Abstract: 

Objective: The goal of this study was to design and manufacture the novel spider splint and compare its effect with conventional metallic splint on improvement of hand function in patients with radial nerve injury.Material & Methods: In this quasi experimental study, 24 patients with radial nerve injury from clinics of Sina and Imam Khomeini hospitals who were randomly selected and assigned into two groups by balanced randomized method, were treated with new splint and conventional metallic splint. For the first visit range of motion at Wrist, Metacarpophalangeal, Proximal interphalangeai and Distal interphalangeal joints was measured and evaluated the comfortance level and skin eczema. After 4 months follow up the variables was evaluated again. Data were analyzed by Chi square and Independent T tests. Results: The range of motion of joints at Wrist (p<0.001), Metacarpophalangeal (p=0.003) and Proximal interphalangeal (p=0.029) were significantly more in spider splint group than other group, but there was no significant difference in range of motion of Distal interphalangeal joint (p=0.054), although function improvement of Distal interphalangeal joint was better too in spider splint group than other group.Conclusion: The new splint and conventional metallic splint were effective on improvement of radial nerve problems (fingers disextension, drop wrist and secondary deformity), but new splint was more effective on hand function improvement.

Abstract: 

Backgrounds: Rheumatoid arthritis is a chronic inflammatory disease and rest splinting may reduce inflammation and swelling and pain in acute joints, the aim of this study is to determined effect of Rest splint in reduction of inflammation in wrist and hand joints of rheumatoid arthritis patients.Materials and methods: in 50 rheumatoid arthritis patients (according to ACR criteria 1987) prescribed thermoplastic rest splint in either hands randomly for one month at night, and before and after study assessment of pain (VAS) and duration of morning stiffness and swelling and range of motion of wrist and PIP and MCP joints and hand volume and grip strength performed.Results: in final assessment of splinted hand there was significant less pain (p=0.002), less morning stiffness (p=0.01), less swelling (p=0.0003), less tenderness (p=0.004) and better range of motion of wrist (p=0.0005). In addition in final assessment in splinted and unsplinted hands there was less swelling (p=0.006), less tenderness (p=0.03) and better range of motion of the wrist (p=0.04) in splinted hands, but there was not any significant change in pain and morning stiffness, joint count, flexion of MCP and PIP joints, hand volume and grip strength.Conclusions: according to results we offer more using of this splint in treatment of RA patients.

Abstract: 

Rigid and soft splints are used in septorhinplasty for fixation, prevention of adhesion, straightening of the septum, and repair of septum perforation. Recently surgeons have been using the soft splint, which has a channel through which the patient can breathe after septorhinoplasty with tampon. Instructions: One of the most important problems in septoplasty is the return of the deviation of the septum in the long term; i.e. if the septum has a convexity or concavity, despite the septoplasty, it tends to move to the previous situation. The reason is that the septum is not completely dissected, and this deviated part is stitched to the upper part or the perpendicular plate of the ethmoid and in long time moves this deviated section. In septoplasty, we dissect the septum in lower –infront back part, and we keep the upper part. We create the swimming door if a deviation exist in this part in long term may be move to previous deviation. In septoplasty, after the dissection, the surgeon may be morselization or cut the septum and suture and fix the septum with tampon after eliminate the tampon till to 3-6 mount septum maybe deviate. We using rigid splint in long term (612mounths) and has good results in keeping septum straight mode.

Abstract: 

BACKGROUND AND OBJECTIVE: Carpal tunnel syndrome (CTS) or compression neuropathy of median nerve in carpal tunnel is the main cause of pain, numbness and tingling in the upper limb. The most common conservative interventions in mild to moderate CTS are wrist splints. The purpose of this study was to compare the effect of the Manu splint to wrist cock-up splint on the symptoms and functional status in CTS.METHODS: In this clinical trial study, we randomized 20 hands (12 female patients) with CTS into a group wearing the splint that keep 3rd and 4th fingers in extension (Manu splint) and into another one wearing the short cock-up splint with natural wrist angle, at night for 4 weeks. The patients were evaluated and compared after 2nd and 4th weeks by Boston Carpal Tunnel Questionnaire (BCTQ) and Pain Visual Analog Scale. The questionnaire comprised two parts: symptoms severity and functional statues that maximum scores were 55 and 40 for symptom severity and functional statues, respectively. Maximum score was 100 for PVAS.FINDINGS: The average symptom scores of BCTQ and pain scores significantly reduced in both groups after 4 weeks. Functional statues score was diminished significantly only in Manu splint group after 2 weeks (p=0.004). In comparison between 2 groups, after 4 weeks, the mean symptom severity score in Manu splint group was -31.15±15.37 and in cock-up splint group was -51.38±15.21 (p=0.008), the mean pain severity score in Manu splint group was -61.72±34.86 and in cock-up splint group was -98.59±2.96 (p=0.004). After 4 weeks no significant differences exited between 2 groups in term of functional status.CONCLUSION: The results of this study show that a 4 week treatment with either the hand brace or the wrist splint induced symptomatic benefit in patients with CTS. In the orthotic treatment of CTS, wrist posture is more important than finger posture.

