Abstract: 

Introduction: Sexual dysfunction is a primary concern for individuals and causes a significant reduction in quality of life. One of these disorders is orgasmic disorder. Therefore, the present study was conducted to investigate the effect of distance education on patients with orgasmic disorders. Material and methods: The present study is a quasi-experimental study and the available sampling method was performed on 80 married women with orgasmic disorder in Tonekabon-Iran from 2019 to 2020. Patients were randomly divided into two groups, the members of one group were given distance education, but the members of the other group were given face-to-face training. Inclusion criteria included: having a sexual partner and consent to participate in the study and orgasmic disorders (score below 80 according to the Halbert questionnaire). Exclusion criteria included: pregnancy, birth control pills, epilepsy, hypothyroidism, hypertension above 140 over 90 over 90, diabetes, and depression. A two-part questionnaire was designed based on demographic information and Halbert's questionnaire and was given to individuals at the beginning of the study and 6 weeks after distance learning, which is attached to the questionnaire. Data obtained from the questionnaires were analyzed using chi-square and t-test statistical tests such as the chi-square test. The significance level (p-value) for the present study was considered 0. 05. Results: The highest frequency of women in the study belonged to the age group of 18 to 30 years with 52. 5%, followed by women aged 31 to 44 years at 38. 8%, and finally women over 44 years at 8. 8%. Sixty percent of the women in the study were less than 10 years old and 40% were more than 10 years old. The mean orgasm of the women in the study in the close education group was 72. 61 with a standard deviation of 28. 3 and the mean score in the distance education group was 93. 41 with a standard deviation of 25. 64. According to paired t-test, there was a significant difference between the orgasmic scores between the two groups and in the distance education group the female orgasmic score was significantly increased (p<0. 001, t=-4. 196). The results showed in the distance education group that there was no significant difference in terms of low and normal orgasmic dispersion between women with less than 10 years of marriage, both before and after training. Also, the duration of marriage had no effect on the orgasm score of the training group. Remotely before (p=0. 774, t=1. 642) and after training (p=0. 874, t=1. 421). As well as, in the group of close education, there was a significant difference in terms of the distribution of low and normal orgasms among women with a history of the marriage of fewer than ten years and more than equal to ten years, both before and after training. This result demonstrated that the duration of marriage had an effect on the orgasmic score of this group before (p=0. 168, t=1. 102) and did not have it after training (p=0. 749, t=0. 902). Conclusion: The results of this study show that distance education can be effective in promoting sexual health and improving women's orgasmic scores.

Abstract: 

Introduction: The most common cause of death in pregnant women in the first trimester of pregnancy is ectopic pregnancy. Considering the importance of the issue, this study was conducted with the aim of investigating the five-year statistics of patients with ectopic pregnancy treated with methotrexate in the maternity ward and operating room of the Sarem Women's Hospital (Tehran, Iran). Material and methods: This is a retrospective descriptive study. The method was to collect and review the necessary information from archived patients' medical records referred to Sarem Women's Hospital over a five-year period from November 2011 to November 2015, when their ectopic pregnancy was diagnosed and used for methotrexate treatment. Data were analyzed by SPSS software, version 19. Results: In this research, the total number of patients was 126 and their average age was 32 years. In the history of drug use, most used drugs related to infertility treatment (7. 2%), and the most common history of surgery (37. 2%) among referring women was related to surgery for ovarian cysts, cesarean section, and appendectomy. 86. 5% of cases had no history of infertility. Out of all the patients, only 21 people (16. 7%) were successfully treated with methotrexate and in other cases, it led to surgery. The most common surgery after methotrexate was laparoscopic salpingostomy (54%). Conclusion: According to the results of this study, different risk factors have an impact on ectopic pregnancy. The history of infertility and history of abdominal surgery can be one of the most effective factors in ectopic pregnancy.

