Background: Escalating economic costs due to prevalence of diabetes, particularly in epidemiologic transition era, makes assessing of socio-economic status of diabetic patients as a necessity.Materials and Methods: A cross sectional study was conducted on type 2 diabetic patients with complications in four major teaching hospitals affiliated with TUMS during July 2009 to March 2010. Based on a randomized sampling, 530 patients were interviewed through a questionnaire with 85% response rate (450 patients). The applied questionnaire composed of 42 questions based on “Yes” or “No”, and likert questions. Internal validity of applied tools in this study was 0.89. Data analyzed by SPSS version 11.5.Results: The majority of patients had experienced one or more complications. Findings revealed that (50%) of the patients suffered from one complication, (33.6%) had two complications, and (16.4%) had three complications of type 2 diabetes. Patients with one complication (22.2%) had cardiovascular, participants with two complications (12.7%) had both cardiovascular and eye outcomes, and patients with (cardiovascular, eye and foot ulcer) outcomes were (14%). In the year 2006, some changes were applied and healthcare sector was separated from other segments. Also in the year 2010, the model revised but there was no introduction of any specialized model for healthcare sector. The aim get of this article is to describe the necessity of re-conceptualization of EFQM model for healthcare sector to be applied in National Productivity & Excellence Award.Materials and Methods: Showing the necessity of re-conceptualizingexcellence model in the healthcare sector, two questions were carried out: “Is there any evidence of using excellence models (especially EFQM model) by the healthcare sector (especially hospitals) for improvement planning?” (In fact, is it demanding by healthcare sector to specialize and re-conceptualize the EFQM model?) And if the answer of this question is yes, “What is done so far now to specialize and re-conceptualize the EFQM model for healthcare sector?” to answer the two mentioned questions, different studies were done in science bases. The gathered sources and materials divided in three groups for more reviews. Results: The findings of studied sources were presented in three following groups: “Proportionate the excellence models for healthcare sectorand using of it”, “Assessment and performance management in base of organizational excellence models in healthcare sector”, and “Statistical examination of the effectiveness of using organizational excellence models and quality models in healthcare sector”.Conclusion: The study shows that there are too many experiences of using excellence models (especially EFQM model) in healthcare sector (especially hospitals) for improvement planning, but there was no experience of specializing and re-conceptualizing the EFQM excellence model for healthcare sector so far. Abstract:

Background: in activity based on costing, first, activities determined, then direct and indirect costs of activities based on allocation methods computed. The aim of this study was to compute the cost price by using Activity Based Costing (ABC) method in a radiology ward of Golestan hospital in Ahvaz University of medical sciences. Methods and materials: This is an applied, cross-sectional study. In this research, the cost prices of 35 routine services of radiology wards (based on a table in public definition) calculated through using activity based on costing method in Golestan hospital in Ahvaz University of medical sciences. Data gathering tools included lists and the pertain Forms to compute the direct and indirect costs of radiology services and other activities in centers. To calculate of proportion in other wards and radiology units, first, allocation and then costs were determined, and then direct allocation method was used. Results: The results showed that the cost prices in three services including: Lamb sacral (6348 Rails), Sulumar (3995 Rails), Vertebra and Back (12001 Rails) were less than public prices based on TAREFE, however, in others services (23 services) there were significant more pertain prices in comparison to public prices based on TAREFE in years of 1387 and 1388. Generally, the costs pertain of radiology services computed 5795152383 Rails that was the most proportion (43.3 percent) related to workforce costs. Conclusion: Based on the results there was differentiation between cost prices of services and public TAREFE. This differentiation appears as hazards or financial crises to the hospital. To improving financial systems in hospitals and reducing of financial costs, it should be used the right methods to compute the costs relating to Activity Based Costing to services prices.

Abstract:

Background: Hospitals play the most important role in health care system utilizing the majority of the health system resources. Appropriate and efficient management of hospitals could decreases the cost of diagnostic and therapeutic services while improve the quality of services. Efficient hospital management requires recognizing and prioritizing factors affecting hospital performance and choosing the correct interventions. In the current research managerial factors which affect Tehran Heart Center's (THC) performance were recognized and prioritized.Materials & Methods: A qualitative study using interview technique was performed in 2010 to identify managerial factors affecting T.H.C.'s performance. Top and middle line managers were included in this study. To prioritize the factors a questionnaire was designed and used. The results were analyzed by one sample t-test and Fuzzy DEMATEL techniques. Results: In this study 9 managerial factors affecting on T.H.C.'s performance were recognized which 7 of them were influencing based on the positive (D~i-R~i) and the others were influenced. The factors according to their intensity based on the quantity of (D~i+R~i). So the influencing factors' ranking were as follow: delegation (3.16), management accountability (2.96), appropriateness of authority and responsibility (2.67), management motivation (2.61), process approach (2.26), systematic approach (2.25) and Top Management stability (1.68).Conclusion: Paying more attention and focusing on the main influencing factors such as delegation, management accountability, authority and responsibility appropriateness, top management stability and management motivation, lead to higher performance in the hospitals.

Abstract:

Background: Standards play an important role in evaluation and monitoring organizational procedures; In the other hand medical personnel performance has a major influence on increasing efficiency, effectiveness and productivity of the hospital. In this research we are going to understand weather environmental, equipmental, immunity, hygiene standards has effect on personnel performance in Ors or not.Material &Methods: the project was happened in OR of the 10 hospitals affiliated by Esfahan University of Medical Science. There are 2 check lists used in this project: one checklist for standards in 4 parts and the other one for personnel performance. After gathering basic information they were put on SPSS soft ware for further processes. for analyzing the information both analytic and descriptive statistic methods were used. Results: The result shows that there is a significant correlation between following environmental standards and improving personnel performance; also between personnel gender and their performance a relationship has found: female follow the instructions better and have higher rate of performance than men. No significant relation between other variables and performance were found. Conclusion: it seems like by following standards in designing the environment of the Ors we can heighten the personnel performance; it can be concluded that with the appropriate design, unnecessary movements decreased, so personnel can do their job better. Also we should find out why men have lower performance, maybe should find we some ways to make our male personnel more eager to their job to increase their performance or educate them and etc.
