Abstract: 

Back ground: Laryngeal cancer though accounts for only 1.5% of all malignant neoplasm. Scientific evidence has shown that cancers of the oral cavity, larynx, lung and esophagus are associated with Cigar smoking. The present study is to examine by case-control method the impact of smoking on laryngeal cancer.Methods: One hundred and six hospitalized cases of otolaryngology department of Imam and Apadana Hospital of Ahvaz Jundishapur university diagnosed and pathologically confirmed laryngeal cancer in the period between March 1990 and March 2007 were selected for study. At the same time 176 controls of the otolaryngology department hospitalized in the same period for non-tumor diseases of the head and neck were selected. The questionnaire used in the present study contained items such as the social and demographic background of the subject, history of otolaryngology diseases, and lifestyle of the patients before hospitalization, smoking habit, and amount and duration of smoking. All data was analyzed by SPSS1.Results: cases and controls were not significantly different in sex, marital status, race educational level, religious belief and area of residency (P>0.05). Age, smoking experience,total year of smoking, way of smoking and the total amount of smoking were statisticallyassociated with laryngeal cancer (P<0.05). A total of 148 patients from 176 patients were using tobacco compounds that about 50 cases above 20 cigarettes per day and the rest less than this number were consumed. The average duration of smoking 41.8 years. An interesting fact that of 25 female patients, 10 cases were used tobacco. Type of cigarette smoked in the 24 cases was handmade, 67 cases of cigarettes produced domestically and 57 foreign type was used. In the control group from 176 cases reviewed 18 smokers has been reported with average time of 4.8 years.Conclusion: Smoking is by far the strongest risk factor associated with the development of laryngeal cancer. Since it is fairly uncommon for a non-smoker to be diagnosed with laryngeal cancer, smoking cessation is the best way to prevent laryngeal cancer.

Abstract: 

Background: One of domestic violences is wives injury which is a health and psychological chalange. The aim of this study was to determine the prevalence of domestic violence and related factors in pregnancy.Methods: This descriptive – analytic study evaluated 300 women who attended six health centers in Ahvaz in the last month of pregnancy or had labored during past 6 months. Abuse Assessment Screen (AAS) standard questionnaire was used for data collection. Data were analyzed by spss version 15; compeuterized progrom chi square test and regression of logistic were used for data analysis.Results: The mean age of participants were 26.4±5.6 years. The prevalence of any kind of violence before pregnancy was 44.7%. While the rate decreased to 19.3% duting pregnancy. Prevalence of physical, psychological and sexual miolene impregnavcy were 2.7%, 14%, and 9.3% respectively.Conclusion: The results showed that the prevalence of domestic violence during pregnancy decreased comparing to before pregnancy. Despite the reduction of the prevalence of domestic violence in pregnancy, we recommend routine screening of violence in pregnant women in health centers.

Abstract: 

Background: The collection of information concerning neoplasm is of major interest, but medical data are only meaningful once they are classified according to a valid and reliable classification. So the purpose of this study was to survey current neoplasm classification systems.Methods: This research study is descriptive and useable. Information’s collected by library studies and by question arises.Results: Research result show that most of medical record yards in study group use of ICD10 book to neoplasm classification also 44precent of statistical society use of commandments and rules of dead cause’s classification in second part of ICD10 book. 100 percent of statistical society didn’t have any training about ICD-O.Conclusion: according to research result we can conclude that clinical data extracted from correct sources. But because staff of medical recorders lack of knowledge and they don’t use of ICD-O, tumor registry statistics is so different from WHO standards.