Abstract: 

Background and Aim: The aim of the present study was to investigate the effect of the Hand Static Rest Splint on reducing the pain and other compliance of Rheumatoid Arthritis (RA). Materials and Methods: In the present clinical trial, 12 patients, aged 35-50 (average: 39 years) with RA, referring to the Occupational Therapy Unit of Taleghani hospital and meeting America College of Rheumatology diagnostic criteria, were included. Visual Analogue Scale (VAS) was administered and swelling, tenderness and morning stiffness were assessed. Moreover, assessments of active range of motion using goniometry and grip strength with Jamar dynamometer in both hands were performed. Statistical analysis of data was performed using t-test and Mann-Whitney Test for two independent samples and willcoxon tests and paired t-test for paired samples run in SPSS, version 18. The level of significance was set at p˂ 0. 05. Results: The findings showed that the Static Rest Splint led to improvement in the wrist range of motion (flexion p=0/005, extension p=0/03) when the two groups were compared, but it did not have a significant effect on Morning Stiffness (p=1), pain (p=0/1), Swelling (p=0/06), and Tenderness (p=0/2). However, when compared between after and before intervention in hand with splint, pain (p=0/02) and wrist range of motion (p=0/004) improved. Conclusion: Based on the findings of the current study, and since using splints only improve range of motion in the wrist compared between two groups and pain relief before and after the intervention, it seems that using hand Static Rest Splint to reduce other symptoms need further investigation and also given that the use of this position without stress on the tendon can be effective in reducing the causes deformity, further studies are needed to more delineate these points.

Abstract: 

Background and Aim: In clinical practice it is commonly assumed that occlusal splints have therapeutic value in the treatment of temporomandibular disorders (TMD), however there is still lack of sufficient evidence to support this claim. The purpose of this double-blind study was to evaluate the short-term (8-week) efficacy of a stabilization splint in subjects with temporomandibular disorders seeking treatment.Materials and Methods: Seventeen TMD patients were treated with stabilization splints for eight weeks. In addition to filling out questionnaires, all patients underwent a preoperative and postoperative clinical-functional investigation of the masticatory system. Qualitative data were analyzed using Fisher exact and Chi-square tests and Wilcoxon signed ranks test was applied to report quantitative data.Results: A statistically significant decrease was observed postoperatively in the Helkimo clinical dysfunction index (P=0.000) as well as the anamnestic dysfunction index (P=0.020). In addition there was a statistically significant decrease in muscle tenderness, after treatment (P=.002).Conclusion: The results of this study indicate that the use of a stabilization splint is beneficial in treatment of the signs and symptoms of TMD, and especially has a positive effect on muscle tenderness.

Abstract: 

(ID) Internal Derangements دسته مهمی از اختلالات TMJ را تشکیل می دهند که از نظر شیوع در مرتبه دوم بعد از اختلالات عضلانی قرار دارند. درمانهای متعددی برای این گروه از اختلالات ذکر شده است که شامل تصحیح اکلوژن، استفاده از انواع اسپلینت های اکلوزالی، تکنیکهای مختلف Manipulation و درمانهای جراحی است.در این تحقیق 26 مفصل مبتلا به ID در 16 بیمار تحت درمان با Anterior repositioning splint قرار گرفتند. نتایج به دست آمده از بیماران با گروه کنترل که تحت چنین درمانی قرار نگرفته اند مقایسه شد.بین دو گروه از نظر بهبودی و حذف صدای مفصل، درد مفاصل TM، درد و محدودیت حرکات فکی اختلاف معنی داری در محاسبات آماری وجود داشت.این مطالعه نشان داد (A R S) Anterior repositioning splint یک روش مفید و محافظه کارانه برای درمان اختلالات داخلی مفصل گیجگاهی فکی است.

Abstract: 

Background and Objective: Carpal tunnel syndrome (CTS) is one of the most common disease among the entrapment neuropathies. The purposes of this study was to compare the efficacy of a new dorsal wrist splinting versus common palmar splinting for CTS based on sensory nerve conduction measurements.Materials and Methods: This single blind randomized control trial study was carried out on 22 idiopathic CTS patients. Subjects were randomly divided in two groups: Dorsal splint group (n=12) and palmar splint group (n=10). Both groups used splints for 4 weeks. Sensory conduction study of median nerve were done initially for having baseline and after 4 weeks follow up. SPSS-16 and Kolmogorov–Smirnov, independent T, and paired T tests were used for analysis of DataResults: After four weeks median nerve sensory distal latency and conduction velocity improved significantly in both groups (P<0.05). There was significant difference between both groups due to electro-diagnostic improvement.Conclusion: This study showed that based on electro-diagnostic method, dorsal wrist splint is more effective than cockup splint in carpal tunnel syndrome.
