Abstract: 

Combat medicine, as one of the key and strategic specialties in military medical sciences, stands at the intersection of theoretical knowledge and field-oriented pragmatism. This field, which plays an unparalleled role in maintaining and enhancing the physical and mental health of armed forces personnel, faces several challenges. One of the most fundamental challenges is the lack of alignment between existing training programs in this discipline and academic standards. This misalignment not only affects the quality of education but also hampers the ability of medical personnel to effectively respond to complex and critical situations.

Abstract: 

Introduction: Dietary approach to stop hypertension (DASH) has become a diet for management of many chronic physical and mental diseases. The aim of this study is to investigate the effect of DASH diet on sleep quality and daytime sleepiness. Methods: This study is a randomized, controlled clinical trial. The participants included 563 students and their families. Participants were randomly assigned to one of the DASH or standard diet groups. Pittsburgh and Epworth questionnaires were used to evaluate sleep quality and sleepiness score, respectively. Results: At the end of the study, 240 patients in the intervention group and 195 patients in the control group completed the study. The sleep quality score was reduced after the intervention in the control group only in the students group. In the comparison of changes between groups, after adjusting for confounding variables, the sleep quality score in the DASH diet intervention group was significantly improved compared to the control group. In the intra-group analysis, the daytime sleepiness score was significantly improved in the DASH diet intervention group. For daytime sleepiness score, there was no significant difference between the group receiving DASH diet and the control group. Conclusion: Following the DASH diet improved the total sleep quality and daytime sleepiness score, However, more research is needed to determine the effect of healthy diets on sleep disorders.

Abstract: 

Introduction: One of the complications of surgery is changes in the patient's temperature, which is caused by multiple factors and exacerbates the occurrence of other complications. This study investigated the effect of two methods of anesthesia, general anesthesia and spinal anesthesia on body temperature and shivering intensity in patients undergoing surgery was investigated. Methods: In this study, 60 patients undergoing orthopedic, neurological, general, and urological surgeries were randomly divided into two groups of 30 each. In group A, patients underwent general anesthesia and in group B, patients underwent spinal anesthesia. The groups were compared in terms of operating room temperature, patient body temperature during surgery, and shivering intensity. Data correlation was performed using Spearman's criterion, and data were compared using the T-test. Results: There was no significant difference between the study subjects in terms of demographic characteristics (P>0. 05). Operating room temperature, patient body temperature, and surgery duration did not show a significant difference between the groups, but there was a correlation between operating room temperature and patient body temperature. There was also an inverse correlation between patient body temperature and shivering intensity and surgery duration. The two groups showed a significant difference in terms of shivering intensity. Conclusion: The results of this study showed that the choice of anesthesia type, the physical condition of the patient and operating room play an effective role in temperature changes and the occurrence of shivering, which the surgical team should consider preventing surgical complications.

Abstract: 

Introduction: Meniere's disease is associated with many emotional symptoms and the lifestyle of patients can play a role in the disease. Therefore, this study was conducted with the aim of investigating the effectiveness of schema therapy on emotional symptoms, experiential avoidance, and lifestyle of patients with Meniere's. Method: This study was conducted in a semi-experimental way with a pre-test, post-test and follow-up design. The statistical population of the present study included all patients with Meniere's who referred to the Ear Specialist Polyclinic under the supervision of Mashhad Medical Sciences Hospital during the research period. Thirthy people who had the conditions to participate in the study were selected from among the referents by available sampling method. Using a random method, the participants were replaced in two groups of 15 people, schema therapy and control. All three groups were evaluated before, after the intervention and one month after the intervention with Smith's lifestyle questionnaires, Hayes' experiential avoidance, and the reduction of DASS21 emotional symptoms. Findings: The results showed a significant decrease in depression, anxiety and stress of Meniere's patients in the experimental group compared to the control in the post-test and follow-up (P<0. 05). In addition, a significant difference in lifestyle was shown between the treatment schema group and the control group (P<0. 05). The difference between the two groups in experiential avoidance was significant (P>0. 05). Conclusion: Therefore, the results showed that schema therapy is effective on anxiety, depression, stress, experiential avoidance and style of Meniere's patients

Abstract: 

