Abstract:

Introduction: Propofol can be used for intravenous sedation in gastrointestinal endoscopic procedures but a safe and effective dose for outside operating room usage is not defined. In this single blind clinical trial we compare safety and efficacy of low dose intravenous propofol sedation (0.8mg/kg stat and 25µg/kg/min maintenance) with intravenous midazolam sedation (0.1mg/kg).Method: During a 7 month period of this clinical trial, 140 consecutive patients undergoing routine colonoscopy and ERCP randomly received either midazolam or propofol for sedation. Vital signs (systolic blood pressure, heart rate, respiratory rate and SPO2) were continuously monitored for safety measurement and procedure related variables, patient's autonomic and somatic response to stimulus, the recovery time and postanesthesia discharge time, patient’s intraoperative awareness and gastroenterologist satisfaction were assessed for efficacy measurement of two methods. Results: In propofol group mean systolic blood pressure changes (initial systolic blood pressure vs. maximum systolic blood pressure during procedure) was less than midazolam group (P<0.05). Mean respiratory change (initial respiratory rate vs. minimum heart rate during procedure and initial SPO2 with room air vs. minimum SPO2 during procedure with 5 lit/min O2) had no differences in two groups. Apnea and hypoxia was not observed in any patient. Mean recovery time and mean post anesthesia discharge time were significantly lower in propofol than midazolam group. Other variables had no significant differences in midazolam and propofol group. Conclusions: Low dose propofol sedation (0.8mg/kg stat and 25mg/kg/min maintenance) is safer and more effective than midazolam (0.1 mg/kg) sedation and is associated with less systolic blood pressure changes, faster recovery and shorter postanesthesia discharge time.

Abstract:

Introduction: The present study was conducted to assess the effect of BCAA supplementation on serum C - reactive protein, after acute resistance exercise in soccer players. Methods: 20 soccer players in a randomized one-blind design completed in random 3 sets of 8-10 repetitions of 7 exercises. Subjects were randomized to two ten-member groups and consumed 200 mg. kg-1 BW of either BCAA or dextrin (placebo) 30 minutes prior to exercise. To identify HS-CRP, venous blood samples were obtained 30 min prior to and immediately following exercise and at 1 hr, 2 hrs, 24hrs post exercise. Data were analyzed using 2-way repeated measure ANOVA and Bonferroni post hoc test. Results: Baseline serum values for Hs-CRP were not statistically different between groups in the 30 minutes before the exercise test (P>0.05). However there were significant increases (P<0.05) between the pre exercise and post exercise values for Hs-CRP from 24 hrs posttest, (P<0.05). Importantly, the BCAA supplementation significantly reduced this Hs-CRP 24hrs post-test (P<0.05).Conclusion: These results indicate that supplementary BCAA decreased serum concentrations of the inflammation biomarker Hs-CRP following resistance exercise. This observation suggests that BCAA supplementation may reduce the inflammation biomarker associated with resistance exercise.

Abstract:

Introduction: Illness perception is the beliefs that patients have about their illness and it influences patients’ decision for effective behavior to achieve positive results such as adapting with the disease and improving functionality. Since there are no studies about illness perception of MS in Iran, the present study aims to evaluate the impact of peer group on illness perception level in patients with MS.Methods: This was a quasi-experimental before-and-after design research that was conducted on 33 patients with MS in three supporting peer groups (10 participants in men group, 11 in women group, and 12 in the mixed group). The participants attended two-hour sessions for eight weeks. Illness perception level in patients was assessed before and after the formation of groups by the "Revised Illness Perception Questionnaire (IPQ-R)” whose Cranbach’s alpha coefficient was 0.80.Results: The results showed that although the mean score of the patients’ illness perception increased, but it was not significant. Between three groups, attending peer support group increased illness perception in the mixed group (p=0.01).Conclusion: The findings of the present investigation asserted that participating in peer groups ends in augmentation of illness perception level in MS patients also, as social support create a feeling of belonging and thus may help sufferers of MS adapt better to life events then it would be beneficial to develop interventions for MS patients based on social support and illness perception. The obtained results of this study can be utilized in education of medical groups and clinical management.

Abstract:

Introduction: Under sedation and over sedation cause several problems for critical care patients. There are no truly validated instruments for monitoring intensive care unit sedation. The purpose of this study was to determine the validity and reliability of Glasgow Coma Scale Modified by Palma and Cook in adult patients hospitalized in intensive care unit by untrained evaluators. Methods: The study is analytical. After translating the English version of (GCSC) into persian through international Quality of life assessment method, eight surveyors (principle investigator (PI), six nurses and one resident anesthesiologist), 70 patients hospitalized in Critical Care Unit were selected through purposeful sampling. PI and all investigators evaluated the patients independently and recorded a GCSC score. In addition, the PI marked a 10cm Visual Analog Scale and recorded Richmond sedation agitation score in order to determine of validity GCSC. Evaluators had not received training about Glasgow Coma Scale Modified by Palma and Cook. Results: Interclass correlation coefficient between inter-rater in untrained group for Glasgow Coma Scale Modified by Palma and Cook showed 0.898 and the weighted kappa score for interrater agreement was 0.894with, P>0.001. Also positive and significant (P<0.001) correlations between GCSC score with visual analog scale (r=0.91), with Richmond sedation agitation scale (r=0.897) and with Glasgow Coma (r= 0.897) were observed. Conclusions: Validity and reliability of Glasgow Coma Scale Modified by Palma and Cook for assessing the level of sedation in adult patients hospitalized in intensive care can be approved even by untrained evaluators.

Abstract:

Introduction: The role of community assessment is to identify factors which influence the health of community and to determine the accessibility of the sources within the community. This study was carried out with the aim of identifying the problems of two different regions in Shahroud and of selecting a problem for a deeper analysis and determining the size of the problem in the two regions. Methods: Using the 8-stage model of North Carolina community Assessment, the problems of two regions of Mahdiabad and Bastam were identified and prioritized. Then, the mental problem was selected as a common priority problem in the two regions and in order to determine the size of this problem, 450 people in Mahdiabad and 407 people in Bastam were selected through random cluster sampling and were studied using a demographic questionnaire and the general health questionnaire (GHQ-28) and WHO-5 well- being index. Results: Mental health problem was the sixth priority problem in Mahdiabad and the fourth priority problem in Bastam. Overall prevalence of undesirable mental health (confidence interval) in Mahdiabad and Bastam were 43.9% (39.3-48.5) and 25.1% (20.9-29.3). There were greater percentages of men and women with mental health problems in Mahdiabad than those in Bastam. In terms of somatic and anxiety sign subscales, Bastam, and in terms of social function and depression subscales, Mahdiabad had more undesirable conditions. Conclusion: The process of community assessment is a highly useful tool for needs analysis, identification and solution of society problems and its application is suggested for promotion of health and welfare of any community.
