Abstract: 

Introduction: One of the expected consequences of society aging is an increase in the number of frail elderly. Frailty if persistent, It probably has consequences for the individual and social life of the elderly in society. Therefore, the present study aimed to determine the prevalence and frailty-related factors were assessed in the elderly hospitalized. Methods: The present study is a cross-sectional descriptive-analytical study on 120 elderly patients hospitalized in selected hospitals of Iran University of Medical Sciences that were included in the study by convenience sampling method. Data via demographic characteristics Form and Tilburg Frailty Indicator (TFI) Questionnaire it was gathered. Data analysis by independent t-test, analysis of variance and Pearson correlation coefficient, using SPSS software version 22 was performed. Results: 64/2% of the elderly were men. The mean age of the elderly was 68. 3 ±,8. 11. 62. 5% of research samples were frail. The social domain was identified as the most common and pervasive domain of frailty. Psychological and physical domains of frailty were also ranked next in terms of prevalence, respectively. frailty also with different demographic characteristics such as age (P= 0/026), gender (P= 0/021), marital status (P= 0/035), lifestyle (P= 0/014), etc. had a statistically significant relationship. Conclusions: Based on the findings, total frailty as well as separately in all three physical, psychological and social domains is relatively common among the elderly hospitalized and is affected by various factors.

Abstract: 

Introduction: Aging population and diseases in old age will increase the need for outpatient and inpatient services and consequently increase the costs of the health system. Studies on health needs and quality of life in the elderly in different countries of the world However, in the field of integrated care for the elderly, with a qualitative approach and considering the experiences of the health care care team, there is a need. Since the care programs in the country do not meet the challenge of providing services the elderly, The researcher decided to conduct a study with the aim of understanding integrated care for the elderly in health care centers with a qualitative method. Methods: This study was conducted using a qualitative design and content analysis approach to explore integrated care experiences for the elderly in nursing health centers. The participants in the present study include 35 people. It consists of 15 physicians, of which 2 are health workers and 13 are family physicians. There are 10 male and 5 female physicians. The age range of physicians in this study was between 30 and 53 years and the average was 42. Fifteen elderly caregivers were health professionals, one was a health services manager, and one was a senior geriatrician. The inclusion criteria in this study were health care providers with the age range of participants between 33 to 51 years and the average was 42 years. The method of data collection was semi-structured interview and the method of content analysis was analysis. Results: From the findings of this study, 241 primary codes were extracted and 141 codes emerged by matching similar codes. The codes were analyzed and categorized based on features, which resulted in the emergence of 86 codes and were placed in 14 subcategories. The extracted codes and statements of the participants were divided into 5 main categories. Concerns expressed by the elderly and caregivers, including physicians and health care providers, include: the complexities and concerns of the elderly, the lack of health services in the centers, the lack of health services in the centers, cultural services and the culture of healthy aging in the centers, support The welfare economy was for the elderly to benefit from the services. Conclusions: In general, it can be concluded that according to the results of this study on the process of integrated care for the elderly, the main success of this care and the provision of services by caregivers in health care centers based on structural, executive, resource reform Human and financial are the center and families of the elderly. In the past, the focus was on the elderly only in some health services in health care centers and less attention was paid to services, welfare and home care services in old age. In Iran, according to the results of this study, providing more developed services but still challenges There is a lot in terms of specialized organizational structure and resources and how to provide services.

Abstract: 

