Introduction: Elderly people experience more resentments, thus they have more need & more things for forgiving associated with self, others and their situation. Professional response to this need is measure it’s objectively. As such, the aim of this study was preparing of Persian Version of Heartland Forgiveness Scale and determination of psychometric properties it in Sample of Iranian elderly. Methods: This study was descriptive- survey. The sample included 324 elderly people (male and female) of 60 years and older from Tehran city which were selected through cluster random sampling method. Measuring instruments were Heartland Forgiveness Scale (HFS), Satisfaction with Life Scale (SWLS), Anger rumination scale (ARS) & Geriatric Depression Scale (GDS). In order to provide evidence of validity was used exploratory factor analysis (EFA), Confirmatory factor analysis (CFA), convergent & divergent validity. Reliability was calculated using internal consistency & Test- retest in 2 weeks’ interval. Results: The results of the exploratory factor analysis by principal component and varimax rotation revealed four factors in Persian version of Heartland Forgiveness Scale (PVHFS), But fourth factor was ignored, because of their items were loaded on more than one factor. Confirmative factor analysis approved the three-factor model. Correlation coefficient between PVHFS & parallel instruments showed enough support for convergent and divergent validity of the scale. Additionally, showed suitable internal consistency & test-retest coefficient. Conclusions: Persian Version of Heartland Forgiveness Scale has appropriate reliability and validity for the measurement of Forgiveness among Iranian elderly & thus can be used in future research.

Introduction: Deficits in social and living skills of chronic mental patients and the unsatisfactoriness of current drug, provide a strong rationale for developing new interventions. Community Re- Entry Program is one of the programs related to social and independent living skills that provides patients with chronic mental disorders with some information and skills. The aim of the present study was to compared symptoms severity and communication and interaction skills of outpatients with severe mental disorders after the Community Re-entry and conventional Psychotherapy programs. Methods: This study was a quasi- experimental design with pre-test, post-test. To this end, 40 patients with severe mental disorders (schizophrenia and mood disorder) referring to daily rehabilitation centers of Hamadan and using simple randomization divided into an Community Re-entry Program (N=20) and a Conventional Psychotherapy Programs (N=20) group. The community rehabilitation program group received in 16 60-90-minute educational sessions held twice a week for two months. The conventional Psychotherapy group were used from psychotherapy and occupational therapy. Both groups were evaluated using Assessment of Communication and Interaction Skills (ACIS) questionnaire and positive and negative symptom scales (PANSS) before and after the intervention. To analyze of Mean difference in the two groups, were used from U Mann- Witny test. Results: The results showed that the two groups were the same in terms of age, marital status and employment levels. Patients who received Community Re-entry Program showed a significant change in information exchange (P=0.001), relations (P=0.0001) and total score of ACIS questionnaire (P=0.0001) and in the positive symptoms (P=0.01) of PANSS questionnaire compared to the conventional psychotherapy group. Conclusions: The results showed that Community Re-Entry Program in compared with conventional psychotherapy daily centers played an important role in improving communication and interaction skills among patients with severe mental disorders in the study population. Yet, future studies are required to assess the long-term clinical effects of this program.

Introduction: Knee osteoarthritis, owing to its progressive route, is often debilitating and affects the elderly’s life. We should be self- efficient to manage the disease problems. This study was conducted to investigate the effect of self-management training on the self- efficacy of elderly patients with knee osteoarthritis. Methods: A quasi- experimental study was conducted on 88 elderly with knee osteoarthritis referred to the orthopedic clinic of Khoramabad City, Iran, in 2014. The subjects were allocated into the intervention and control groups based on the random blocks. First, the patients’ self- efficacy level was measured using the rheumatoid arthritis self- efficacy (RASE) questionnaire. Afterwards, the intervention group was trained for four weeks, one 70-minute session per week. After the training, the intervention group was given four weeks to implement the training program at home. Follow-up assessment after the intervention was conducted via telephone. Finally, the changes of the self- efficacy levels were evaluated and compared. The collected data were analyzed using the chi-square, independent t test and ANCOVA with the SPSS software version 18.Results: The two groups showed no significant difference in terms of background variables. Also, the two groups were similar in terms of self- efficacy before the intervention. Moreover, the results showed a significant improvement in the mean of self-efficacy before (84.2±7.28) and after (94.08±10.08) the intervention in the intervention group (P<0.05). However, the difference between the mean values of self-efficacy before (84.8±6.44) and after (83.7±8.68) the intervention was not significant in the control group (P>0.05).Conclusions: The results show that implementation of self-management training is effective in improving the self- efficacy of the elderly with knee osteoarthritis. Educations about pain control techniques, physical exercises, and diet can reduce knee pain in the subjects and improve their self-efficacy.

