Abstract: 

Background & Aims: Nowadays, diabetes is a silent epidemic of the present century, and it is one of the biggest health problems in all countries, known as the "diabetes epidemic", which includes pregnant women as well. The aim of present study was to determine the impact of the consultation on promoting healthy lifestyles in women with gestational diabetes.Materials & Methods: This Randomized Clinical Trial study was performed on 60 mothers with gestational diabetes who admitted in Motahhari hospital of Urmia.The samples were assigned randomly in to two groups. The intervention group received 5 counseling sessions. Data was collected by completing demographic questionnaire, and Health Promoting Lifestyle Profile II questionnaire.Results: This study showed that after intervention, counseling, and health promotion behavior scores were (158.04±17.04) significantly higher in the intervention group than control one (111.4±12. 15) (P<0.001).Conclusions: Midwives as the health promoting lifestyle counseling at health centers play important role in improving the lifestyles of mothers with gestational diabetes, and reduce complications associated with Gestational Diabetes Mellitus in pregnancy.

Abstract: 

Background and Aims: preconception care focuses on pregnancy health and its related consequences before and after impregnation. The present study was conducted in 2015 in order to determine attitude of midwives', suggested solutions and current problems of preconception care.Materials and Methods: This is a cross-sectional study was implemented on entire midwives (202 people) working in Tabriz health centers, Iran, using demographic, attitude questionnaires and open questions. The instrument validity was determined based on CVI and CVR methods while its reliability was determined from two aspects of repeatability (ICC) and internal consistency (Cronbach's alpha coefficient). The data were statistically analyzed using descriptive and inferential methods.Results: Mean and standard deviation of midwives’ attitude was 83.15 (7.82). Most of midwives offered motivating to improve preconception care, and mentioned lack of adequate opportunity to provide preconception care as current problems of it. According to One-Way ANOVA and independent t-test, there were no significant relationship between attitudes and demographic factors.Conclusion: Based on the results midwives had a good attitude toward preconception care. In order to improvement of preconception care, considering motivational factors and providing required conditions such as enough opportunities to do qualitative preconception care are recommended.

Abstract: 

Background & Aims: Diabetes is the most prevalent disease during pregnancy. Self-care and prevention of diabetes type II in future are the key factors in patients’ education. Cultural and religious beliefs are of the effective key factors on self-care by diabetics. Gestational diabetes is one of the main health issues in Iran. Therefore, the present study is aimed at elaborating on the beliefs, and coping strategies of the mothers with gestational diabetes Materials & Methods: The study was carried out as a qualitative-content analysis method. Face to face semi-structured deep interviews were performed for data gathering. Participants were pregnant women diagnosed with gestational diabetes in 24th-36th week of pregnancy who referred to the clinics affiliated with Shahid Beheshti Medical Science University. Data saturation was achieved with 12 interviews and the collected data was analyzed using conventional content analysis method Results: conventional content analysis highlighted two themes; Attitude toward the disease and Coping strategies. The former was featured with 2 categories: religious beliefs and medical beliefs. The latter was featured with three layers: compatibility, spirituality, and incompatibility.Conclusion: calrifyng beliefs and coping behaviors of mothers with gestational diabetes leads to better and proper education planning and programming toward improvement of health, self-care, and prevention of diabetes .

Abstract: 

Background & Aims: Obesity is now reaching epidemic proportions in both developed and developing countries and is affecting not only adults but also children and adolescents. Increasing prevalence of adolescent obesity has become a growing matter of public health concern worldwide. The aim of current study was to determine the prevalence of general and abdominal obesity, overweight among high school girls in Urmia.Materials & Methods: This analytical cross-sectional study was performed on 800 girl's high school students in Urmia. Required data was collected through interviews and anthropometric measurements.BMI and WHR were measured using standard methods. According to the WHO standards, WHR equal to or greater than 0.8 were considered as abdominal obesity. Collected data were analyzed by SPSS software version 21Results: The prevalence of underweight, overweight and obesity among all girls' students were %2.9, 20.5% and %10.9 respectively. These values were among girls were reported %3.1, %21.5 and%13.8 in first period and %2.8, %20.0 and %9.5 in second period respectively. The prevalence of abdominal obesity was also %42.75. There was not a significant relationship between BMI percentile and the first and second period high school students.Conclusion: According to these findings, the prevalence of overweight, general and abdominal obesity in students, especially in older age groups, are on warning aspect. Therefore, intervention programs to prevent risky behavior of overweight and obesity in students is essential.

Abstract: 

Background & Aims: Hypertension is a major public health issue. Lack of medication adherence in patients with hypertension can lead to adverse outcomes. Therefore, patients need to be educated on how to control their blood pressure and increase their behavioral preventive skills. Among the numerous models of health education, BASNEF model is the most comprehensive model used to study, identify and create new behaviors. This study aimed to determine the effect of an educational intervention based on BASNEF model on the medication adherence in patients with hypertension.Materials & Methods: This quasi-experimental study with pretest-posttest design conducted on 80 patients with hypertension referred to Health Centers in Urmia, Iran, and met the eligibility criteria. They were selected using convenience sampling method and then, randomly assigned to two groups as control and experimental. A 4-part questionnaire was used for collecting the data. The collected data was analyzed by SPSS v.16 software using descriptive and inferential statistics (independent t-test, Mann-Whitney U test and Chi-square test).Results: The results showed that the mean score of medication adherence in patients with hypertension in the experimental group was significantly more than the control group (P<0.001).Conclusion: It seems that educational intervention based on BASNEF model has considerable advantages and increases the medication adherence in patients with hypertension significantly. Therefore, it is recommended to apply this educational model as a key complementary factor to increases compliance of patients with hypertension.

