Abstract: 

Background & Aim: Patient safety is a major concern for health care organizations. The first step to improve culture of patient safety is its assessment. Accordingly this study was performed with the aim of assessing culture of patient safetyfrom nurses’ perspective in the hospitals affiliated to Hormozgan University of Medical Sciences.Material & Methods: It was a descriptive cross-sectional study. The sample consisted of 190 nurses working in educational hospitals of Hormozgan province who were recruited by stratified random sampling in 2012. Data was collected by Hospital Survey on Patient Safety Culture questionnaire (HOSPIC) and analyzed by independent T. test, one-way ANOVA and Post hoc test of LSD using SPSS- PC (v.16).Results: Among the different dimensions of patient safety culture, the lowest score was related to the number of employees and its related issues (40.25%) and the highest score was related to team work in the unit (85.60%). In the past 12 months, 63 nurses (35.7%) did not report any errors and 53 nurses (30.1%) reported only 1 or 2 error. Significant differences was seen between the score of different dimensions of patient safety culture and nurses’ work experience, the type of employment, hospitals, and the amount of working hours (p<0.05).Conclusion: Regarding the findings, it is suggested that managers and policy makers identify the factors involved in patient safety culture and improve the working environment, and promote supportive organizational climate in hospitals. Farther research is needed to detect the causes of nurses' failure to report medical errors.

Abstract: 

Background and Aim: In order to improve the patient's safety, countries are required to monitor the safety of systems, and in this path, patient safety culture should be explored and developed among staff. The purpose of this study was to evaluate the effect of teaching on patient safety culture on the promotion of safety culture and self-efficacy of nurses in Vali-e-Asr Hospital of Birjand. Materials and Methods: The present study was carried out in a quasi-experimental design with pretest-posttest with the control group. The statistical population of this study was all nurses working in Vali-e-Asr Hospital in Birjand in 2018 (100 people). From this statistical society, based on the Morgan table and by stratified sampling, A total of 80 nurses were selected and Randomly and evenly divided into two experimental and control groups. The group training sessions of the patient safety culture were conducted for 4 sessions of 2 hours for the experimental group and the control group did not receive any intervention. In the pretest and posttest, both groups responded to the research tools (Patient Safety Culture and General Sherer Self-efficacy Questionnaire). Data were analyzed using SPSS software (version 22) by using covariance analysis. Results: The results showed that in the variable of safety culture due to the variable effect, The education of the patient safety culture was significant (P <0. 01); in the self-efficacy variable with respect to the effect of the variable, The education of the patient safety culture was significant (P = 0. 001). Conclusion: Training on patient safety culture to nurses, affects the promotion of their safety culture and self-efficacy; Therefore, it is suggested that planners, administrators, use this methodology to promote the safety culture.

Abstract: 

Introduction: Over the past two decades, this thought that the health system is not safe enough and need to improve, has been studied globally. So, assessing the views' of medical staff would be beneficial for improving health safety programs.Aim: This study was done to determine patient safety culture from the perspective of the medical staff in Khorramabad teaching hospitals.Methods and Materials: A cross-sectional study was done in (2015) that (280) medical staff who selected by convenience sampling by using stratified sampling then quota sampling participated. Data were gathered by questionnaires; Hospital Survey on Patient Safety Culture (HSOPSC) that printed in Quality & Research in Health in USA and demographic questionnaire. All data analyzed by SPSS16 and findings with (p<0.05) presented.Results: findings showed that the safety culture at the university hospitals of Khoramabad in staff view point is not desirable and among twelve patient safety culture dimensions, the most important dimensions need to address includes; organizational learning, continuous improvement, non-punitive response to errors, frequency of adverse event reporting, communication openness, management support for patient safety, feedback and communication about error.Conclusion: It is suggested to nurse managers that planned change in hospitals for improving safety culture for decreasing errors and increasing reporting them.

Abstract: 

Background and Objectives: Patient safety is crucial for promotion of healthcare quality and is monitored as an important parameter by all healthcare provider organizations worldwide. Nurses play a major role in maintaining and promoting patient safety due to the nature of their work. The purpose of this study was to compare the effect of patient safety education program on patient safety culture among nurses working in Ilam education-treatment centers.Methods: In this experimental study, 40 nurses were selected from Ilam educational-treatment centers and randomly allocated to two groups of virtual patient safety education and lecture-based education. Hospital Survey on Patient Safety Culture questionnaire was completed by nurses of the two groups before and after patient safety education program. Statistical analysis was performed using Kolmogorov-Smirnov, independent t-, and paired t-tests. The significance level was considered less that 0.05.Results: Patient safety culture was significantly improved in both virtual and lecture groups after educational intervention (p<0.01). In the virtual education group, the mean score of patient safety culture was higher than lecture group, and this difference was significant (p<0.01).Conclusion: The findings of this study indicated that educational programs could be used by administrators of health service providers to improve patient safety culture among nurses. Also, providing patient safety educations as virtual or lecture both will be helpful.

