal of Kashan University of Medical Sciences over 2002 to 2008. Materials and Methods: This Study was Carried Out by citation analysis methods. Data were gathered via checklists and analyzed, descriptively. Individual or group authorship, gender of the authors, number of citations, distribution of the cited sources, language of the citations and their half-life were under focus of this study. Bradford' law was used to specify the core journals.Results: 394 articles were produced by 1333 authors. 31.2% of articles had 3 authors. Totally, there were 6771 citations for 394 articles. Average citation number for each article was almost 17. The highest percent of articles was belonging to English journals. Average half-life of Persian and English citations was 6.8 and 9.6, respectively. Conclusions: Our findings indicated that English journals have an important place in acquisition of information. Through organizing the most usable core journals, medical librarians can help to economize expenses in medical uni versities.

Background: Exclusive breast feeding, an important need for infants, can be affected by numerous factors. This study was designed to survey the pattern and period of exclusive breast feeding.Materials and Methods: The study was performed on mothers of 391 newborns at six months of age taking healthcare in health centers of Kashan. The necessary information was recorded and exclusive breast feeding patterns and related factors were determined. Results: Our findings uncovered that the rate of exclusive breast feeding was 33.1%. The exclusive breast feeding showed a significant relationship with the mothers' education level (P=0.001), baby weight loss (P=0.046), exclusive breast feeding education (P=0.004), type of delivery (P=0.045), father’s job (P=0.012), birth weight (P=0.007) and growing (P=0.002) status. However, sexuality of the neonate, sexual preference by family, bedding room, technique of breast feeding education, mother's job, immaturity and weight not considerably influenced the exclusive breast feeding. Conclusion: Concerning low rate of exclusive breast feeding it seems increased understanding and knowledge of pregnant wemon, mothers, as well as their family, can improve exclusive breast feeding.

Abstract: 

Background: Non-alcoholic fatty liver disease is among the prevalent causes of chronic hepatitis and cirrhosis. Here, we discuss the best diagnostic and therapeutic approaches for the disease.Materials and Methods: Epidemiology, pathogenesis, etiologies, natural course, differential diagnosis, treatment options, complications and follow up of the disease are reviewed in this paper.Results: Non-alcoholic fatty liver disease seems to be the hepatic manifestation of the metabolic syndrome. Liver function tests and sonography can be used as the appropriate screening tests for diagnosis. Weight loss and control of hyperlipidemia and diabetes mellitus might be the best therapeutic approaches.Conclusion: Early diagnosis and appropriate treatment may decrease mortality and morbidity rate associated to the cardiovascular complications of the metabolic syndrome.
