Abstract: 

Male sexual disorder includes hypoactive sexual desire disorder, male erectile disorder, premature ejaculation and male orgasmic disorder. Premature ejaculation is the most common problem. Premature ejaculation could have an effect on the various aspects of the quality of life including emotional happiness, interpersonal relationship, and social behavior. One of the important aspects of premature ejaculation is definition of this problem and explanation to the individuals. Currently, various treatments are available for premature ejaculation consistent with psychological/behavioral management and pharmacotherapy. Serotonin Specific Reuptake Inhibitors (SSRIs) especially dapoxitin, are first-line pharmacotherapy; however tramadol might be used for unresponsive premature ejaculation. New therapeutic approaches are being researched including modafinil, botulinum toxin, acupuncture and circumcision.

Abstract: 

Introduction: Premature ejaculation (PE) is one of the most prevalence men problems. There are a lot of explanations such as psychosomatic and even genetics problem for it. Because the patients suffering varicocele are more complain from this problem, we decided to evaluate it in these patients.Materials and Method: In a case control study, we evaluated Premature-ejaculation in a 55 patients with varicocele (20 - 35 years old) that they had come to the andrology clinic for treatment of infertility and other genital problems and were compared their data with a control group of 60 men who were in the same range of age without varicocele.  The data statistically were compared by X2 and T-test. Results: The premature-ejaculation (Intravaginal Ejaculatory Latency Time ((IELT)) < 2 min) was found in 36.3% (n=20) and 8.3% (n=5) of the subjects in study and control groups respectively, (P<0.001). This Study showed that PE has a direct relation with the severity, time length of affection (age group), and varicocle distribution.Discussion: Since premature ejaculation has been found with a high frequency in men with varicocele, we stress the importance of a careful examination of the patients with PE before any pharmacologic or psychosexual therapy.

Abstract: 

Background: Premature ejaculation (PE) is the inability to delay ejaculation, occurring sooner than they or their partner would like during sexual activities. PE is a challenging problem that can affect sexual enjoyment and may harm relationships of couples and affect their quality of life. In idiopathic PE, several helpful techniques and medicines are recommended, but none of them has yielded satisfactory results.Objectives: Our objective in this study was to evaluate the efficacy and safety of clomiphene as a selective estrogen receptor modulator on the treatment of idiopathic PE.Patients and Methods: In a randomized clinical trial, 178 married men with idiopathic PE defined according to the Diagnostic and Statistical Manual of Mental Disorders Third Revised Version (DSM-III-R) who referred to urology clinics over a 10-month period in 2012 were randomized into two groups, namely the study (clomiphene) and control (placebo) groups. They completed self-administered questionnaires that included intravaginal ejaculatory latency time (IELT), erectile dysfunction indexes, quality of life (QOL), sociodemographic characteristics, lifestyle, and medical illness. After 6 months of intervention, all data were compared with the baseline data and between the groups.Results: Within the 10-month study course, 126 patients (70.8%) completed this study. After intervention and comparison of the results between the two groups, IELT, sexual indexes, and QOL improved in the study group, but significant differences were observed only in the IELT and QOL findings.Conclusions: Clomiphene seems to be useful in the pharmacological treatment of PE compared to the placebo.

Abstract: 

Introduction: Premature ejaculation (PE) is themost common male sexual dysfunction. Although its etiology is not fully understood, several organic and psychological disorders have been identified as risk factors. Themain aim of this study is to find any association between religious orientation (RO) and PE. Methods: We designed a cross-sectional study and 80 participants with PE as the main chief complaint were enrolled. After filling informed consent, all participants have filled two questionnaires including the Premature Ejaculation Diagnostic Tool (PEDT) for PE and the Allport & Ross intrinsic-Extrinsic Religious Orientation Scale for RO assessment respectively. Spearman’, s rho analysis was used for assessing the association between PEDT scores and ROS scores by SPSS 22. 0 version. Results: Out of 80 patients with mean age 35. 56 §,8. 46 years, 70 patients (87. 5%) had PEDT positive score. Of them, 45 (64%) had lifelong PE and 25 (36%) experienced an acquired PE. Correlation analysis was shown a reverse correlation between PEDT positive scores and Intrinsic RO (P = 0. 05) with a correlation coefficient-0. 311, PEDT positive and negative scores was not correlated with extrinsic RO (P= 1). Conclusion: Patients with a lower intrinsic religious orientation score have a higher probability for PE and PEDT positive score.