Abstract: 

Background and Aims: Cardiovascular diseases such as coronary artery disease are the most important causes of death in the world and Iran as well. For this reason, These kinds of diseases cause anxiety on the patietns. Discharge planning is crucial to ensure timely discharge, and continuity of care. It also helps healthcare providers to use limited resources most effectively, and to avoid unnecessary readmissions. The aim of this study is to determie the effect of IDEAL (Include, Discuss, Educate, Assess, Listen) discharge plan model on anxiety levels in patients with myocardial infarction in Shahid Madai hospital in Khoy, Iran, 2016.Materials and Methods: In a semi experimental study, 108 patients with myocardial infarction were assessed in Shahid Madani hospital affiliated with Urmia University of Medical Sciences in 2016. They were selected through convenience sampling, and divided randomly into two groups of intervention and control. Every patient in the intervention group received IDEAL discharge plan model from their admission to 3 months after their discharge, while patients in the control group received no intervention. Spielberg’s state-trait inventory was used to assess their level of anxiety before intervention, and 3 months after their discharge.Results: According to findings, the mean score of anxiety for experimental group before the intervention and after intervention were 72.71±3.09 and 32.25±4.94 respectively. There was a significant difference between the mean scores of anxiety reduction in two groups after the intervention (p<0.001).Conclusion: As a result, providing the patients with IDEAL discharge plan mode will diminish their anxiety level of myocardial infarction. Therefore, it is recommended that the nurse managers provide programs to highlight the instructional role of nurses.

Abstract: 

Background & Aims: Emotional problems are common and annoying for patients affected by epilepsy. Despite the increasing attention of medical science, the neurological aspects of epilepsy and its psychological effects are remained almost unknown.This study aimed to evaluate the impact of cognitive-behavioral stress management on reducing the frequency of seizures in dealing with the type of epilepsy in adolescents with epilepsy.Materials & Methods: In this pilot study, 40 patients with epilepsy (20 patients with general epilepsy, and 20 patients with focal epilepsy) were selected by convenience sampling from Tabriz Epilepsy Society, and then they were randomly divided into two groups: Control and Experimental.The rage of their age was 14 to 21 years old. To obtain the data, the frequency of seizures of the patients was collected two months before the intervention. (The frequency of attacks was about 3 to 6 times). Before the start of the first session, Lvynda questionnaire was delivered to measure stress, depression and anxiety of the patients. The experimental group (10 patients with General epilepsy, 10 patients with Focal epilepsy) was intervened by cognitive-behavioral stress management intervention for two months. Data analysis was performed using the spss 21software.Results: The results showed that Cognitive-behavioral stress management intervention in patients with epilepsy resulted in the reducing of the frequency of seizures. There was no significant difference between general and focal epileptic patients. Based on the results of the study, there was a significant relationship between emotional distress and the frequency of seizures.Conclusion: Psychological interventions to reduce the frequency of the seizures can be an effective factor. So in addition to medicine therapy, this method can be used to improve the condition of in the epileptic patients.

Abstract: 

Background & Aims: Uncertainty in illness, as a psychological stressor in adolescents with cancer, is a challengeable factor in the care of the children. In order to measure the effectiveness of interventions on reducing uncertainty, there is a vital need for a valid and reliable instrument. This study was conducted to translate and localize the Persian version of the “Uncertainty Scale for Kids”, and to examine its psychometric properties in Iranian adolescents with cancer.Materials & Methods: The Persian version of the “Uncertainty Scale for Kids” (USK) was validated based on the model proposed by Wild et al. (2005). The mentioned scale was translated into Persian and back-translated into English, and revised by the scale designer. Two-hundred adolescents with cancer were enrolled who attended clinics and oncology wards in Children's Medical Center of Tehran, clinics and oncology wards in hospitals of Urmia (North West of Iran), Children's Medical Center of Tabriz (North West of Iran), and the Cancer Research Center of Isfahan (Center of Iran).Then, content and face validity assessments, construct validity and internal consistency, and reliability of test-retest were conducted for the Persian version. Data were analyzed by SPSS version 20, and AMOS version 21 as well.Results: Confirmatory factor analysis of the 22-items “Uncertainty Scale for Kids” demonstrated less than adequate fit with three subscales, and ultimately the scale fit was improved by removing one item (No. 18 “I don’t know why (of all people) I got cancer"), and by displacing of another item (No. 17 “I don’t know if I’ll be better or not”). Internal consistency reliability was obtained for the “Uncertainty Scale for Kids” (α=0.76) and for each of the subscales including ignorance, and lack of knowledge (uncertainty) (α=0.72), the inability to predict (α=0.75), and uncertainty about the meaning of events (α=0.73). In addition, the results of the calculating Pearson's correlation coefficient was calculated between two tests r=0.77 (p<0.001).Conclusions: The Persian version of the "Uncertainty Scale for Kids”, with 21 items and 3 subscales has good psychometric indicators. It can be used to evaluate uncertainty in Iranian adolescents with cancer.
