Abstract:

Introduction: Nipple soreness is one reason that mothers wean their infants from breast milk. In this study we examined the effectiveness of lanolin on alleviating sore nipples. Materials and Methods: One hundred forty seven breastfeeding women were randomly assigned to one of two groups, application of lanolin on nipple (Lanolin group n=74) and no treatment (control group n=73). All women received education about breastfeeding technique. Patients were seen for a maximum of 3 follows up visits within 7 days for resolution of symptoms. The data were analyzed by SPSS version 11.5, Statistical was done by chi-square, Fisher exact, Mann Whitny tests and Mean Rank. P value <0.05 was considered statistically significant. Results: Although both groups received instruction in breastfeeding technique, longer improvement was seen in the group using lanolin for resolution of soreness and repair of wound (p=0.023, and p=0.028 respectively). Conclusion: Because of cost effective, side effect, nipple trauma second to frequent spreading of lanolin on each breast and the use of cream on nipple may introduce pathogens that cause mastitis, and it is not advised for sore nipples.

Abstract Introduction: Nipple problems are the most important reason for early cessation of breastfeeding among mothers. Considering the high prevalence of nipple problems, the present study aimed at evaluating the effect of lanolin cream on the treatment of traumatic nipples. Methods: This double-blind randomized controlled trial was carried out on 80 breastfeeding mothers that attend health centers and the children hospital of Sanandaj-Iran. The participants were randomly divided into two groups (lanolin and control). The control group receives all supports of the intervention group but their cream contains colorless and odorless, Gelatinous. Nipple pain and trauma were measured using Storr and Champion scale. Analyses were carried out through the Chi-square, Wilcoxon signed-rank test, and U-Mann Whitney by using SPSS software version 13. Results: There were no significant differences in pain and trauma score before intervention in lanolin and control groups which were (P = 0. 336) and (P = 0. 21), respectively, but, the results of the study demonstrated that the mean score of pain and trauma in the third, seventh and fourteenth days of study in lanolin group was decreased (P < 0. 001). In intragroup comparison, there was a significant difference among the mean pain and trauma scores the four times in the lanolin group. Conclusions: According to the results of the study, lanolin cream is an effective, simple, and non-invasive technique with no side effects for prevention and treatment in nipple fissures and pain

Abstract:

Background: Anolin, also known as wool wax, is an oily product secreted by the sebaceous glands beneath the skin of sheep. It possesses anti-inflammatory, antimicrobial, and skin-protecting properties, making it a potential agent for treating skin conditions, such as eczema. Objectives: This study evaluates the healing efficacy of lanolin in an experimental model of eczema induced in albino rats. The primary objective of this study is to assess the potent healing role of lanolin compared to glycerin in the treatment of acetone-induced eczema in albino rats. Specific aims include evaluating histopathological changes and measuring the reduction in inflammatory cells in the treated skin. Methods: A total of 48 mature albino rats (weighing 250-350 g) were randomly assigned to two groups, namely a control group and an eczema group induced by applying 99.9% pure acetone to shaved skin using cotton swabs. After three days of acetone application, the eczema group was further divided into two equal subgroups (16 rats each), treated with either glycerin or lanolin. After 24 h, five rats from each subgroup were sacrificed for initial assessment. The remaining rats continued their respective treatments for an additional 72 h before being sacrificed on the third day. Histopathological analysis was conducted to compare tissue sections of eczematous skin with control sections, focusing on changes in the epidermal layer, the presence of fissures and cracks, aggregation of inflammatory cells, keratinization, and skin color changes. Results: Histopathological examination revealed significant changes in the eczematous skin compared to control sections. These included noticeable thickening and elongation of the outermost skin layer, fissures, cracks in the epidermal layer due to skin dryness, and an aggregation of inflammatory cells. Additionally, keratinization of the epithelial layer and changes in skin color were observed. After 72 h of treatment, the lanolin group exhibited optimal healing effects compared to the glycerin group. The outermost layer of the skin returned to its normal length, and there was a marked decrease in the number of inflammatory cells in the lanolin-treated group. Conclusion: Lanolin has a superior healing effect compared to glycerin in the treatment of acetone-induced eczema in albino rats. Lanolin effectively restored the skin’s outermost layer to its normal length and significantly reduced the number of inflammatory cells, highlighting its potential as a therapeutic agent for eczema.

Abstract:

Introduction: Maternal nipple pain is one of the common complaints on the first few days post birth, which is the second cause of early discontinuation of breastfeeding. Lanolin is the recommended treatment for relieving the nipple pain. In addition, purslane has analgesic and anti-inflammatory effects due to containing flavonoids and saponins. Regarding this, the present study was conducted with the aim of determining the effect of purslane cream and lanolin on nipple pain among the breastfeeding women.Material and Methods: This double-blind, randomized controlled trial was conducted on 86 breastfeeding women suffering from nipple pain in Mashhad, Iran, in 2016. The samples were randomly assigned into two groups of purslane cream and lanolin (n=43 cases in each group). Both groups were trained about the correct method of breastfeeding and using cream. The nipple pain scores were measured and recorded by the researcher before the treatment, as well as 3 and 8 days post-intervention using the numeric pain scale. Data analysis was performed in SPSS software (version 22) using the t-test, Fisher's exact test, Chi-square test, and Wilcoxon test. P-value less than 0.05 was considered statistically significant. Results: According to the results, the two groups were comparable before the intervention in terms of pain severity. The two groups showed a significant difference in pain severity on days 3 and 8 post-intervention, compared to the pre-intervention stage, accordingly, the severity of pain was decreased in both groups (P<0.001). Furthermore, there was a significant difference between the two groups regarding the pain severity on the third and eighth days post-intervention (P<0.001). In this regard, the purslane group showed a lower pain severity than the lanolin group. Conclusion: As the findings of the present study indicated, purslane cream was more effective in the treatment of nipple pain than lanolin.

