ازجمله مشکلات جدی در کشور ما که نیازمند توجه فوری می باشد، روند پرشتاب کهنسال شدن جمعیت است. علیرغم اینکه متعاقباً رشد جمعیت و میزان باروری کل کاهش یافته است، شاخص امید به زندگی در حال افزایش می باشد. این بحران به زودی مسئولین را با چالش های متعددی ازجمله هزینه های گزاف مراقبت های بهداشتی و درمانی حاصل از بیماری های مزمن سالمندان، مواجه خواهد نمود. راه حل کلیدی موجود برای مقابله با این چالش ها، ارائه خدمات مراقبت در منزل به سالمندان می باشد.

Abstract: 

Introduction: Patients are required to take medications to control and prevent the progression of hypertension, one of the most common chronic diseases. The widely use of cellphone text messages make it easy and convenient to enhance medication adherence The current study aimed to evaluate the effect of text message reminders on blood lipids control in patients with chronic hyperlipidemia. Methods: The sample population in this randomized clinical trial, consisted of sixty patients divided into control and intervention groups at Imam Hossein Comprehensive Health Service Center in Borazjan in 2018. The patients in the intervention group received a daily reminder message for six weeks. Data were collected by Morisky questionnaire was completed by both groups at baseline and six weeks later. The data were analyzed by SPSS software version 25 and R version 2. 5. 3. Results: the results indicated a significant difference between the two groups. (p<0. 001). Conclusion: Sending reminder messages to patients with chronic hyperlipidemia over a six-week period is effective and leads to improved medication adherence.

Abstract: 

Introduction: Accessibility of reliable and up-to-date information to health care providers can lead to effective community health; therefore, experts are needed to retrieve information. These experts, called clinical librarians in developed countries, need special training. Therefore, this study aimed to design a clinical librarian program for training librarians to provide information services in clinical settings. Methods: In this descriptive– applied research, a qualitative approach was adopted. In the first phase, 188 sources related to clinical librarianship were found and validated, and finally 44 sources were selected for analysis. The sources, were reviewed to extract skills which were then reviewed and validated in two Delphi rounds by professors and Ph. D. students of Medical Librarianship in Iran. The selected skills were used to propose an educational program. Results: In the first phase, four skills among 62, i. e., information literacy, evidence-based medicine, and critical analysis and assessment obtained the highest frequency. Based on the results of two Delphi rounds, the educational program was designed as short-term training courses consisting of skills with the highest frequency in sources, and the highest mean points from the perspectives of participants in Delphi rounds. Conclusion: Based on the results it can be concluded that the viewpoints of Delphi contributors concerning clinical librarian skills are largely consistent with those presented in the sources. Therefore, the program designed in this study can be used as a guide for clinical librarianship curriculum designers.

Abstract: 

Introduction: Self-care in breast cancer patients undergoing chemotherapy is an important factor in controlling and reducing the side effects of chemotherapy which leads to a greater adherence to chemotherapy and improved quality of life. Given the role of smartphones in individuals' lives and their apparent capacities for self-care, the aim of this study was to present a mobile-based self-care application for patients with breast canceundergoing chemotherapy. Methods: This applied developmental research was conducted in two stages. At first, needs assessment was performed to identify the needs of oncology and general surgery specialists and patients with breast cancer undergoing chemotherapy. Then, the prototype of the application was designed under the Android platform. Finally, to evaluate the usability and satisfaction of users, the standard Quiz questionnaire version 5. 5 was completed by 30 patients for one week. The data were analyzed by SPSS software version 21 using descriptive statistics. Results: According to the findings 32 elements (among 36) of the needs assessment questionnaire were considered as necessary by the participants. General features of the program included: patient profiles, side effects management, life style and system capabilities. The usability assessment of the application showed that the users rated the application as good with a mean score of 7. 9 (out of nine points). Conclusion: Mobile-based care applications can be used to help patients with breast cancer undergoing chemotherapy in managing side effects and obtaining self-care skills.

Abstract: 

Introduction: Articles are retracted due to scientific misconduct and serious errors. The current study aimed to analyze the retracted articles in Endocrinology Metabolism journals during 1990-2017. Methods. This applied study was conducted by using scientometric indicators with a descriptive approach. The sample of the study comprised of 106 retracted articles published in 45 Web of Science-indexed journals of Endocrinology Metabolism. Results. The findings indicated that, on average, 2. 35 Retraction Notices were published per journal. The average retraction index was 0. 65. Duplicate publication and manipulation were the most common reasons for retraction and accounted for 29 and 23 retractions. The median time from publication of the original article to retraction was 49. 4 months. The most retracted articles (n= 42) belonged to the United States. Conclusion. Scientific misconduct and retractions in endocrinology and metabolism, as a clinical discipline, is alarming. Therefore, publishers, editors, and reviewers are required to pay more attention to publication ethics and enhance researchers’ awareness of research misconduct.

