Abstract: 

Introduction: Informed consent is a legal document whereby patients' relatives (patients) or his legal representatives, their written consent for treatment to be announced. Consent before treatment is legal rights of patients. This study aims to examine the legal validity of consent obtained from hospitalized patients was conducted at the University Hospitals.Methods: This study was a applied and cross - sectional. The research community and inpatient medical records of patients who consent process was conducted in compliance unit, From April to July 1391 were a teaching hospital of Kashan. Systematic sampling and medical records of 266 patients were selected.3 check lists associated with each treatment and surgery consent, Discharge with personal taste and use of medical information was collected through interviews and observation. Data from the SPSS software and were analyzed using descriptive and inferential statistics. To examine the relationship between variables and the significance level of the chi-square test and Fisher exact tests were used.Results: Legal validity of consent for treatment and surgery, 60 percent, and discharge with of personal taste was 76/1%. Also in consent of Use of medical information from 89 consents, only 30 had been completed and 70 percent of the amount was the credit.Conclusion: The results of this study showed that some consents were obtained from the low credibility And the need to improve the quality of patient consent necessary measures to be taken.

Abstract: 

Introduction: Hospital information systems are one of the most important systems in improving the quality of health care. Because of the enormous costs that would be spent for installation and implementation of health information systems, it is important to evaluate these systems. The purpose of this study was to evaluate the performance of Hospital Information Systems in the Laboratory module of University Hospitals of Isfahan based on the International standard ISO 9241-110.Methods: This was an applied, descriptive study and was performed in 2013-2014. The Statistical population included 225 laboratory users in hospitals affiliated to Isfahan university of Medical Sciences. Sampling method was random stratified sampling and the Sample size was calculated based on Cochran formula as 141 subjects. Data were collected by using a standardized Isometric questionnaire 9241-110. The questionnaire contains 35 questions in seven sections. Its validity and reliability were confirmed by Information Technology specialists and Chronbach's alpha (r=0.95) respectively. Data were analyzed by descriptive (measures of frequency, frequency percentage, mean, standard deviation) and inferential statistics (Independent T-test and Anova) by using the SPSS software.Results: Findings showed that all hospital information systems (KOSAR compliance averaging 3.65, Rayavaran Tosee compliance averaging 3.47, Sayan Rayan Ekbatan compliance averaging 3.25, Pooya Samaneh Diva compliance averaging 3.09) based on the principles of ISO 110-9241 was upper than average. Data analysis suggested the following percentages concerning the software: Suitability for the task 3.81, Self-descriptiveness 3.68, Conformity with user expectations 3.67, Error tolerance 3.45, Controllability 3.44, Suitability for learning 3.42, Suitability for individualization 3.05.Conclusion: System designers should consider to promoting the Seven Principles of Iso 9241-110 and specially paying attention to improving the ability of self-learning and Suitability for individualization principles.

Abstract: 

Introduction: Telemedicine is defined as using telecommunication and information technology for sharing information and medical care from distant places in medical scopes. However, it seems that this technology does not have a strong position in Iran yet. Therefore, the main goal of this research is to identify and analyze barriers to implementation.Methods: This research which is an applied research is in the category of descriptive researches. The population of this research is all doctors throughout Iran which is about 100, 000 doctors. The research has been done in the end of 2013and the beginning of 2014 in Iranian date. The sample has been chosen judgmental and the sample size has been estimated about 384. A Likert scale questionnaire asking for the views of doctors teaching in medical sciences universities and those who work in hospitals throughout Iran was the main research instrument in the study. Validity and reliability of this research has been confirmed by the idea of experts and Chronbach’s alpha respectively (alpha=0.86).Results: Findings of this study showed that all of the indices were key barriers to implementing telemedicine in Iran because their loading factor was more than 0.3. The categories have also been confirmed by the second order factor analysis.Conclusion: As long as these barriers exist, telemedicine can’t be implemented effectively, so future studies should be focused on finding solutions for these barriers.

Abstract: 

Introduction: considering the information threats and the need to procedures for develop and improve security and confidentiality, international standard organization (ISO) established information security standard ISO/IEC 27001. Getting ISO/IEC 27001 standard certificate helps the organization to identify the problems and defeats in its departments and processes, in addition to promoting organization’s competitive position and giving the organization the competitive advantage that it needs. The goal of this study is to evaluate information management systems in Isfahan University of Medical Science using ISO/IEC 27001 standard.Methods: This applied research is a descriptive study. Research community is all departments of information technology at Isfahan University of Medical Science, computer centers of faculties and hospitals, in 2011. In this research we used ISO/IEC 27001: 2005 international checklist as a tool for collecting the information. The checklist includes 11 primary parts and each part includes several additional parts and questions. The information was gathered through interviewing, observation and documents of researchers and was analyzed by Excel 2010.Results: the assessment results indicates that in standard main parts including security policy, organization of information security, asset management, human resources security, physical and environmental security, communications and operations management, access control, information system acquisitions, development and maintenance, information security incident management, business continuity management and compliance, the organizations implemented 31, 40, 28, 65, 73, 54, 54, 44, 58, 38 and 54 percent of the requirements.Conclusion: considering the importance of developing information security management in organizations that deliver information technology services and also the importance of international standard ISO/IEC 27001 in establishing the organization’s processes based on information security and confidentiality protection, integrity and accessibility, the organization should put more effort into implementing this standard in its processes. The results indicate that except for the human resources security and physical and environmental security areas, the organization didn’t develop information security management requirements properly in its internal processes.