Abstract: 

Background: Exclusive breastfeeding as the most important part of children's lifeline has the most efficacy in health promotion and prevention of diseases in children. Several lines of evidence emphasis on positive effects of breastfeeding on physical and mental child growth. The aim of this study was determinate prevalence of exclusive breastfeeding and related factors in children attending health centers of OmidiehMethods: In a descriptive–analytic study 400 children 6-18 months who seek for receiving routine child health care, during the period of sampling were studied. Interview with mother, filling a questionnaire by trained questioner were used for data collection. Chi square, T test and Regression Logistic were done.Results: The prevalence of exclusive breastfeeding was 61.6%. A significant association was found between exclusive breast feeding and birth weight, start breast feeding immediately after birth, encourage of family to artificial feeding, type of delivery and number of children (p<0.05).Conclusion: In spite of relative high prevalence of exclusive breastfeeding in children attending health center of Omidieh, role of education of mother about benefits of exclusive breast feeding must be in priority in health centers. Also education about immediately start of breast feeding after birth, avoid feeding of children with other things and choose of vaginal delivery is very important.

Abstract: 

Background: Clinical decision support systems (CDSS or CDS) are interactive computer programs, which are designed to assist physicians and other health professionals with decision making tasks. The main purpose of modern CDSS is to assist clinicians at the point of care. This means that a clinician would interact with a CDSS to help determine diagnosis, analysis, etc. of patient data.Methods: In this study to systematically review controlled clinical trials assessing the effects of clinical decision support systems (CDSSs), through the Health System Researches (H.S.R) method, We searched Medline, EMBASE, EBESCO, Ovid, MEDLINE, SID and IRANMEDEX databases and searched JAMA and BMJ Journals too for publications from 1980-2010 by two English and Persian language.Results: Forty five studies met our inclusion criteria. From 28 studies that addressed the role of CDSSs in Improving the quality of health care, 22 (78%) study were useful, and from 13 studies that addressed the reducing of medical errors, 11 (85%) found improvement with the use of a CDSS. finally, in reducing Inappropriate diagnostic and therapeutic practice all of the CDSSs in 8 studies were completely useful.Conclusion: The use of CDSSs in clinical practices can improve the quality of health care, reduce the amount of Inappropriate diagnostic and therapeutic practice and decrease medical errors.

Abstract: 

Background: The board of directors of health institution had left the responsibility of improvement of observation information to the medicine cadre. Therefore, the medical record committee is formed. This study is done to survey the function of medical record committee of Educational Hospitals in Ahvaz based on committee’s records. This subject is studied due to positive impact on the decisions of medical centers based on the removal of the medical record committees’ functions deficiencies.Methods: This applied study is descriptive-segmental, that is accomplished with method of statistic of the whole records of the convened meetings of the medical record committee of Educational Hospitals in Ahvaz in 2009 and the first half of the 2010. The tool of collecting data is a checklist that is codified by the researchers based on internet and biblical references. The number of records was 50. Data analysed by Microsoft office excel 2007.Results: the most important issue that was discussed was the Improvement of removal of Medical records’ deficiencies (49.66%).Conclusion: Regarding to the lack of convening the meetings regularly, and imperfect presence of members, function of medical record committee of selected Hospitals isn’t appropriate. Hence, convening the meetings regularly and presence of the major members of the committee is considerable.

Abstract: 

Background: Diaphragm of eventration is a defect of whole or a part of diaphragm muscle and replacing it with fibroplastic tissue. The incidence of eventration is uncertain. The article aims to report a newborn with congenital diaphragmatic eventration.Case report: The case is a dysmorphic male premature newborn which born at 33 gestational age. Due to sever respiratory distress the patient admitted immediately in NICU and underwent associated ventilation. Despite thoracotomy and also exploratory surgery, he died after thirty days.Conclusion: We as health care givers have responsibility to inform people about prevention of genetic abnormalities and importance of genetic consoling before decision for having a child.

Abstract: 

Background: Caroli disease is a rare inherited disorder characterized by dilatation of the intrahepatic bile ducts.Case report: The presented case is a female middle age patient that during clinical and para-clinical evaluation for an unrelated compliant found that has Caroli disease. Although, the disease is hereditary but the family history was negative. After three years follow up, no involvement of kidney or serious damage of live is reported.Conclusion: Prognosis is fairly good unless recurrent cholangitis and renal failure develops. Since the disease has slow and silent progress while is rare and fatal, it should be considered as differential diagnosis in any unknown cholangitis case.