Introduction: The rate of patients’,admission to the emergency department is not planned exactly. Due to the bad condition of the patients, the triage management is also very momentous. Therefore, up-to-date training has a particular importance for nurses. Methods: 37 emergency nurses were selected by census method through quasi-experimental study. Data collection tools were four questionnaires of demographic characteristics, triage attitude assessment, triage accuracy and triage knowledge which determined the total staff performance. They were completed by them once before the training, one month after training and three ninety-minute training sessions. Data were analyzed by Wilcoxon, Man Whitney U, t-test and SPSS version 20. Results: 59. 5% of them were females, 51. 4% were married, 94. 6% did not have a bachelor's degree, 64. 9% had not rotating shifts and 62. 2% had not the experience of participating in training programs about accidents and disasters. The mean score of nurses’,knowledge increased from97±,3. 59before the intervention to 17. 46 ±,3. 22 after the intervention, also, the mean score of nurses’,accuracy ranged from 15. 16 ±,5. 86 to 20. 89 ±,4. 25,finally, the mean of attitude score ranged from 125. 8 ±,15. 9 to135. 4±,14 (P <0. 0001). The most impact of interactive learning was related to triage attitude with the mean score of 9. 62. There was also a significant association of the knowledge improvement indicator with the last degree (P = 0. 04) and the association of the knowledge improvement indicator with gender (P = 0. 05). Conclusions: Emergency nurses had better knowledge, accuracy and attitude than before interactive training programs. Significant increase in knowledge and attitude scores could indicate an increase in nurses' readiness and vigilance to perform better triage at emergency departments. The impact of interactive training on nurses’,attitudes on disasters triage can enhance emotional performance and create deeper cognitive learning which has everlasting effects on nurses’,performance,the emergency staff can have better performance through triage of accidents and disasters.

Abstract: 

Introduction: Diabetes is a chronic, common, and progressive disease that poses a major challenge to community health management. People with diabetes usually have high levels of depression and low level of hope. Therefore, present research aimed to comparison the effectiveness of mindfulness and psychological well-being education on depression and hope in patients with type 2 diabetes. Methods: Present study was semi-experimental with pretest and posttest design with control group. The research population was patients with type 2 diabetes living in areas one to three of Tehran city and member of the Tehran Diabetes Association. After reviewing the inclusion criteria, 75 people were selected by purposive sampling method and randomly replaced in three groups (each group 25 people). Each of the experimental groups separately educated with methods of mindfulness and psychological well-being education for 10 sessions of 90 minutes (per week two sessions) and the control group was placed on the waiting list for education. The data were collected by questionnaires of Beck and Clarck depression and Snyder and et al hope and analyzed by methods of chi-square, one way ANOVA analysis, multivariate analysis of covariance and Bonferroni post hoc test in SPSS software version 19. Results: The results showed that the groups did not significantly different in terms of gender, marriage, education, age and duration of illness (P>0. 05). Also, mindfulness and psychological well-being education in comparison with the control group led to significantly decreased depression and significantly increased hope in patients with type 2 diabetes (P<0. 001), but between the two methods of mindfulness and psychological well-being education in their decreasing depression and increasing hope there was no significant difference (P>0. 05). Conclusions: The results of this study indicated the effectiveness of both methods on reducing depression and increasing hope and no difference between the two methods. Therefore, therapists and health experts can be used from mindfulness and psychological well-being education methods to improve the health-related characteristics of patients with type 2 diabetes, especially to decrease depression and increase hope.

Abstract: 

Introduction: Family is a natural social system that men and woman are its major subsystems in all of stages of its development and many factors such as communication patterns and resiliency play a role in their marital satisfaction. The purpose of this study was the modeling structural role mediator resiliency in relationships communication patterns with marital satisfaction. Methods: This is a descriptive-correlation research. The statistical population consisted of all Married men and women are referred to counseling and psychological services related to counseling system Tehran city in year 2019. The sample size was 480 subjects based on formula Cramer was selected by convenience sampling method and after deleting the missing values and outlier detection data, 302 people remained in the analysis. The tools were used included Enrich marital satisfaction questionnaire of Olson, Fournier & Druckman, communication patterns questionnaire of Christensen & Sullaway and resilience scale of Connor and Davidson. Results: The results showed that was significant correlation communication patterns and resiliency with marital satisfaction (P<0/01). Also results showed, Also this model showed indirect path communication patterns with marital satisfaction through moderating role resiliency. The proposed model had an acceptable fitness to the data (RMSEA=0/061). Conclusions: Some factors, including communication patterns and resiliency were found to affect couples' marital satisfaction. It is recommended that therapists pay more attention to these factors in providing solutions to promote marital satisfaction.

