Background: Despite the development in maternal care peripartum hysterectomies are increasing. The purpose of this study is the survey incidence rate, indications, demographic features and complications of peripartum hysterectomies in Kermanshah’s hospitals, 2007-2011.Methods: 81 patients in 5 hospitals were studied during the years 2007-2011 in Kermanshah. Postpartum hysterectomy had been done on them during or within 24 hours after delivery. Results: The rate of peripartum hysterectomy was one out of 1000delivery.The mean age of mother was 34.3 years and the mean parity was 2.9.in 70. 4% during cesarean and in 29.6% after vaginal delivery hysterectomy was done. 44 cases (54.3%) placenta adhesion, 19 cases (23.4%) uterine rapture, and 13 cases (13.4%) uterine atony were operated (hysterectomy). Cor pulmonale was the most common complication due to extensive bleeding and transfusion in 39 cases (48.1%). Vesicle trauma in 13 cases and ureteral trauma in 7 cases occurred. Maternal death occurred in two cases. Conclusion: The amount of hysterectomy peripartum was similar to developed countries and lower than neighbor Asian countries. The highest rate was from placenta adhesion and then uterine rapture. Uterine rapture is also considered as a cause of maternal death. Decrease in cesarean without medical indications and paying more attention during vaginal delivery decreases peripartum hysterectomy and maternal death.

Abstract:

Introduction: Pseudomonas aeruginosa and Enterobacter cloacae are the most common organisms involved in clinical disease among patients admitted in ICUs. The treatment of these infections is often difficult due to several virulence factors and drug resistance mechanisms. The aim of this study was to evaluate multidrug-resistance pattern among clinical P. aeruginosa and E. cloacae isolates collected from ICUs.Methods: In total, 82 and 49 P. aeruginosa and E. cloacae isolates were collected from ICUs of Tehran and Qazvin hospitals. The species identification was performed by standard laboratory methods. Antimicrobial susceptibility and multidrug resistance (MDR) pattern were further evaluated by Kirby-Baure method according to CLSI guideline. Results: In this study, the highest resistance rate was shown to cefpodoxime (98.8%) and cefotaxime (97.6%) in P. aeruginosa and to amoxicillin-clavulanic (98%) acid and ampicillin (95.9%) in E. cloacae isolates. Twenty six (53.1%) E. cloacae and 43 (52.4%) P. aeruginosa isolates showed the multidrug resistance pattern. Conclusion: The present study showed the considerable frequency of MDR pattern among P. aeruginosa and E. cloacae isolates collected from studied hospitals. Therefore, there is need for efficient infection control and appropriate therapeutic practices, especially in ICUs.

Abstract:

Introduction: Cryptosporidiosis is caused by various species of protozoan Cryptosporidium. This disease has more importance in children and immunosuppressive individuals. In this study, some of the epidemiologic features of infection with Cryptosporidium in diarrheic and non-diarrheic Children admitted to some hospitals of Kermanshah city were studied from 2011-12.Methods: In this descriptive study, 480 diarrheic and 220 non diarrheic Children in the ages of ≤15 years old were studied. Epidemiological information of Patients was collected using a questionnaire. Stool samples of the patients concentrated by formalin-ethyl acetate concentration technique and after staining with modified Ziehl-Neelsen, were examined for presence of oocysts of Cryptosporidium by light microscope. The intensity of infection in each sample was detected using a semi quantitative method. The data were analyzed using SPSS 21 software. Results: Of the 700 studied children, 15 (2.14%) were found to be infected with Cryptosporidium. Nine of the patients were female and six of them were male. The highest and lowest rate of infection were seen in the age groups of 4-6 (6.25%) and 13-15 (0.83%) years old, respectively There was statistical difference between the rate of cryptosporidial infection in diarrheic and non-diarrheic children. The intensity of infection in most of the children (93.3%) was assessed as 1+. Most of the patients were living in urban areas. Conclusion: Although the overall rate of infection with Cryptosporidium showed a significant decrease compared to previous years, but it seems that this infection especially in diarrheic children under 5 years old must be in consideration.

Abstract:

Introduction: M. genitalium is a member of genital Mycoplasmas which is an important factor for sexually transmitted diseases (STD) in men and women. M. genitalium often causes urethritis, cervicitis, vaginitis and acute endometritis. This study aimed to determine the frequency of M. genitalium infection in women with cervicitis. Methods: In this descriptive- analytic study, 223 women with cervicitis were sampled using cervical swab. Then the bacterial DNA was extracted and PCR was carried out using specific primers for the detection of M. genitalium MgPa operon gene. Results: Of the 223 women who had cervicitis, 11 individuals (4.9%) showed M. genitalium infection and the maximum rate was for the age group 35-26 year old. Of women who had the history of abortions 2 (4.5 %) cases were infected with M. genitalium and in women with the history of infertility no infection was found. Vaginal discharge was found in 178 women and this group had the lowest infection rate. However, the highest infection rate was found in women with dysuria. Couples who used condom showed the least infection rate. Conclusion: A considerable proportion of women with cervicitis had M. genitalium infection, although in compare to some countries, it is a low rate. The infection was more common in the age groups with sexual activity which indicates the necessary of education and counseling to prevent or reduce the rate of infection.
