Abstract: 

Background: The efficiency and effectiveness of human resources depends on many factors that job satisfaction is one of the most important of them. This study aimed to assess the job satisfaction among the staff of Alzahra health center in Isfahan, Iran, and the effective occupational and non-occupational factors. Methods: In this cross-sectional study conducted in Alzahra hospital during the years 2017-2018, 603 personnel of this hospital were enrolled the study. Demographic checklist including general information such as age and gender, as well as occupational checklist including work experience and type of shiftwork, were completed for the participants. Then, Minnesota Satisfaction Questionnaire (MSQ) was completed for the health care workers. Finally, the results of the questionnaires were evaluated based on the demographic and occupational variables using SPSS software. Findings: The nurse aides had the highest, and nurses had the lowest job satisfaction. There was a significant relationship between job satisfaction and shift-work in all subscales of job satisfaction. It was also observed that all subscales of job satisfaction, except job type and organizational situation, had significant relationship with type of the employment relationship. Conclusion: Based on the results of this study, nurses had the lowest job satisfaction; regarding the subscales of job satisfaction, managers should plan for increasing job satisfaction.

Abstract: 

Background: Magnetic resonance imaging (MRI) is a valuable diagnostic tool by its noninvasive/nonionizing nature. The aim of the current study was to determine the justification of MRI in outpatients referred to Hajar university hospital, In Shahrekord City, Iran, in a one-year period of 2017. Methods: A retrospective descriptive cross-sectional study was conducted on 438 outpatients referred for MRI during the year 2017, who were selected using systematic random sampling method. Intended data including age, gender, investigated organ, the specialty of requesting physician, MRI with or without contrast, and MRI diagnostic outcome were extracted from patient charts, and collected using checklists. Descriptive statistics and chi-square test were used for data analysis. Findings: Requested MRI led to a diagnosis in 81. 3% of the patients. Women patients represented 64. 4% of the requested MRI cases. 45. 2% of the exams were performed for the patient with the age range of 41 to 60 years. The most prevalent investigated organ was the lumbosacral (43. 2%). Age classes significantly influenced investigated organ (P = 0. 001), as well as the prevalence of MRI diagnostic outcome (P < 0. 001). The percentage of MRI diagnostic outcome differed significantly among clinical physicians (P <0. 001). The prevalence of MRI diagnostic outcome significantly differed in respect to the investigated organ (P < 0. 001), with lumbosacral (50%) as the most prevalent. Conclusion: Requested MRI in tertiary referral center of Shahrekord city led to a diagnosis in an acceptable manner. However, there is need to study appropriate imaging in view of taking most advantages from medical practice guidelines.

Abstract: 

Background: Suitable vascular access is an important prerequisite for uncomplicated hemodialysis. The purpose of this study was to comparison the efficacy and complications of end-side anastomosis of brachial artery to cubital perforating vein versus side-side brachiocephalic arteriovenous fistula formation in hemodialysis candidates. Methods: In a clinical trial study, 85 patients undergoing arterial venous fistula placement were randomly divided in two groups of 42 and 43 patients. The first group was fed into the cephalic vein fistula to the adjacent brachial artery by side to side method, and in the second group, the cephalic vein fistula was transferred to the brachial artery by end to side method. The efficiency and duration of catheter operation were compared between the two groups. Findings: The mean duration of function of the fistula was 36. 66 ± 2. 34 weeks. The mean duration of fistula function in the two groups of side to side and end to side was 21. 68 ± 2. 03 and 38. 4 ± 3. 8 weeks, respectively, and there was no significant difference between the two groups according to the log-rank test (P = 0. 41). Conclusion: The type of anastomosis in patients undergoing arteriovenous fistula does not have an effect on the efficacy and survival of fistula. However, since the lateral anastomosis has a greater flux of blood flow and less pressure in the vascular wall, this method seems to be preferred to anastomosis to the side shaft. At the same time, more studies are recommended.