Abstract: 

Introduction: The evaluation and ranking of scientific journals is according to differences between them, a single indicator of assessment, despite its high importance cannot exactly reflect the quality of the articles. The analysis not only on differences between the journals, but they also occur on the background reasons. In addition, it is useful to improve the academic standards of the journals that rank lower are considered a pattern or a measure from the floor up magazines. For this purpose, in this paper, the model of DEA provides Multi Criteria model will be used.Methods: This research is applied research, which was conducted in 2014, and data required by referring to the site of “Web of Knowledge” and obtaining characteristics of 174 scientific journals in the management field was extracted. Among this number, 115 of them have had Journal Impact Factor 5-year-old Showing that it is at least 5 years. Then, 22 Journal Impact Factor 5-year average was between 0.5 to 2, Which was used as a model to study, The input oriented and output-oriented CCR model and the collective model SBM in both cases constant returns to scale and variable returns to scale, by Using QSB software, for a efficiency measure used magazines and efficient units were ranked according to the AP.Results: According to the evaluating of defined inputs and outputs, 11 Journals was non-efficient And other 11 Journals was efficient. The Journals those entries from a lower side and the output side of the high Located in a higher position, in ranking efficient units. Journals that their efficiency was evaluated under an one, Improvement patterns were determined for each individual.Conclusion: Despite widespread use of impact factors in evaluating the journals, the tool also has obvious shortcomings and limitations. According to the DEA technique is able to consider several performance indicators or dimensions simultaneously for calculate magazines efficiency, it can reduce take many limitations and drawbacks of using only one factor to assessment is caused, and a more reliable criteria to rank journals provide.

Abstract: 

Introduction: Inherent capabilities of the mass media in cultural changes, and their effect on behavior and public taste and its position as the dominant source for providing subjective images of cultural expression is undeniable. International media as the new challenges of globalization in various fields, has been involved many health policymakers. Therefore, this study aimed to investigate the role of transnational media in nutrition health among women.Methods: The study is a cross-sectional survey. The sample of study includes 618 women in Shiraz city which were selected through Simple random sampling. Data analysis is carried out using the SPSS software.Results: The findings show a significant difference in dietary habits of girls in comparison with their mothers. The mean score of dietary habits of girls is 11.54 while of moders is 8.86, Which is a confirmation of the girls' tendency towards more modern food habits. The mean score of daily use of the Internet and satellite among girls is 1.99 and 2.4 hours, respectively, while of mothers is 0.17 and 2.66 hours a day.Conclusion: The use of mass media, especially internet and satellite as an indicator of globalization can encourage the consumption of unhealthy food.

Abstract: 

Introduction: Hospital environments are workplaces that have high risks with negative effects. About Health Information System (HIS), there are researches that show the negative effects of information technology applications in health care on patients and workers. There are several methods for risk assessment, which the most reliable methods are FMEA - Failure Modes and Effects Analysis -. In this article we apply this technique to examine Failure Modes and Effects in the HIS system.Methods: This study is a descriptive - multi-step and applicable research. The researchers review processes by interviewing with staff of IT (Information Technology) and Nursing departments in 2013. And they identify potential fault modes and effects by brainstorming techniques. To this end, researchers use the standard worksheet (Effects Analysis & Failure Mode: FMEA) that used by several researchers in the many fields of health in other countries, and its reliability has been confirmed, and professional professors has confirmed its validity.Results: from 12 founded failure modes, 9 items were related to software section and 3 failures were related to hardware section. In test order process, the greatest risk priority number is related to entering of misdiagnose with 245 points. In drug order process, the priority numbers of additional drug order and reorder (by different people on different shifts), are equal in 36 points. The most significant failure in equipment order process is additional equipment order with 300points. In hardware section the greatest risk priority number is related to fire risk with 60points.Conclusion: According to this study, a prospective method as FMEA has high efficiency and effectiveness for identifying and prioritizing failure modes of running processes of "HIS". Risks associated with the use of the most important parts of the system were identified and provided useful suggestions. By applying the proposed measures appropriated in this study by applying this technique, we can reduce the risks of "HIS" greatly and help to improve hospital performance.