Abstract:

Introduction: Multiple sclerosis (MS) is a chronic and progressive disease of the central nervous system that causes sensory impairment. The aim of this study was to evaluate and compare the proprioception of the knee joint and ankle of dominant and no-dominant extremities in MS patients with an emphasis on sex. Methods: In this descriptive comparative study, 60 patients with MS were selected from the Iran MS Society in Tehran using the convenience sampling method and divided into the two groups (males and females). The joint position sense was measured by a digital electrogoniometer in three different angles in both right and left knees and ankles. Data were analyzed using the paired t test and independent t test with the SPSS software version 16.Results: According to the results of this study, the joint position sense in dominant extremities of the knee showed a significant difference between the two groups only in 25 extension (P=0.01). Also, the results showed that there was a significant difference between the two groups in 10-degree ankle dorsiflexion and 20 plantar flexion (P=0.01), but no significant difference was observed in the non-dominant extremities. Conclusions: The results suggest that the MS patients suffer from disorders of proprioception, especially in the ankle joint. Thus, paying attention to this joint is recommended for rehabilitation of these patients.

Introduction: Falls are one of the most frequent and serious problems facing the elderly. It has negative impacts on physical, psychosocial and financial situations of elderly people, and can reduce their quality of life. The aim of the present study was to investigate the effect of balance exercises on the risk of falling among elderly women with a chronic schizophrenia diagnosis. Methods: In this randomized controlled trial study, 60 elderly women were selected from the Razi Psychiatric Hospital by convenience sampling and assigned into the control and experimental groups (N=30 for each group). Demographic questionnaire was completed for all the participants. The Timed up and Go (TUG) test was used to measure the risk of falling before and after the intervention. The intervention group attended a 12-week training program. The sessions were conducted three times per week (30 minutes each session) by supervision of an occupational therapist and researcher. The control group did not receive any intervention. Data were analyzed using independent and paired t tests with the SPSS 19 software. Results: Results showed that a 12-week simple balance training program increased balance ability and reduced the falling risk significantly among the elderly women (P<0.05). Before the beginning of the intervention, there was no significant difference between the control and intervention groups regarding demographic characteristics, history of fall, and duration of the TUG test (P>0.05). However, the results showed significant improvement in completing the TUG test in the intervention group compared with the control group (P<0.05).Conclusions: It can be concluded that using simple balance exercises can result in reduction of the falling risk among elderly women. Considering the low cost and simplicity of the intervention, it is suggested to use this training along with other appropriate interventions to reduce the falling risk among elderly people.

Abstract:

Introduction: Drug adherence is poor among patients with bipolar I disorder (BID) and lack of adherence to medication is one of the frequent causes of relapse and readmission in these patients. This study aimed to examine the effect of a community re-entry rehabilitation program on drug adherence in patients with BID.Methods: This randomized clinical trial study was conducted on all patients with BID admitted to Iran Psychiatric Haspital in the first 6 months of 2016. Forty patients were selected by purposive sampling and randomly divided into two groups of intervention and control. The intervention group received the theoretical and practical training of the community re-entry program (CRP) over 16 sessions (three times a week). Pateints in the control group only received current treatment at the center (drug therapy, occupational therapy, and psychotherapy). The patients completed medication adherence rating scale (MARS) before and threre months after the intervention. Data were analyzed using chi- square test, independent t-test and ANOVA with the SPSS 19 software. Results: The results showed no significant difference between the intervention and control groups regarding the distribution of demographic charactristics. The mean score of patients’ medication adherence before the intervention was 4.05±1.05 in the intervention group and 4.35±1.59 in the control group, while after the intervention it was 8.31±0.92 in the intervention group and 4.65±1.56 in the control group, which showed a significant difference in the intervention group (P£0.001).Conclusions: Results showed that implementing CRP can enhance drug adherence in patients with BID.