Abstract: 

Introduction: Nurses work long hours, and the growing demand for health services can affect their mental health and Quality of professional life. The aim of this study was to investigate the mediating role of self-transcendence in the relationship between perceived stress and Professional quality of life of nurses. Methods: This study was a descriptive correlational study. The statistical population included all nurses working in public and private hospitals in Birjand in 1399 (N = 795). The research sample included 440 nurses. Participants were selected by convenience sampling method. They answered Perceived Stress Scale, the subscale Self-transcendence the Temperament and Character Questionnaire and the Professional Quality of Life Questionnaire. The data were analyzed using structural equation modeling and SPSS22 and LISREL8. 8 Software. Results: The results showed that the direct path of perceived stress to self-transcendence was negative and significant and direct path of perceived stress to to professional quality of life was positive and significant. The direct path of self-transcendence (p<0. 01) to professional quality of life (p<0. 01) is negative and significant. In addition, indicated that the indirect path perceived stress to professional quality of life through self-transcendence was significant (p<0. 01). Conclusions: The findings of this study support the mediating role self-transcendence in the relationship between perceived stress and professional quality of life. These findings indicate that self-transcendence can play a significant indirect role in reducing the negative effects of perceived stress on the professional quality of life of nurses.

Abstract: 

Introduction: Hospitalization in intensive care units results tactile deprivation, which may cause level of consciousness and physiologic imbalance. Foot tactile stimulation can influence the level of consciousness in comatose patients. The aim of this study was to examine the effects of foot tactile stimulation on level of consciousness and physiological parameters in comatose patients admitted to the intensive care unit. Methods: This randomized clinical trial was conducted on 60 patients with coma who were hospitalized in the intensive care units of Shohadaye Tajrish Hospital in Tehran in 2019. The subjects were assigned in two groups of experimental and control, by using randomize blocked allocation. In experimental group, feet, ankles and soles of patients were touched for 10 minutes twice a day for three days (morning and evening). The level of consciousness was recorded using Glasgow Coma Scale. The level of consciousness and physiological parameters (blood pressure, respiratory rate, and pulse rate) were recorded from the 1st to 3th day of admission (before, immediately, 30 min after intervention). Data were analyzed using descriptive and inferential statistics and analyzed using SPSS-v 20. Results: According to the results, foot tactile stimulation significantly decreased physiological parameters in the experimental group each therapeutic session (P < 0. 05). Moreover, Multivariate analysis revealed a significant difference between experimental and control groups in level of consciousness at all-time points (P<0. 05). Conclusions: The foot tactile stimulation could increase the level of consciousness and stability of physiological parameters in comatose patients. it should be recommended for increasing level of consciousness in comatose patients., if it is administered frequently for long term.

Abstract: 

Introduction: Paying attention to the rights of psychiatric patients, protecting and observing their rights is one of the challenging issues of the health system. One of the most important components of providing humanistic and ethical care is awareness of the ethical concepts and rights of the patient. Methods: This descriptive-analytical study was performed in 2013 on 100 patients and 43 nurses in Ahvaz. Data collection tools were a researcher-made three-part questionnaire including demographic information, measuring patient's awareness and patients' rights, which were used after validity and reliability. SPSS16 software and descriptive statistics of mean, standard deviation, variance and correlation coefficient were used to analyze the data. Results: A total of 143 people participated in the study that the level of knowledge of nurses and patients about the rights of the mentally ill were 79. 1% and 36%, respectively, and the level of observance of these rights in nurses and patients were 51. 2% and 30%, respectively. The level was good. There was no significant difference between the level of knowledge and the level of observance of psychiatric patients' rights in the two groups, respectively (P = Value: 0. 996 and P = Value: 0. 411). Conclusions: The level of knowledge and the level of observance of mental patients 'rights in nurses was at a good and average level, but this level was very low in patients and the level of patients' rights was at a good level. Given that it is the right of every patient to be aware of their rights, health care providers should inform patients of their rights from the beginning of their admission and provide the necessary training in this regard to the best of their understanding.