Abstract: 

Introduction: Insurance deductions by insurers often lead to financial turmoil in hospitals. With due consideration to lack of hospital resources, hospital deductions double the problems stemming from limited resources, therefore, the investigation of hospital indicators which affect deductions can lead to cost reduction. This study aimed to explore factors affecting insurance deductions in hospitals in Yazd province before and after the implementation of Health Reform Plan. Methods: This analytical-applied study was conducted in 2018. The study population consisted of all hospitals (N=11) affiliated to Yazd University of Medical Sciences. A form was designed by the researcher to collect data which were then analyzed by using descriptive statistics and Pearson correlation test. Results: The comparison of hospital deductions before and after the implementation of Health Reform Plan revealed an increase in deductions in all hospitals. There was also a significant and direct relationship between the number of hospital admissions (P=0. 000), bed occupancy coefficient (P=0. 034), and the number of active beds (P=0. 001) with the amount of hospital deductions. Conclusion: A significant amount of the hospital revenues are wasted due to deductions which can be reduced through analyzing the causes and finding solutions to remove them, training and educating hospital staff and authorities, and implementing effective mitigation strategies.

Abstract: 

Introduction: Nowadays, changes in the field of competition and environmental uncertainty have led executives to pay more attention to factors affecting performance in order to take the advantage of leading opportunities in global markets. The purpose of this study was to investigate the effect of entrepreneurial orientation, learning orientation, and entrepreneurial self-efficacy on business performance. Methods: This applied-descriptive study was conducted on managers, administrators, supervisors and experts of Samen Pharmaceutical Company in Mashhad. The population consisted of 187 people, 132 of whom were selected through stratified sampling method. Data were collected by using a standard questionnaire, the construct validity of which was assessed through confirmatory factor analysis. The reliability of the questionnaire was calculated as 0. 8 using Cronbach's alpha. The data were then analyzed by SPSS and Amos software. Results: According to the findings, entrepreneurial orientation and entrepreneurial self-efficacy directly affected the business performance (with coefficients of 0. 18 & 0. 33, respectively). Learning orientation (with a coefficient of 0. 22) influenced organizational learning which affected business performance with a coefficient of 0. 40. Moreover, entrepreneurial orientation, learning orientation, and entrepreneurial self-efficacy influenced business performance through organizational learning (with coefficients of 0. 20, 0. 08, and 0. 16, in that order). Conclusion: An increase in organizations’ desire for entrepreneurial and self-efficacy enhances learning and leads to a better performance.

Abstract: 

Introduction: Internet of things is a hot topic in the modern age of information and communication technology has become pervasive in various industries, particularly in healthcare sector. The security and privacy issue has attracted a lot of attention and has become a controversial issue in this area. The purpose of this study was to identify security mechanisms of Internet of things in health and non-healthcare industries. Methods: The present study was a systematized review conducted by searching the Web of Science, Scopus, PubMed, IEEE Xplore, and ProQuest databases from 2005 to 2019. After the elimination of duplicate cases, studies related to the purpose of the study were selected on the basis of inclusion and exclusion criteria. Results: We found 71 out of 2340 studies, related to security mechanisms of Internet of things in the healthcare (four studies) and non-healthcare (67 studies) industries, which were then reviewed. Security mechanisms have been organized into 11 major categories, including cryptography, key distribution and management mechanisms, digital identity management, and system life-cycle maintenance management, and secure routing in both the healthcare and non-healthcare industries. The findings showed that five types of security mechanisms, including energy saving, secure design and physical protection of system hardware, intrusion detection and prevention system, trust management, and fault detection and tolerance had not been addressed in the health industry. Conclusion: Given the emergence of this technology in the healthcare industry and its security sensitivity to other industries, the findings of this study provide a broad insight for researchers, managers and information security professionals to encounter threats and attacks and develop a secure Internet of things architecture.

Abstract: 

Introduction: Human resources, as the most important resources in health systems, account for nearly three-quarters of current expenditures. Therefore, it is necessary to determine the optimal level of human resources in healthcare facilities. This study aimed to use workload indicators of staffing need to estimate the required human resources in medical physics department at Reza Radiotherapy-Oncology Center. Methods: This descriptive cross-sectional study was conducted in 2019. Data were collected through different means. Experts’ opinions on the components of workload and activity standard were collected during several meetings. Moreover, interviews with the managers of radiotherapy center manager and human resources, and exploration of rules and regulations and personnel systems were conducted to extract factors related to available working time. Finally, the annual workload was determined by searching the hospital management system and the activity logs. Data were analyzed by using Excel and SPSS version 19 software. Results: The results demonstrated seven factors related to staff annual available working time, that is, 1069 hours for medical physics. In this study, treatment planning, treatment recheck, mosaic and manual data entry were determined as the main activities of the medical physics department. Furthermore, the workload indicators of staffing need calculation showed that the medical physics department needs four more staff. Conclusion: The results of the study showed a shortage of workforce in medical physics department at Reza Medical Center. The workload indicators of staffing need clearly identify the components of the workload and are recommended to be used for estimating human resources and professionals in specialized occupations associated with cancer treatment.
