Abstract: 

Introduction: Post operative pain causes complications and dissatisfaction in patients. The aim of this study was to compare the perceprion of pain from the viewpoint of nurses, patients and surgeons and its relationship with analgesics at post open prostatectomy. Methods: Perceprion of pain from the viewpoint of the nurses, doctors and patients was assessed by questionnaire. Validity and reliability has been approved by Faculty of Isfahan University of Medical Sciences. Pain intensity is measured by Visual analogue scale (VAS). The type and amount of analgesics were recorded by the cheek List. Results: Results showed that most of physicians (72. 2%), nurses (72. 3%) and patients (90%), have moderate understanding to pain. The majority of nurses (53. 3%) rated pain as moderate, while the majority of patients (83. 3%) rated pain as severe. The majority of patients (76. 7%) received Pethidine. Chi-square showed a significant difference between the Patients' perception of pain and intensity of pain (P ≤ 0. 05) and significant difference between Nurses' perception of pain and intensity of pain (P ≤ 0. 05). Chi square showed significant relationship between patients' perception of pain with analgesic but did not show any significant relationship between nurse s' perception of pain with analgesic and the doctor s' perception of pain with analgesic (P ≤ 05. 0). Conclusions: Postoperative pain control, reduction of pain and patient satisfaction were considered. The findings showed that nurse s’ and doctor s' perception of pain was moderate. Since most doctors and nurses haven’ t studies about pain, it is recommended that training should be given to them.

Abstract: 

medical problems in older men with high rate of morbidity. Urinary retention and bladder stone are two important complications of Benign Prostate Hyperplasia. The aim of this study was to evalute these complications in patients with Benign Prostate Hyperplasia who referred to outpatient urology department of Besat hospital of Hamadan. Materials and methods: In a descriptive cross-sectional study, for clinical diagnosis and ultrasonography, 105 patients with Benign Prostate Hyperplasia were enrolled to study. Results: The mean age of patients was 10. 8± 69. 5. The prevalence of urinary retention and bladder stone was %44. 8 and %14. 3, respectively. Urinary retention significantly increased by age (p<0. 05). The mean prostate volume was 24± 67. 9 cc. The decrease in pressure and caliber of urine with %78. 1 and dribbling with %54. 3 were the most common obstruction symptoms and irritant symptom was the most frequent (%82. 9). Conclusion: Benign Prostate Hyperplasia symptoms are increased with age. In addition, the high frequency of urinary retention and bladder stone in patients showed that they referred when the diseases worsened and consequently the complications of disease increased.

Abstract: 

Introduction: Urinary incontinence is any urine leakage without control from bladder that is classified into persistent incontinence, urge incontinence, overflow incontinence and stress incontinence. Stress incontinence is more prevalent than another types of incontinence and results in economy expenses and social costs. . In this study, we compared the transurethral platelet rich plasma combined with autologous fat injection versus autologous fat injection on abdominal leak point pressure in patients with stress urinary incontinence in Hamadan’ s Beheshti Hospital. Materials and Methods: In this clinical trial study, we studied female patients with stress urinary incontinence in Hamadan’ s Beheshti hospital from September 2012 to March 2012. Platelet rich plasma combined with autologous fat were injected into the patient’ s bladder neck as a bulking agent. Data were analyzed using SPSS 16. At the end, all analyses were compared at 95 percent level of confidence. Paired t-test statistical analysis was used. Significant level was considered less than 0. 05. Results: The interior – posterior thickness of injected fat tissue along with PRP in MRI and also the level of ALPP increased significantly and the symptoms in patients improved based on questionnaires data. Conclusions: With regard to different aspects of our study such as the intensity of urinary incontinence in accordance with the questionnaire, changes of thickness in fat tissue using MRI in Sagittal sections, changes in ALPP using Urodynamic and complications resulting from the treatment, the obtained results were promising and its seems that this is one of the treatment options recommended to the patients. Its seems that PRP+FAT may be used as factors to treat females with stress urinary incontinence.

Abstract: 

Introduction: Gender and sexual satisfaction are important aspects of female health and well-being. Reviews from the primary medical care suggest that less than %50 of physicians ask questions about sexual behavior and sexual disorder from their patients. A true understanding of normal responses related to sexual relationship, evaluation and treatment of sexual dysfunctions requires providing General Gynecological. The main aim of this study was to evaluate the prevalence of FSD (desire, arousal, orgasm, and satisfaction and pain problems) in female who referred to Hamadan Fatemiye Hospital. Methods: This cross sectional study consisted of 134 sexual active women between the ages of 16 and 53 years who referred to Fatemiye Hospital in 1385. We collected and recorded data in IV_FSFI questionnaire and analyzed them with SPSS16. Results: the frequency of sexual dysfunction was detected in details as follows: frequency of Sexual desire disorder in %83. 6 of women, arousal problem in %88. 8, orgasm problem in %79. 1, satisfaction problem in %51. 5, and pain problem in %58. 2. Conclusions: On the basis of this study, it seems that the aspects of female sexual dysfunction are common in our country.

Abstract: 

Introduction: Prostatic specific antigen (PSA) is an excellent tumor marker, but it is not specific for prostate cancer. We evaluated the accuracy of PSA adjusted for transition zone volume calculated by transrectal ultrasonography in predicting prostate cancer in men compared with PSA density. Material and Methods: PSA adjusted for transition zone volume was obtained from 100 patients who underwent transrectal ultrasonographically guided biopsies (50 patients with prostate cancer and 50 patients with BPH). PSA density was calculated by through dividing total serum PSA by total prostate volume, and total serum PSA was divided by transition zone volume to yield PSA adjusted for transition zone volume. The PSA density and PSA adjusted for transition zone volume were compared and analyzed in patients with cancer of prostate and patient with BPH. Results: 50 fifty patients with BPH, 50 patients with prostate cancer were evaluated. Mean PSA adjusted for transition zone volume and PSA density were 0. 21 ± 1. 17 and 0. 17 ± 0. 91 in patients with prostate cancer and 0. 14 ± 0. 65 and 0. 064 ± 0. 34 in patients with BPH. PSA adjusted for transition zone volume had sensitivity of %26. 6 and specificity of %86. 6 and PSA density had sensitivity of %30 and specificity of %93. 3 for prostate cancer. Conclusions: PSA adjusted for transitional zone volume is not more accurate than PSA density in for distinguishing prostate cancer from BPH. Determination of transitional zone volume and total prostate volume by transrectal ultrasonography may be helpful for predicting the probability of positive prostate biopsy results.

Abstract: 

Hemangiomas of the glans penis are very rare lesions. Only a few cases of glans penis hemangiomas have been reported. Different treatments for hemangiomas of the glans penis have been described such as Nd: YAG laser ablation, sclerotherapy or surgical excision. A case of hemangioma of the glans penis was diagnosed in a-26year-old man. After its evaluation with color Doppler, it was treated with sclerotherapy using %3 sodium tetradecyl sulfate with satisfactory result. The case is rare and different treatment methods such as using sodium tetradecyl sulfate were described.