Introduction: The prevalence of nipple pain is reported to be 34-96% which is among the common causes of early breastfeeding stopping. Lanolin is the recommended treatment to improve nipple pain. The anti-inflammatory and analgesic properties of boswellia have been approved. Therefore, this study was performed with aim to determine the effect of boswellia and lanolin ointment on nipple pain intensity in lactating women. Methods: This triple-blind randomized clinical trial was performed in 2018 on 68 lactating women with pain intensity of > 3 in pain visual scale in Mashhad. The subjects were placed in two groups of lanolin and boswellia ointment. The correct method of lactation and drug use were taught to the two groups. The score of nipple pain was recorded by the researcher before the intervention, on day three and eight after the intervention. Data were analyzed by SPSS software (version 21) and T, Fisher, Chi-square and Wilcoxon statistical tests. P<0. 05 was considered statistically significant. Results: Before the intervention, mean pain severity in the two groups of lanolin and boswellia ointment was not significantly different (P = 0. 23). On the seventh day after the intervention, mean pain severity was not significantly different in the two groups of lanolin and boswellia ointment (P = 0. 40). But on the third day, mean of pain severity was significantly different between the two groups of lanolin and boswellia ointment (p <0. 001) and pain intensity was lower in boswellia group than lanolin group. In intra-group comparison, mean pain intensity after the intervention (3rd and seventh day) was lower than the pre-intervention stage, which was statistically significant (P <0. 001). Conclusion: Boswellia ointment and lanolin are same effective in reducing nipple pain.

Abstract:

Background & aim: Despite the global efforts to promote breastfeeding, women report some problems leading to stop breastfeeding in the early postpartum. Nipple soreness is considered as one of the main causes of early breastfeeding discontinuation. Treatment of this problem could keep breastfeeding for a prolonged period of time. Therefore, the present study aimed to investigate the effect of nipple pain treatment with lanolin and purslane cream on frequency and duration of breastfeeding. Methods: This double-blinded, randomized clinical trial was conducted on 86 breastfeeding women with nipple soreness who attended healthcare clinics in Mashhad, Iran, in 2016. The participants were randomly allocated to two equally sized group (43 in each), receiving purslane cream and lanolin for eight days. The correct method of breastfeeding and using the cream were trained to both groups. The nipple pain score was measured before intervention and on the third and eighth days post-intervention using a numeric pain rating scale. The subjects were also trained to record the duration and frequency of breastfeeding during the study period. Data were analyzed by SPSS version 22 using t-test, Fisher's exact test, Chi-square, and Wilcoxon tests. Results: Two study groups were identical in terms of the pain intensity before intervention. There was a significant difference concerning the pain intensity between the two groups on days 3 and 8. The pain intensity in the purslane cream group was significantly lower than the lanolin group (P<0. 001). The frequency of breastfeeding in the purslane group after the fourth day was significantly higher than the lanolin group (P=0. 017). However, there was no significant difference between the two groups regarding the duration of breastfeeding (P=0. 423). Conclusion: Improving nipple pain by purslane cream can promote the frequency of breastfeeding. As a result, it can be considered as an effective agent in treatment of nipple soreness in nursing mothers.

Abstract:

IntroductionSelf-efficacy is an important motivating factor for success in lactation، so that increasing self-efficacy in the postpartum period leads to continued lactation. Performing lactation self-efficacy promotion interventions based on the sources of practical self-efficacy seems to be reasonable. Therefore، this study was performed with aim to compare the effect of Purslane cream and Lanolin for treatment of breast fissure on lactation self-efficacy. MethodsThis double-blind randomized clinical trial was performed on 86 breastfeeding women with nipple fissure in Mashhad in 2016. The subjects were randomly assigned to two treatment groups of Purslane cream and Lanolin (n=43 in each group). Appropriate method of lactation and drug use were trained to both groups and they were asked to use treatment three times a day and up to one week. The score of nipple fissure was measured and recorded before the intervention، the third and eighth day after the intervention using the Store scale by the researcher. The tool of lactation self-efficacy was completed before the intervention and one day after treatment (8th day) through interview with the research units. Data were analyzed using SPSS software (version 22) and t-test، repeated measures variance analysis، Mann-Whitney، and Chi-square. PResultsAccording to the results of Mann-Whitney test، two groups were homogeneous in terms of mean score of fissure before the intervention، but there was significant difference between two groups on the third and eighth day (P <0. 001). So that، recovery was faster observed in the Purslane creamgroup than Lanolin group. Self-efficacy score increased in both groups and this increase was significant in each group (P<0. 001). However، this difference was not statistically significant when two groups were compared (P=0. 434). ConclusionPurslane cream in comparison with lanolin accelerated the recovery process of breast fissure and increased lactation self-efficacy score، although this difference was not significant.
