Abstract: 

Parthenogenesis (PG) is a rare phenomenon occurring in humans, and understanding this may help us develop an explanation for such occurrences. Moreover, it may help reveal the cause of idiopathic ovarian teratoma (OT). We aim to explain the occurrence of PG and OT in humans based on a new hypothesis. Previous literature has been searched through relevant scientific websites and international journals on the causes and mechanisms of PG and OT in humans. The previous literature on human PG was sparse and mostly contained case reports. It appears that human PG is not as rare as previously reported but may occur spontaneously, resulting in OT formation. The difference between PG and sexual reproduction is that PG has no embryonic diversity. The biopsied embryonic samples in the PG correspond exclusively to those of the maternal side. Spontaneous PG in humans often degrades or leads to formation of OT. The cause and mechanism of spontaneous PG remain unclear in the available literature. Here, we hypothesized that in some cases the secondary oocyte and first polar body enclosed in the zona pellucida may fuse together to form a single cell that restores the diploid number of chromosomes and initiates cell division to form PG. It may go unnoticed or be represented by the OT. Future studies are recommended to investigate this hypothesis.

Abstract: 

Background: Cyclophosphamide (CP) has clinical applications in treating diverse malignancies and autoimmune disorders,at the same time, it also has harmful effects on the body tissues, particularly the genitals. The most significant side effects of CP are changing the reproductive system’, s function and infertility. Objective: This study determines the Ephedra hydroalcoholic extract (EP) role on testicular tissue and the pituitary-gonadal axis in CP-treated male rats. Materials and Methods: In this experimental study, 48 adult Wistar rats were separated into 6 groups (n = 8/each): control, sham, CP recipients, and CP recipients with gavage, fed EP (250, 500, and 1000 mg/kg). On the 29th day, the blood of the weighed animals’,was drawn from their heart, and serum concentrations of follicle-stimulating hormone, luteinizing hormone, and testosterone were measured. After preparing testicular tissue segments, cells were counted. Results: While CP decreased follicle-stimulating hormone, luteinizing hormone, and testosterone levels (p < 0. 05), the use of EP changed them and even reached the control. Serum gonadotropin-releasing hormone increased significantly in all EP groups compared to the control and CP groups. Compared to the control, a significant decrease in total antioxidant capacity and plasma glutathione peroxidase was observed in the CP groups. EP (all doses) significantly increased their concentration compared to the CP group (p < 0. 05),significant reduction in serum total oxidant status and malondialdehyde in CP groups changed by EP (p < 0. 05). Although CP’, s role on spermatogonia counts (57. 5 ±,5. 2 in CP, 67. 1 ±,6. 0 in control), higher doses of EP had no significant effect on this but did affect spermatocyte and spermatid cells count. Conclusion: Due to its antioxidant characteristics, EP mitigated the effects of CP on the investigated parameters in rats

Abstract: 

Background: Scientific evidence support that imbalance between inflammatory and anti-inflammatory cytokines play a critical role in preeclampsia (PE). Objective: To investigate the relationship between the maternal serum level of interleukin (IL)-23, a pro-inflammatory cytokine, PE and its severity risk was investigated. Materials and Methods: The case-control study included a total of 145 women counting 75 PE cases, 35 healthy pregnant and 35 healthy non-pregnant controls from Zahedan, southeast of Iran. The maternal levels of IL-23 in circulation were determined via enzyme-linked immunosorbent assay. Results: The maternal serum levels of IL-23 were increased in PE and its 2 subgroups severe PE and mild PE, so that these increases were significant in PE and severe PE, but not in mild PE compared with the controls (p < 0. 001 and p < 0. 001, p = 0. 08, respectively). Besides, the maternal IL-23 serum level was statically significant in the early onset PE, but not in the late onset-PE group compared to healthy pregnant controls (p < 0. 001, p = 0. 46 respectively). Conclusion: The results of our study showed a positive association between IL-23 level and PE, especially in severe type and early onset PE, which suggests that IL-23 may be involved in the pathogenesis of this systemic syndrome.

Abstract: 

Background: The male reproductive system undergoes several adverse age-related changes like decreased hormone synthesis, sperm count, and testicular alteration that can impact on fertility. Objective: The study aims to investigate the effects of testosterone propionate (TP), and ayurvedic formulation Swarna Bhasma (SB) on D-galactose (D-gal) induced reproductive aging in male Wistar rats. Materials and Methods: 60 male Wistar rats were divided into 10 groups of 6 animals. Reproductive aging was induced by D-gal (150 mg/kg Bwt) exposure for 60 days. The rats were then treated by post and combination treatment with TP (2 mg/kg Bwt) and SB (6. 75 mg/kg Bwt). Then sperm parameters, reproductive hormones, inflammatory markers, testicular antioxidant enzymes, steroidogenic enzymes, and histological manifestation of testis were evaluated. Results: Exposure of D-gal caused significant (p < 0. 001) decrease in serum testosterone (T), testicular steroidogenic, and antioxidant enzymes. Administration of TP increased the serum T level, testicular antioxidant enzymes, and spermatogenic profile at a significant level of (p < 0. 001) compared to D-gal. Further, the SB treatment significantly (p < 0. 001) elevated the serum T level, sperm count, testicular antioxidant enzymes, steroidogenic enzymes, when compared to D-gal. Conclusion: Both the treatment of TP and SB treatments recovered the reproductive impairments caused by D-gal. However, exogenous T supplementation via TP administration is associated with various side effects during long-term use. SB is an Ayurvedic formulation having a long history of usage in India. The current findings suggest that the SB may be used as a good alternative for potentiating reproductive function in aging males.