Introduction: unpredictable event, that results in death, injury, serious physical or mental danger. Purpose: This article investigated the factors affecting unpredictable events in the emergency department of Baqiyatullah Hospital in Tehran to find some solutions for decision making. Method: In this qualitative study with the content analysis approach, 20 hospital health care personnel from the fields of medicine and nursing participated. Purposeful random sampling and semi-structured interviews were used to collect data. Atlas. ti version 5. 2 software was used for data analysis. Findings: Four main themes and 32 sub-themes were identified with multiple revisions and combinations of codes. Unpredictable events were: causes, barriers to occurrence, solutions to prevent causes, and solutions to improve barriers. tthe main factors were related to these topics: staff and patients, education, communication, evaluation, , leadership, continuing care, human factors, physical environment, Information management and drug use. Originality/value: This study validates strategies to reduce unpredictable events in emergency situations.

Abstract: 

Introduction: For brain-compatible working environments, the study conducted a specific type of leadership aimed at designing a neuroscience-based command model in the Islamic Republic's Army. Working method: This study was conducted in terms of development and applied, and in terms of strategy of qualitative research with Delphi methods and implications. The statistical population, including military command experts, was identified by targeted sampling and data collection from the interview tool for the Delphi Method section and the theoretical sampling for the signing method section. Conclusion: Based on data analysis and inferential analysis results, command dimensions include discipline, interaction, self-esteem and authority, and based on the share of brain chemistry and brain structure, neurological implications of discipline, interaction, self-esteem, and ultimately authority became.

Abstract: 

Introduction and purpose: Applying efficient management of hospitals and healthcare centers during a crisis has a significant effect on the optimal performance of these institutions. Therefore, the purpose of this study is to clarify the concept of hospital crisis management: concept analysis with Rogers approach. . Materials and methods: In this study, Rogers' evolutionary approach was used to clarify the concept of hospital crisis management. For this purpose, Web of Sciences, Science direct, Proquest, Scopus, Pubmed, Magiran, Iran Medex and SID databases were searched using the keywords. Finally, after reviewing 11 articles, they were selected for study. Results: In this study, the characteristics of the concept of hospital crisis management were identified in the dimensions of hospital management readiness, national management, infrastructure and resource management. Antecedents include strengths, weaknesses, numerous opportunities and threats, sufficient knowledge of managers, mobilization of organizations, provision of physical and human resources, existence of general and special plans, existence of processes, preparation plans, political commitment, communication And the suffixes also include such things as designing and implementing programs and policies, necessary support and training, developing methods to strengthen management and acquiring resources, establishing monitoring programs, approaches based on multi-departmental and people-centered systems, operational formulation of hospital programs, guidelines and accreditation standards to increase readiness Discussion and conclusion: The concept of hospital crisis management has a multifaceted, dynamic and permanent nature. Prioritizing needs and determining short-term, medium-term and long-term goals and developing necessary plans for preparation and response are considered essential requirements of hospital crisis management.

Abstract: 

Introduction: Clinical care changes in emergency departments have occurred due to technological advancements and emerging crises, and nurses are at the forefront of these changes, facing numerous challenges. Therefore, this study aimed to identify emerging challenges and provide innovative solutions to improve the performance of emergency nurses in crisis management. Method: The study was conducted as a scoping review based on the Joanna Briggs Institute guidelines and structured around three axes: population (emergency nurses), concept (emerging challenges and innovative solutions to improve performance), and context (crisis management). The study's design was based on the Omeli, Askay framework, considering four main reasons for the study: 1) scope, 2) necessity, 3) dissemination of results, and 4) research gap. A comprehensive search was performed in Embase, Scopus, CINHAL, Web of Science, Cochrane, PubMed, ProQuest, and Google Scholar databases using MeSH keywords from 2014 to 2024, resulting in 60 articles. After applying inclusion and exclusion criteria, 32 articles were selected. Findings: A comprehensive analysis of dynamic changes, emerging trends, challenges, and the impact of new technologies on the role of emergency nurses led to the identification of innovative and operational solutions to improve their performance in crisis management. Conclusion: Addressing emerging challenges requires innovative solutions, such as advanced training and learning from new global crises, to enable nurses to master crisis situations. By leveraging new technologies, such as artificial intelligence, wearable health devices, and remote nursing systems, nurses' resilience and adaptability can be increased, allowing them to play a key role in crisis management in emergency departments.