Abstract: 

Background & objectives: Medical errors are one of the five most common causes of death. Patient safety is an important issue in healthcare systems, and the existence of a strong patient safety culture in hospitals influences safety-related behaviors and reduces medical errors. The present study aimed to evaluate patient safety culture in Ardabil hospitals and investigate the attitude of employees towards their own performance compared to their colleagues in the field of patient safety culture. Methods: The data of this cross-sectional study were collected from 420 staff of four hospitals of Ardabil University of Medical Sciences by HSOPSC questionnaire and a researcher-made questionnaire for staff opinions on their own practice in the field of patient safety culture. One-way Anova and T-tests were used to determine the statistical relationship between staff demographics and dimensions of patient safety culture. SPSS24 software was used for analysis. Results: The overall mean percentage of positive response to the dimensions of patient safety culture was 57. 71% and was in the middle range. The highest percentage of positive responses was related to the dimensions of "Feedback & Communication about Error" (86. 82%) and "Teamwork within Units" (86. 64%). The lowest percentage of positive response was related to the dimensions of "Frequency of Events Reported" (24. 83%) and "Non-punitive Response to Errors" (27. 93%). Investigating the attitude of employees towards their own performance compared to their colleagues shows that in most dimensions, employees perceive their performance better than their colleagues. Conclusions: The status of patient safety culture in the studied hospitals needs to be improved. The error reporting by staff is also very low. Therefore, it is necessary to increase the number of staff, especially nurses in proportion to the number of patients and the workload, to reduce staff working hours, adopt policies of safety and non-punitive culture, support staff and implement effective training programs in hospitals.

Abstract: 

Introduction: Patient participation is among the essential factors in providing high-quality and safe services in health care centers and a primary concernfor health system managers and policymakers. This study aimed to identify the components of patient participation in improving the patient safety culture. Methods: The scientific model of this research was based on qualitative interviews with hospital experts (faculty members of universities), and the sampling method was both purposive and snowball. A total of 16 qualitative interviews were conducted with the hospital experts, and the grounded theory technique was adopted to identify the components of patient participation in improving the patient safety culture. The necessity of each component extracted fromthe patient participation model was examined. The research data were analyzed using the coding method in three stages: open, axial, and selective. Results: The results revealed100 unreplicated nodes (codes) extracted based on the conceptual similarities of the composition and 58 common codes. Based on the researchers’,intuition and understanding of the subject, the main components and sub-components of organizational factors (giving a sense of control and responsibility, heavy tasks and the number of manpower, management and leadership, and response to errors), environmental and perceptual factors (safety communication, patient empowerment, teamwork, and evaluation), and treatment decision-making (patient’, s characteristics, patient’, s collaboration, and raising awareness) were categorized as tree nodes. Conclusion: According to the results of this study, Patient participation is an important issue in healthcare systems. Patients can play an important role in improving immunity by actively participating in health care. Patient participation is effective in improving the quality and safety of care.

Abstract: 

Introduction: Patient safety is one of the main components of health service quality, and patient safety culture is considered as one of the necessary factors in promotion of the patients’ safety. On the other hand, the application of critical thinking skills, by developing evidence-based practices, leads to positive outcomes in patients. This study aims to determine the relationship between critical thinking and patient safety culture in the nurses. Methods: This correlational study was conducted on 196 nurses working in Shariati Hospital of Isfahan city who had been selected by convenience sampling method. Data was collected by hospital survey on patient safety culture (HSOPSC) questionnaire and the California critical thinking skills test; form B (CCTST). The data was analyzed by descriptive statistics and Pearson correlation test in SPSS 22 software. Results: Most of the participants (52. 6%, 103 persons) were woman and were in the age group of 31-40 years (35. 2%, 69 persons). The mean score of patient safety culture in study samples was 3. 03± 0. 45 which was at a weak level. Also, the mean score of nurses’ critical thinking was 29. 74 ± 3. 36 which had a desirable level. A significant direct relationship was found between patient safety culture and nurses’ critical thinking; so that improvement of the nurses’ critical thinking led to promotion of patient safety culture (r=0. 338, P<0. 001). Conclusions: According to the results of this study, improvement of the nurses’ critical thinking can help to promote patient safety culture. Therefore, it is necessary for nursing managers and nursing education policy makers to plan for promoting critical thinking of nurses and nursing students as one of the ways to develop the patient safety culture, to provide the necessary context for improving patient safety and ultimately improving the quality of nursing care.

Abstract: 

Introduction: Health care organizations, especially hospitals, must focus on patient safety as their main strategy. Cultural change is the most important challenge that they are faced with in creating a safe system. The present article aims to improve the patient safety culture with an assessment of the safety in Fatemeh Zahra Hospital, Najafabad, Iran.Methods: This descriptive and cross-sectional research was conducted in 2010. Patient’s safety culture was measured using the Persian adaptation of the Hospital Survey on Patient Safety Culture. This survey was validated by the opinion of 15 experts. The reliability of the questionnaire was determined by Cronbach’s alpha and was 0.846. The questionnaire was sent to all medical employees of Fatemeh Zahra Hospital. Data were analyzed with t-test and one-way ANOVA by SPSS Software. A P-value of less than 0.05 was considered as statistically significant, and 95% confidence interval was calculated.Results: Overall, 196 of the 350 individuals responded to the surveys. The overall response rate was 56%. Supervisor expectations and actions gained 76% of all scores, teamwork within hospital units 75%, organizational learning 73%, feedback and communication about error 70%, transitions 69%, communication openness 68%, management support 65%, teamwork across units 62%, non-punitive response to error 54%, and staffing gained 48%.64% of all employees had no reports of error. The mean patient safety score was 3.36 (out of 5). According to t-test results, there was no significance difference between patient safety scores in direct and indirect contacts with patient (P=0.373). Moreover, according to the results of ANOVA, there was a significant difference between the scores of the maternity department and midwifes, and other wards and employees (P<0.000). There was no significant difference between the patient safety culture scores of employees of different wards, work experience in the ward (their current place of work), work experience in the hospital, and type of contact with patients (P<0.05).Conclusion: Although the total score of patient safety culture was acceptable, non-punitive response to error, staffing, teamwork across units, and management support are in need of immediate intervention. The hospital can improve the patient safety culture by planning and taking action in these areas.