Abstract: 

Background: Varicocele is an abnormal dilation and enlargement of the scrotal venous pampiniform plexus that impairs normal blood drainage and finally leads to infertility if not treated. Objective: This study aimed to figure out the impact of mitochondria status through the mitochondrial membrane potential (MMP) and adenosine triphosphate (ATP) assessment and its correlation with semen parameters to illuminate the impact of sperm mitochondria healthiness on normal sperm functionality. Materials and Methods: This analytical cross-sectional study was conducted with 100 men including 50 cases in the normozoospermic group (normal) and 50 in an infertile group with the non-varicocelectomy operation (varicocele) referring to Infertility Research and Treatment Center, ACECR Khuzestan, Iran. Routine semen analysis was performed according to World Health Organization guidelines, DNA fragmentation index, the MMP assay, ATP content, and apoptosis were carried out for all samples. Results: The results showed that the concentration, progressive motility, normal morphology, MMP, and ATP contents of sperm in varicocele were significantly lower than the normal group. In addition, the sperm DNA fragmentation index was significantly higher in the varicocele group in comparison with the normal group. Conclusion: Reduction in MMP and ATP contents, besides the loss of sperm parameters quality and increase in sperm DNA fragmentation, were seriously implicating sperm mitochondria dysfunctionality in varicocele men.

Abstract: 

Background: Fewer studies were on micronutrient intake in women with endometriosis, and the etiology of endometriosis remains unclear between dietary micronutrients and the risk of endometriosis. Objective: This study aimed to investigate the relationship between dietary micronutrients and the risk of endometriosis. Materials and Methods: This case-control study was conducted on 156 women (18-45 yr) with and without endometriosis in the gynecology clinic of Arash hospital between May 2017 and May 2018 in Tehran, Iran. According to the laparoscopic findings, the participants were divided into 2 groups (n = 78/each), women with pelvic endometriosis as the case group and women without endometriosis pelvic as the control group. Dietary data were collected using a validated 168-item semiquantitative food frequency questionnaire with the standard serving. A logistic regression model was used to determine the association between micronutrients and the risk of endometriosis. Results: Data analysis showed a significant relationship between micronutrients such as: potassium (OR: 0. 74,CI: 0. 56-0. 99,p = 0. 01), calcium (OR: 0. 70,CI: 0. 52-0. 94,p = 0. 003), and also among the vitamin C (OR: 0. 70,CI: 0. 52-0. 94,p = 0. 02), B2 (OR: 0. 73,CI: 0. 55-0. 98,p = 0. 01), and B12 (OR: 0. 71,CI: 0. 53-0. 95,p = 0. 02) with endometriosis, so those who used fewer micronutrients were at higher risk of endometriosis. Conclusion: The findings showed that the dietary intakes of calcium, potassium, vitamins B12, B2, B6, and C are inversely related to the risk of endometriosis.

Abstract: 

Background: Repeated implantation failure (RIF) affects 15% of women of reproductive age. There is a high endometrial expression of both estrogen receptors and progesterone receptors (PRs) during the window of implantation in women with RIF. Objective: To evaluate the effects of intrauterine administration of human peripheral blood mononuclear cells (PBMC) on estrogen receptor α,(ERα, ) and PRs expression in the endometrium of women with RIF during the implantation window. Materials and Methods: This randomized clinical trial study was conducted on 22 women with RIF history from January 2018 to August 2019 in Erfan hospital, Tehran, Iran. Participants were divided into 2 groups (PBMC-treated group [n = 11] and control group [n = 11]). Endometrial tissue samples were collected at the implantation window time, during the mid-secretory phase (luteinizing hormone surge +7 days) of each menstrual cycle. The quantitative real-time polymerase chain reaction technique was used to measure the mRNA levels of ERα,and PRs isoforms (PR-A and PR, B) in endometrial tissues. Furthermore, the protein expression of ERα,and PRs was investigated using immunohistochemical staining. Results: PBMC treatment significantly decreased the mRNA expression of endometrial ERα,and PRs isoforms at the time of the implantation window (p < 0. 001). Moreover, the endometrial ERα,and PRs protein localization were significantly lower in PBMC-treated women compared with controls (p = 0. 01, and p < 0. 001 respectively). Conclusion: The intrauterine administration of PBMC decreased the endometrial ERα,and PRs expression during the window of implantation in women with RIF. This local response to PBMC therapy could promote endometrial receptivity and embryo implantation.

Abstract: 

Background: Ovarian torsion (adnexal torsion) is a rare event in pediatric patients which is primarily managed by pediatric general surgeons. Case presentation: This study presents a case of ovarian torsion in a 2-yr-old girl with a history of episodic lower abdominal pain, nausea, and vomiting for 2 days. Her physical examination was normal except for mild tenderness in the lower abdomen with no palpable mass. A color Doppler ultrasound was performed for further investigation, and an ovarian torsion was reported without sonographic signs of intussusception and acute appendicitis, so she underwent laparotomy. A relatively complete torsion was observed in the left ovarian pedicle. Initially, the left ovary and fallopian tube had a dark appearance, and 10-15% of the ovarian tissue was still normal. Detorsion of ovary was done and it was decided to preserve the ovary. After about 20 min, the color of ovary and fallopian tube returned to relatively normal, indicating normal blood flow. The patient was discharged 2 days later because a follow-up color Doppler ultrasound showed normal ovarian blood flow. Conclusion: The possibility of ovarian torsion must be considered in all female infants with suspicious abdominal pain.
