Abstract: 

Introduction: Treatment of gingival recession has been one of the most important aims of periodontal procedures and therefore introduction of a predictable technique for covering the denuded roots has been an ideal goal for many years. The purpose of this study was the evaluation of root coverage by "Semilunar coronally repositioned flap"; technique; and the role of topical application of tetracycline hydrochloride in improvement of the results.Methods and Materials: 24 anterior and premolar teeth were selected from 9 patients and divided randomly in two "test and control"; groups. After completion of initial procedure and before operation, some quantitative (Vertical recession, depth of gingival sulcus and width of keratinized gingiva), and qualitative variables (Sensitivity of root surface to air flow and feeling of an unsatisfactory appearance by the patient oneself) were measured. In test group, tetracycline 1% (pH=2.3) was applied 3 minutes by burnishing method on the surface of root, before performance of surgery.The patients were visited at the days 7, 14, 30, and 90 after the operation, and healing of the area and hygiene status were evaluated.Results: Following statistical analysis, it was concluded that recession depth has been reduced significantly in both groups, 1 and 3 months after the operation.• The mean root coverage in test group was 2.29mm (70.33% coverage), and 2.16mm (67.33% coverage), 1 and 3 months after surgery, respectively.• Width of keratinized tissue was also increased significantly in both groups.• Among the quantitative variables, only probing depth did not alter during the period of research.• Qualitative variables were also improved significantly in both groups.• Apparently, the application of solution as a "root conditioning agent"; did not influence the results of this study.Conclusion: In summary, the operation technique may be an appropriate procedure for the coverage of the anterior teeth in maxilla, based on its unique advantages.

Abstract: 

Introduction: Systemic disease is often closely linked to the state of the oral cavity. One of these systemic diseases is diabetes mellitus. The purpose of this study is to determine the effect of scaling androot planning and mouth rinse on blood sugar level in type 2 diabetic patients.Methods and Materials: 30 uncontrolled type 2 diabetic patients suffering from sever chronic periodontitis were selected and divided into two groups; each group consisted of 15 patients: Firstgroup (control): received oral hygiene instruction. Second group (case): received scaling and rootplanning plus oral hygiene instruction and chlorhexidine degluconate mouthrinse. In all groups,probing depth (in 6 sites of each tooth), bleeding index (in 4 sites of each tooth), fasting blood sugarand glycated hemoglobin were measured at first and three months after treatment. Then the before andafter treatment results were compared.Results: Mean fasting blood sugar in second group, reduced from 184.2 to 163.87mg/dl. Glycatedhemoglobin in this group reduced from 8.78% to 7.29% (p<0.05). Bleeding index, plaque index andprobing depth reduction were statistically significant before and after treatment (p<0.05).Conclusion: Phase one periodontal therapy improves periodontal conditions and reduces fastingblood sugar and glycated hemoglobin.

Abstract: 

Introduction: Pulpotomy is one of the most common treatments in Pediatric dentistry. Various modalities and materials have been applied in human and animal clinically. These procedures have been evaluated clinically, radiographically and rarely histopathologically. The purpose of this study is comparison of histopathologic reaction of dog’s dental pulp to formocresol (standard procedure) and ferric sulfate (new modality).Methods and Materials: Forty dog’s sound permanent teeth were selected. After Anesthetizing of animal, 20 teeth was randomly pulpotomised, with formocrosol and 20 teeth with 15.5% ferric sulfate. After one month, animal was anesthetized again and sacrificed by vital perfusion. After that, teeth were dissected from Jaws and sent to Isfahan oral histopathology lab. Histologic sections were blindly observed by oral pathologist and reported state of inflammation, depth of inflammation, calcification, dentinal bridge, internal resorption and necrosis. Then frequency and relative frequency of these parameters were analyzed by Chi-square and Mann-Whitney in SPSS software.Results: Inflammation was not observed in 70% formocresol group and 75% ferric sulfate group. Calcification was not observed in 75% ferric sulfate group and 80% formocresol group. Necrosis was not observed in 55% ferric sulfate group and 65% formocresol group. Dentinal bridge was not observed in 65% formocresol group and 45% ferric sulfate group.Conclusion: Considering the findings of this study and other studies, insignificant differences between the results in two groups, and also other aspects and characteristics, ferric sulfate can be one of proper alternative pulpotomy modalities in primary dentition and developing permanent teeth.

Abstract: 

Introduction: The epinephrine in local anesthetic solution induces elevation of blood glucose. Also anxiety can produce this effect. In this research, blood glucose changes during surgical removal of impacted third molar with use of lidocaine+epinephrine -as a local anesthetic- were evaluated.Methods and Materials: In this prospective analytic-descriptive study, 21 healthy patients, among the patients who came to the faculty of dentistry of Isfahan for mandibular impacted third molar surgery, was chosen. Preoperative orders including, eating breakfast 3 hours prior to surgery, and nomedication consumption prior to operation, were served to every patient. Then, their blood glucose were tested in 4 stages (15 minutes before surgery, immediately before use of local anesthesia, 7 & 15 minutes after that) with Accu Check glucometer; and result were analyzed with SPSS software.Results: Blood glucose changes through the 4 stages were significant (p<0.001). Mean blood glucose level which was 97.19±9.93mg/dl in stage 1, was raised to 107.48±15.57 mg/dl in stage 4. Mean blood glucose level slightly decreased in stage 2, and then increased significantly in stage 3 and 4.Conclusion: After infiltration of local anesthesia and start of surgical process, the level of blood glucose was increased; which is similar to Nakamura and Meechan studies. This result showed that blood glucose doesn’t increase the normal range during dental surgery using two carpools of lidocaine2% pulse epinephrine 1/80.000.

