Abstract: 

Clinical manifestations of COVID 19 is still unknown. We performed this study to determine the occurrence of pulmonary barotrauma as a complication of this disease. In this retrospective study, a total of 955 COVID 19 patients with respiratory insufficiency requiring oxygen support or invasive ventilation admitted to ICU of Sina Hospital from 20 March 2020 to 9 June 2021, were included and their chest imaging reviewed. Here, we report results of chest imaging of first 92 patients of this group. Barotrauma (pneumothorax, pneumomediastinum, pneumopericardium) occurred in 11 (11. 9%) of 92 patients with coronavirus disease 2019 (COVID-19) infection requiring ICU admission for respiratory support and monitoring. It seems barotrauma is a common complication of COVID 19 disease. The role of increased respiratory efforts, patient or ventilation induced lung injury, viral and host response should be assessed. It needs to consider the occurrence of barotrauma in Patients with COVID-19, before expansion of dead space for treatment and limiting the ventilation effects.

Abstract: 

Facial baroparesis is an extremely rare clinical entity which occurs due to otitic barotraumas. It is rarely reported in medical literature which can happen among persons those ascend to high altitude in flight or scuba diving. The overpressure in the middle ear cavity due to eustachian dysfunction may cause exertion of the excessive pressure over the facial nerve through a dehiscence of the horizontal segment of the fallopian canal leading to facial nerve paralysis. The clinical history and imaging help to diagnose this rare cause of facial nerve paralysis. Here, we report a case of a 38‑ year‑ old female who experiences unilateral facial nerve paralysis on ascent to high altitude on a flight, with relieve from symptoms shortly after descent.

Abstract: 

Background: Pneumothorax is the most common air leak syndrome and far more frequent in the neonatal period than any other time of life. Objective: To evaluate the etiolngies of pneumothoraces in the neonatal intensive care unit (NICU). Methods: In a prospective study, barotrauma, secondary pulmonary infections, unskilled endotracheal intubation, asynchrony between the patients breathing and ventilator and ventilation with self-inflating bag were studied on 50 NICU patients of Mofid children hospital. Findings: Mean airway pressure was above the critical level in 18% of the cases. 72% of cases had pneumothoraces in the right side and 16% bilaterally. Asychrony was seen in 38.6% of the cases, secondary pulmonary infections in 46% and malposition of ET tube in 36%. Ventilation with manual bag was done in all cases. The bags lacked manometer and pop-off valve. Conclusion: Barotrauma had the main probable role in the etiology of pneumothoraces and is probably caused by self-inflating bags without pop-off valve and manometer.

Abstract: 

Background: Patients with Coronavirus disease 2019 (COVID-19) pneumonia are at risk of hypoxemic respiratory failure. Hence, many patients may require noninvasive positive pressure ventilation (NIPPV) during their hospital course. Using mechanical ventilation such as bilevel positive airway pressure or a ventilator to provide NIPPV may result in adverse events, including barotrauma. Case Report: We reported two cases (40-and 43-years-old men) of severe COVID-19 pneumonia and hypoxemic respiratory failure who underwent NIPPV for respiratory support. These cases were complicated with barotrauma in their course of hospital admission that manifested with pneumoscrotum. Conclusion: In the cases of pneumoscrotum, it is crucial to understand its underlying etiology and origin since this clinical finding may be the outcome of life-threatening illnesses requiring urgent treatment.

Abstract: 

Background: In the treatment process of hospitalized patients with COVID-19, one of the threatening complications is pulmonary complications such as barotrauma. Objective: This study aims to investigate the prevalence of barotrauma in COVID-19 patients admitted to intensive care units (ICU) of two hospitals in Rasht, Iran. Methods This retrospective study was conducted on COVID-19 patients admitted to the ICUs of Razi and Poursina hospitals in Rasht in 2020. Their files were studied, and their demographic/clinical data and pulmonary complications were extracted using a checklist by an anesthesia resident.   Results Out of 140 patients, 81(57. 9%) received non-invasive mechanical ventilation and 59(42. 1%) had invasive ventilation. Barotrauma was observed in 9 cases (6. 4%), 8(88. 9%) in the form of pneumothorax and one (11. 1%) in the form of both pneumothorax and subcutaneous emphysema. The results showed that the incidence of barotrauma was significantly higher among patients who received invasive mechanical ventilation (P=0. 003). No significant difference was observed among patients received non-invasive ventilation (P>0. 05).   Conclusion: The prevalence of barotrauma among COVID-19 patients in the study hospitals is low, which indicates the proper performance of the attending physicians and compliance with the standard guidelines