Abstract: 

Introduction: Patients perceived price fairness for the service provided an important element of marketing hospitals or health care provider organizations. To establish a fair system of pay for performance in a healthy market, a stable income and high performance centers is essential. The aim of this study was to determine the effect of perceived price fairness on satisfaction and loyalty through the services offered price acceptance the city's private hospitals.Methods: This research according to purpose is the application and the descriptive – survey based on structural equation model. The study population consisted of patients (customers) of the private medical centers in the Isfahan city that questionnaires are randomly distributed among the 345 people of these customers. Tool for data collection was based on standard questionnaire these validity and reliability were confirmed based at expert and Cronbach’s alph. Method for data collecting was randomly classified and kind of result analysis was based on critical value and fitness index. In addition, the data were analyzed using SPSS18 and Amos 20.Results: The results is supporting of the effect of perceived price fairness on customer satisfaction (b=0.48) and loyalty (b=0.34). The data analysis showed that customer satisfaction (b=0.39) and loyalty (b=0.42) are two factors to predicting for price acceptance.Conclusion: Regarding the effect of perceived price fairness on satisfaction, loyalty and acceptance price of medical services provided by the centre, it is recommended that the management of this centre have required considering and comprehensive the price health care provided and as possible by providing marketing guidelines providing feel the price acceptance for patients through Satisfaction and loyalty.

Abstract: 

Introduction: Quality of care from the perspective of patients is increasingly considered an important component of comprehensive chronic disease management and as instrument that use for evaluation of quality. This study aimed to assess of quality of delivered care among patients with rheumatoid arthritis based on a model of Comprehensive Quality Measurement in Health care (CQMH).Methods: A cross-sectional study was conducted with 170 people with rheumatoid arthritis who were received care from specialist clinics of Isfahan University of medical sciences in 2013. Validity of The study questionnaires were reviewed and confirmed by 8 specialist in rheumatology and research and there reliability was confirmed according to Cronbach's alpha index (Service Quality (SQ); a=0.721, Technical Quality (TQ); a=0.766, Costumer Quality (CQ); a=0.803). Final scores for each dimension were referred on a 0-100 scale with high values indicating better quality. Data analyzed using the SPSS-19 statistical package. Independent Samples Test, ANOVA Tests were conducted to compare CQ, SQ, TQ and QI score between categorical variables.Results: The average Quality Index was 72.70 of a 0-100 scale and average SQ, TQ, CQ score were 79.09, 68.54, and 70.25 respectively. For CQ only 19.8% of participations staying the course of action even under stress and financial constraints, there was a significant gap between what Rheumatoid Arthritis (RA) care they received and what was recommended in the guideline for TQ and the concept of service quality was poorly developed in many cases, specifically "availability of support group" that was the lowest scores.Conclusion: According to the present study quality of care the patient’s perspective was low, thus its need to pay attention to patient partnership and empowering them to manage their condition. Also there are need to promote compliance and establish association of rheumatology patients.

Abstract: 

Introduction: Portal assessment is one of major issues in developing services to users. This article is going to suggest a checklist in order to evaluate hospital portals.Methods: In this study library research method is used to study literature and evaluation checklists. In fact specialist viewpoints are used to get reliability of the assessment tool. Afterwards AHP method is used to valuation criteria and components.Results: In this study 14 criteria and 77 components were identified and proposed. The criteria and their score were gained as follow: contact information (with 5 score); features of portal pages (with 5 score); pages design (with 4 score); updating pages (with 5 score); Information, access roads (with 5 score); Public services (with 20 score); Services to Patients (with 3 score); Patient data (with 7 score); Research and education (with 3 score); Public relationship (with 7 score); Technical features of screens (with 4 score); Telemedicine (with 15 score); Pharmaceutical Services (with 3 score); Financial Services (with 15 score).Conclusion: According to finding a checklist for assessment of hospital portals is suggested.

Abstract: 

Access to the right information at the right time is a crucial ingredient of modern healthcare. Today, within health kiosks, health information can be accessed in public places. The term kiosk tends to be used for touch screen computer that is normally used standing up in public places. Health kiosk is a system with various capabilities. This is an overview article. Different scientific databases were searched to identify articles published during the period of 1995 to 2013. The information is also gathered from personal observation and oral interview in Tehran. Health kiosks can be utilized for self-management of chronic diseases, reduce health disparities, better outreach of healthcare services and can be a cost effective way to improve delivery of healthcare services. A search performed on health kiosks in Tehran shows a significant difference in a structure and use of it with global ones. One of the main reason is that theses kiosk are independent from ministry of health and medical education. Several variables such as health literacy, lack of culturally adaptive health information and limited availability of tailored health material are some of the existing barriers to greater adoption of health technologies and their impact in the improvement of health outcomes.