Abstract: 

Introduction: Infertility is a major public health concern that affects 10-15% of reproductive age couples in the world. Male factors are mainly due to sperm morphological and functional disorders, including premature puberty, hereditary diseases and structural problems such as testicular obstruction, damage to the reproductive system leading to sperm dysfunction, and environmental and psychological factors. While, factors related to women are ovulation disorder, abnormal uterus or fallopian tubes, endometritis, primary ovarian failure and pelvic adhesions. According to this research, stem cells under certain conditions can differentiate into germ cells, which can be a way to treat infertility. Conclusion: Researchers worldwide are doing their best to standardize differentiation protocols to achieve safer and more favorable results. However, before we can look forward to their use in humans, much effort will still be required to successfully create fully functional, viable offspring from in vitro differentiated in vitro cells.

Abstract: 

Introduction: Diffuse large B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin's lymphoma (NHL) worldwide, accounting for approximately 22% of newly diagnosed cases of B-cell NHL. Determining the prognosis of patients and choosing the appropriate treatment regimen has been discussed in many studies. In this article, we examine a case of a patient with diffuse large B cell lymphoma with very rare extensive metastases in PET-CT scan, who did not respond to the first line of treatment proposed in the valid guidelines, but the patient responded to the second line of treatment (R regimen-ICE) has been very significant. Conclusion: This case shows that probably in cases where there is extensive involvement in the patient's PET-CT scan and the patient's prognosis is considered poor, IPI, R-ICE regimen can be used first line.

Abstract: 

Introduction: The COVID-19 pandemic posed an unprecedented challenge to healthcare systems. COVID's high mortality rates, lack of intensive care unit (ICU) beds, multiple restrictions, and the vaccination process have affected almost all health policies and aspects of individual life. The high mortality rate of severe acute respiratory syndrome coronavirus 2 (SARS-COV-2) infection in patients with chronic diseases, especially CKD, and concerns about the increasing prevalence of non-communicable diseases during the pandemic have drawn much attention to the association between CKD and COVID-19. One of the most critical efforts to combat CKD is the world kidney day (WKD) campaign. World kidney day is the international society of nephrology (ISN) and the international federation of kidney foundations (IFKF) joint initiative to increase public awareness about the importance of kidneys and related diseases, especially CKD. World kidney day 2023, kidney health for all-Preparing for the unexpected, supporting the vulnerable, aims to raise awareness about the importance of kidneys and CKD. Moreover, this campaign encourages screening, and preventive behaviors, in patients at risk for CKD. Based on limited evidence, the COVID-19 pandemic has increased the risk of CKD risk factors such as obesity, diabetes, hypertension, and higher intake of fat and high-salt foods. On the other hand, reducing patients' access to health care has reduced screening in high-risk patients and reduced primary care for CKD patients.

Abstract: 

Introduction: The present study aims to investigate the immediate discoloration effects of Nd: YAG laser on the surface of sheep iris. In this regard, the iris of a domestic sheep was extracted, and an Nd: YAG dual-frequency laser was applied to the surface of the iris. The laser wavelength setting was 532 nm, energy of 1. 5 mJ, a spot size of 800 microns, and one pulse per burst. At the end of the procedure, the targeted spots were stained without iris penetration, resulting in lighter iris color. When a 20D concave lens was placed between the target and the laser source, the spots became larger in size and scattered the treatment. Results: As a result, this study showed that the Nd: YAG dual-frequency laser could effectively change and brighten the iris, leading to iris discoloration.

Abstract: 

Introduction: Hypothyroidism is one of the most common chronic diseases in pregnant women. It is defined by the level of thyroid hormones is lower than normal. Maternal thyroid hormones have key role for the fetus development. Studies have shown that premature birth, abortion, intrauterine death of the fetus and preeclampsia are among the most important complications that are increased in patients with congenital hypothyroidism. In this study, the files of 300 pregnant women who referred to the health centers of Karaj city were examined. After conducting interviews with participants, the complications and risk factors of hypothyroidism during pregnancy were examined. The results show that the mother's family history of autoimmune diseases, the mother's autoimmune diseases and goiter are among the most important risk factors for the mother's subclinical hypothyroidism, and the mother's type 1 diabetes is the most important risk factor for the mother's clinical hypothyroidism. Also, preeclampsia is the most important complication of both types of hypothyroidism. On the other hand, the risk of IUFD and premature delivery is also higher in patients with clinical hypothyroidism. Conclusion: As a result, with a more detailed examination of mothers with the mentioned risk factors, especially type 1 diabetes, hypothyroidism during pregnancy can be quickly identified and managed.