Abstract: 

Introduction: Today a new restorative resin called “Class V cervical restorative resin” with its specific surface sealer “Seal-n-Shine” has been introduced to use for reducing of micro leakage in class V restorations. The aim of this study was to evaluate the influence of surface sealer or self-etch primer on micro leakage around class V cavities filled with “Class V cervical restorative resin”.Methods and Materials: In this in vitro experimental study, wedge shaped cavities were prepared on facial surface of 40 intact premolars, using No. 56 carbide bur. The occlusal margin was prepared in enamel and the cervical margin was in cement. The teeth then were divided into 4 groups of 10. Four groups of materials were used, one for each group of the teeth: class V restorative resin; class Vrestorative resin and its specific sealer; self-etch primer of Clearfil SE Bond and Class V restorative resin; and self-etch primer of Clearfil SE Bond, class V restorative resin and surface sealer of Clearfil SE Bond. Then all the samples were thermo cycled (1500 cycle, 5-55). Afterwards they were placed in 50% silver nitrate solution and developing solution, respectively. Finally they were sectione=dlongitudinally and were evaluated by stereo-microscope at × 28 magnifications. The data was analyzed using Kruskall-Wallis and Man-Whitney tests.Results: There were no significant differences in micro leakage of enamel margins between the four groups. But, considering the dentinal margins, the differences were significant between groups 1, 2; 1, 3; and 1, 4. The differences between the other two paired groups were insignificant.Conclusion: For decrease of micro leakage, the usage of class V cervical restorative resin per se is not effective, and it is better to apply its specific sealer (Seal-n-Shine) on the surface of restoration, or a self-etch dentin bonding agent under it.

Abstract: 

Introduction: There are various reports regarding the frequency of Temporomandibular joint (TMJ) involvement in Rheumatoid Arthritis (RA). Manifestations of TMJ involvement, which was investigated in this study, are: joint pain and sounds, muscles tenderness, morning stiffness, decreased force bite, and radiographic changes.Methods and Materials: 80 RA patients and 80 normal persons were included as case and controlgroups respectively. The diagnosis of RA was determined according to the criteria of American College of Rheumatology (1987) and confirmed by a rheumatologist. Inclusion criteria for control group were similarity of age and sex with RA group, and no history of RA or other bone-joint diseases.All patients were assessed for clinical signs and symptoms of TMJ involvement.Results: In all aspects, the frequency of TMJ involvement was significantly higher in RA group than control group (p<0.00l). Crepitus was the most prevalent joint sound among RA patients. The incidence of TMJ manifestations, increased with increasing age and duration of RA (p<0.00l). 61.2% of RA patients had at least one of the 4 signs of TMJ involvement, including pain, sounds, muscle tenderness and limited opening. In control group the prevalence of this finding was 38.7%. 61% of RA patients had bilateral involvement.Conclusion: According to this study, prevalence of TMJ involvement is significantly higher in RA patients than normal individuals. However, patients may not complain from joint pain or muscle tenderness, but there may be other signs and symptoms such as limited mouth opening and joint sounds-which could be indicative of TMJ problems. According to the results, the frequency of TMJ involvement increased with increasing age and duration of RA affliction.

Abstract: 

Introduction: Ewing's Sarcoma is a distinctive primary malignant tumor of bone that is composed of small undifferentiated round cells of uncertain histogenesis. 4% of cases involve craniofacial bone, and 1% is seen in jaws.Case report: The patient was a 16 years old girl that referred for a nodule in the soft tissue of vestibule near the roots of canine and premolar of maxilla, with bone invasion. Diagnostic procedures until final diagnosis and treatment of this case have been reported.Conclusion: Ewing Sarcoma is occurred in 90% of cases between 5 to 30 years and average of patients with head and neck involvement is 11 years. Most of jaw lesions are localized in ramus and few cases have been reported in maxilla. Rare soft tissue cases of Ewing's Sarcoma have also been reported. In related report, the features of this case have been compared with previous studies.

Abstract: 

Introduction: Metastasis of malignant tumors to oral cavity is rare. Distal area of mandible is a common location for metastasis. In this paper, we report a case with metastatic adenocarcinoma to mandible, postpone in its diagnosis, and mismanagement. Also, we review the clinical and radiographic signs of the metastatic tumor in jaw bone. Watching clinical signs and symptoms, leads to quick diagnosis; and this means, increasing survival of patients.Case Report: The patient was a 48 year old woman. She had visited a dentist because of pain in distal area of her mandible. In spite of endodontic treatment, pain had not resolved. Then, she came to our clinic. Regarding past history of colon adenocarcinoma, and further clinical evaluation, it seemed the lesion may be a metastatic tumor. Biopsy, and evaluation by a pathologist, confirmed the diagnosis.Conclusion: Although metastasis to the jaw bone is rare, careful assessment of past medical history and attention to clinical signs, like lip paresthesia, and radiographic findings can prevent unnecessary treatment, increase survival, and reduce the problems of patients suffering oral metastasis.