Abstract: 

Introduction: Concept mapping is used as an active teaching method in order to better learning and improve learners' clinical reasoning and decision-making in complex learning environments such as surgery. One of the groups whose knowledge and skills play an important role in the performance of the surgical team are surgical technologists who are present in both scrub and circulating roles in the surgical team. The aim of this study was to compare the effect of concept mapping and lecture teaching method on the surgical technologist students' level of learning, retention and satisfaction. Methods: This was a quasi-experimental study using pre-test and post-test methods, which was performed on 60 surgical technologist students. Study samples were selected by census method with inclusion characteristics. Then they were divided into two groups of intervention and control and were trained with two methods of concept mapping and lecture. Before and after training, their learning rate was assessed and compared using a multiple choice question written test. Student retention was assessed 4 weeks after the last session. Also satisfaction with the two teaching method was assessed in two groups at the end of the last session through a research made questionnaire. Data were analyzed using SPSS statistical software. Results: The results of the study showed that students' learning, retention and satisfaction after teaching by concept mapping method was significantly higher than the lecture method. (0. 001> P). Conclusions: Based on the results of this study, teaching by concept mapping as an active teaching method increases the learning, retention and satisfaction of surgical technologist students. Therefore, using this teaching method is recommended to training of medical education learners in complex education environments such as surgical technologist.

Abstract: 

مقدمه مالتیپل اسکلروزیس ازجمله بیماری های مزمن سیستم عصبی مرکزی است که به رفتارهای خود مراقبتی ویژه در طول زندگی فرد نیازمند است. از طرف دیگر خود پنداره آگاهی و نگرش کلی فرد نسبت به خود می باشد که نقاط ضعف و قوت او را در برمی گیرد. روش مطالعه حاضر بصورت کار آزمایی بالینی تصادفی سازی شده از شهریور 1396 لغایت اسفند 1396 بر روی 86 بیمار عضو انجمن ام اس ایران به روش پیش آزمون-پس آزمون با دو گروه مداخله (44نفر) و کنترل (42نفر) انجام شد. نمونه پژوهش کلیه بیماران ام اس عضو انجمن ام اس ایران بودند که از میان لیست بیماران به صورت تصادفی ساده از طریق جدول اعداد تصادفی انتخاب و به دو گروه کنترل و مداخله تقسیم شدند. محدوده سنی 20 تا 65 سال با تشخیص قطعی ام اس که در مرحله حاد بیماری نبودند، فاقد بیماری ناتوان کننده دیگر، عدم عضویت در کادر پزشکی و پیراپزشکی و سواد خواندن و نوشتن داشتن و رضایت برای شرکت در مطالعه به عنوان معیارهای ورود در نظر گرفته شد همچنین ابتلا به بیماری های ناتوان کننده دیگر یا فوت در طی زمان مطالعه یا عدم تمایل برای ادامه مطالعه در هر مرحله از تحقیق بعنوان معیارهای خروج از مطالعه تعیین گردید. در این مطالعه پرسشنامه های اطلاعات دموگرافیک، پرسشنامه ی خودپنداره راجرز توسط هر دو گروه تکمیل و برنامه آموزش خود مراقبتی در گروه مداخله اجرا شد. پس از آن پرسشنامه ها مجددا توسط هر دوگروه تکمیل شد. در پایان تجزیه و تحلیل داده ها با استفاده از آزمون های آماری کای دو، تی زوجی و آزمون تی مستقل انجام شد. یافته ها قبل از مداخله میانگین و انحراف معیار نمره خودپنداره راجرز در گروه مداخله به ترتیب 34/11 و 39/3 و در گروه کنترل به ترتیب 09/11 و 25/3 بود. بعد از مداخله میانگین و انحراف معیار گروه مداخله به ترتیب 26/8 و 06/3 و در گروه کنترل 82/10 و 76/2 بود. این تغییر از لحاظ آماری معنادار بود (001/0 =p). نتیجه گیری با توجه به یافته های حاصل از این مطالعه اجرای برنامه آموزشی خود مراقبتی با افزایش سطح خودپنداره بیماران ارتباط مستقیمی دارد. از این رو اجرای برنامه آموزشی خود مراقبتی برای بیماران ام اس در امتداد درمان های رایج، می تواند با بالا بردن سطح خود پنداره بیماران عوارض جسمی، روحی و روانی ناشی از این بیماری را به مقدار زیادی کاهش داد.
